Abstract
Bu çalışma, Türk genel sağlık sigortası uygulamasının, ILO´nun sosyal koruma tabanları yaklaşımı çerçevesinde belirlediği temel sağlık hizmetlerine uygunluğunu değerlendirmeyi amaçlamaktadır. Belirlenen amaç doğrultusunda, çalışma kapsamında iki temel soruya cevap aranmaktadır. "Türk sosyal güvenlik sisteminin dönüşüm süreci, sosyal güvenlik sistemlerinin evrensel dönüşüm süreciyle benzer özellikler göstermekte midir?" sorusu çalışmanın iki temel sorusundan ilkidir. Bu soru kapsamında; öncelikle sosyal güvenlik sistemlerinin evrensel dönüşüm sürecine, ardından Türk sosyal güvenlik sisteminin dönüşüm sürecine yer verilmiştir. Çalışmanın ilk temel sorusuna cevap bulabilmek için "Sosyal güvenlik sistemlerinde dönüşüm nasıl gerçekleşti?", "Sosyal güvenlik sistemlerini dönüşüme yönlendiren sebepler nelerdir?" ve "Sosyal güvenliğin dönüşümü kapsamında gerçekleştirilen düzenlemeler nelerdir?" alt soruları oluşturulmuştur. Çalışmanın ikinci sorusu ise, "Türk sosyal güvenlik sisteminin dönüşüm sürecinin son ürünü olan genel sağlık sigortası, evrensel dönüşüm sürecinin son ürünü olan sosyal koruma tabanları kapsamında ortaya atılan temel sağlık hizmetleri ile uyumlu mudur?" sorusudur. Belirlenen ikinci sorunun cevabına ulaşmak için ise öncelikle, "Temel sağlık hizmetlerinin kapsama düzeyi nedir?", "Temel sağlık hizmetlerinin kapsamını etkileyen unsurlar nelerdir?" soruları cevaplanmıştır. Ardından, "Türk genel sağlık sigortası; kapsam, erişilebilirlik, kalite ve finansman bakımından yeterli midir?" sorusu ortaya atılmıştır. Bu soru kapsamında, ulusal ve uluslararası kaynaklardan elde edilen verilerle değerlendirmeler yapılmıştır. Literatür taraması yapılarak tamamlanan çalışma kapsamında, Türk genel sağlık sigortası uygulamasının kapsam, erişilebilirlik, sunulan hizmetlerin yeterliliği ve finansman bakımından, ILO´nun temel sağlık hizmetleri standartlarına önemli ölçüde uygun olduğu ancak uygulamanın geliştirilmesi gereken yönleri olduğu tespit edilmiştir.
This study aims to evaluate the suitability of the Turkish general health insurance practice with the basic health services determined by the ILO within the framework of the social protection bases approach. In line with the determined purpose, answers are sought to two basic questions within the scope of the study. The question "Does the transformation process of the Turkish social security system have similar characteristics to the universal transformation process of social security systems?" is the first of the two basic questions of the study. Within the scope of this question; Firstly, the universal transformation process of social security systems and then the transformation process of the Turkish social security system are included. In order to find an answer to the first basic question of the study, "How did the transformation occur in social security systems?", "What are the reasons that led to the transformation of social security systems?" and "What are the regulations carried out within the scope of the transformation of social security?" sub-questions were created. The second question of the study is, "Is general health insurance, which is the last product of the transformation process of the Turkish social security system, compatible with the basic health services put forward within the scope of social protection bases, which is the last product of the universal transformation process?" is the question. To reach the answer to the second question, firstly, "What is the coverage level of basic health services?", "What are the factors affecting the scope of basic health services?" questions have been answered. Then, "Turkish universal health insurance; Is it sufficient in terms of scope, accessibility, quality and financing?" The question was posed. Within the scope of this question, evaluations were made with data obtained from national and international sources. Within the scope of the study completed by literature review, it was determined that the Turkish general health insurance application is significantly in line with the ILO's basic health services standards in terms of scope, accessibility, adequacy of the services provided and financing, but there are aspects of the application that need to be improved.
