However, the concomitance of urethral duplication and double megalourethra has not been reported previously. Case Report: A newborn was presented with penile swelling during voiding. Physical examination revealed a retractable foreskin and two external meatus of a double urethra. Retrograde urethrography demonstrated two complete megalourethras. Urethro-urethrostomy and urethroplasty were performed when the patient was 10 months old. The patient was followed up for one year without any urinary problems and has good cosmetics and urinary continence. Based on pre-existing anatomical studies, an axial bilobed flap based on the median and medial superficial sural arteries was developed and used in one patient for the reconstruction of a contracture of the popliteal area. MRI was reported as showing bilateral decreased signal on the T1 image and increased signal on the T2 image in both lentiform and caudate nuclei as well as a minimally increased signal on T1 image in the cerebellar white matter consistent with hypoxic-ischemic changes. F-18 FDG PET/CT scan of the brain demonstrated absent FDG accumulation in both putamens. There was also diminished FDG uptake in both caudate nuclei and the cerebellum. The CT scan demonstrated hypodense areas representing encephalomalacic changes both in the putamens as well as in the caudate nuclei. Although all age groups may be affected, the lesions are seen mostly in patients between the ages of 11 and 40 years old.3 They often present with repeated bleeding that is refractory to pressure. A pyogenic granuloma rarely heals without medical ministrations and is usually necrotic. Histologically, proliferating vascular cells and a variable number of inflammatory cells are seen in an edematous stroma.4 There are various conservative treatment methods, such as laser surgery, chemical cauterization, sclerotherapy, and cryotherapy, but the recurrence rate is high. The triggering factors may be various drugs, food, environmental exposure, and intracoronary stents (2–4). In this case report, an extraordinary case of Kounis syndrome (acute myocardial infarction with concomitant allergic reaction to paracetamol/propyphenazone) has been presented.
