Laparoskopik kolesistektomi uygulanan olgularda, deksketoprofen trometamol ile deksketoprofen trometamol-gabapentin kombinasyonunun postoperatif analjezi üzerine etkilerinin karşılaştırılması

| dc.description.abstract | Preemptif analjezi, nosiseptif afferent ağrı yollarının periferik ve santral sinir sisteminde bloke edilmesi ile santral sensitisizasyon ve postoperatif ağrı oluşumunu engellemeyi amaçlar. Çalışmamızda, elektif laparoskopik kolesistektomi operasyonu planlanan olgularda, operasyondan önce peroral uygulanan deksketoprofen trometamol ile deksketoprofen trometamol-gabapentin kombinasyonunun postoperatif analjezi üzerine etkilerini karşılaştırmayı amaçladık.Etik kurul onayı ve hastaların yazılı onamları alındıktan sonra, randomize olarak planlanan çalışmaya ASA I-II grubu, 18-75 yaş aralığında elektif laparoskopik kolesistektomi planlanan 50 olgu dahil edilerek iki gruba ayrıldı. Operasyondan 1 saat önce, grup D (Deksketoprofen trometamol, n=25) olgularına peroral 25 mg deksketoprofen trometamol tablet ve Grup D-G (Deksketoprofen trometamol ve Gabapentin, n=25) olgularına peroral 25 mg deksketoprofen trometamol tablete ek olarak 800 mg gabapentin tablet uygulandı. İntraoperatif dönemde hemodinamik parametreler, postoperatif dönemde ise ilk analjezik gereksinim zamanı, ek analjezik gereksinimleri, ağrı ve sedasyon skorları, morfin tüketim miktarları, yan etkiler ve hasta memnuniyeti takip edilerek kayıt edildi. Demografik veriler, anestezi ve cerrahi süreleri, intraoperatif hemodinamik parametreler, postoperatif sedasyon skorları ve hasta memnuniyeti her iki grupta benzer bulundu. Postoperatif dönemde tüm değerlendirme zamanlarında, HKA ile iv morfin tüketimlerinin Grup D-G' de Grup D' ye göre anlamlı olarak daha düşük olduğu saptandı. (Derlenme, p=0.021; Postoperatif 2.saat, p=0.007; Postoperatif 4.saat, p=0.001;Postoperatif 8.saat, p<0,001; Postoperatif 12.saat, p<0,001; Postoperatif 18.saat,p<0,001; Postoperatif24.saat, p<0,001). Postoperatif dönemde, ilk analjezik gereksinim zamanının Grup D-G' de Grup D' ye göre anlamlı olarak daha uzun olduğu, ek analjezik gereksiniminin Grup D-G' de anlamlı olarak daha az olguda olduğu saptandı (p<0.001, p<0.001).Preemptif 25 mg deksketoprofen trometamol - 800 mg gabapentin kombinasyonu ile yalnızca 25 mg deksketoprofen trometamol uygulanan olguları karşılaştırdığımızda, deksketoprofen trometamol-gabapentin grubunda postoperatif ağrı skorları ve morfin tüketimlerinin daha düşük olduğunu, ilk analjezik gereksinim zamanlarının daha uzun olduğunu, ek analjezik gereksinimlerinin daha az olduğunu saptadık. deksketoprofen trometamol-gabapentin kombinasyonunun, deksketoprofen trometamole göre daha etkin bir postoperatif analjezi sağladığı ve bu kombinasyonu uygulamanın anksiyete, morbidite, hastanede yatış süresi ve maliyeti de azaltacağından dolayı daha akılcı bir yaklaşım olacağı sonucuna varılmıştır. | tr_TR |
| dc.description.abstract | The purpose of preemptive analgesia is blockage of nociceptive afferent pain pathways in peripheral and central nervous system and preventing the establishment of central sensitization and postoperative pain. The aim of our study was to investigate the effects of preoperative administered peroral Dexketoprofen trometamol and Dexketoprofen trometamol -gabapentine combination on postoperative pain scores.After approval from ethical comittie, randomized study, ASA I-II, aged 18-75 years who were undergo elective laparoscopic cholecystectomy 50 cases were enrolled and divided into two groups. In group Dexketoprofen trometamol (n=25), 25 mg Dexketoprofen trometamol were given 1 hour prior to surgical incision, in group Dexketoprofen trometamol -gabapentine (n=25) 800 mg gabapentine added with 25 mg Dexketoprofen trometamol. Intraoperative hemodynamic parameters and postoperative first analgesic requirement, additional analgesic requirement, pain and sedation scores, morphine consumption, adverse effects and patient satisfaction scores were recorded.There were no significant differences in demographic data, duration of anesthesia and surgery, intraoperative hemodynamic parameters, postoperative sedation scores, and patient satisfaction scores between the groups. Total morphine consumptionin all postoperative times was lower in group Dexketoprofen trometamol -gabapentine when compaired with other group (PACU, p=0.021; postoperative 2. hour, p=0.007; Postoperative 4. hour, p=0.001; postoperative 8-12-18 and 24. hour, p<0.001). Also first analgesic requirement time was higher in group Dexketoprofen trometamol-gabapentine (p<0.001) and postoperative additional analgesic requirement was higher in group Dexketoprofen trometamol (p<0.001).When we compare preemptive administration of 25 mg Dexketoprofen trometamol - 800 mg gabapentine combination with 25 mg Dexketoprofen trometamol; we found that the administration of Dexketoprofen trometamol -gabapentine provides effective analgesia with decreasing postoperative opioid consumption, prolonged first analgesic requirement and additional analgesic requirement was also lower. Dexketoprofen trometamol -gabapentine combination can provide effective postoperative analgesia, than Dexketoprofen trometamol alone and also combination can provide less anxiety, morbidity, hospital stay this reduce the hospital costs. | en_US |
