Abstract
Ortalama penis uzunluğunun etnik kökene göre değerlendirilmesi ve güncellenmesi son yıllarda yapılan çalışmalar ile ortaya konmuştur. Bu çalışmada 0–6 yaş grubu Türk çocuklarında kullanılabilecek gerdirilmiş penis uzunluğu ortalama değerlerinin saptanması ve penis uzunluğu/boy arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Uludağ Üniversitesi Tıp Fakültesi Sağlam Çocuk Polikliniği ve Dörtçelik Çocuk Hastanesine 1 Şubat 2010 ile 31 Ocak 2011 tarihleri arasında başvuran, sağlıklı 0–6 yaş grubundaki 948 sünnetsiz, erkek çocuk kesitsel olarak çalışmaya alındı Olgular yaşlarına göre; yenidoğan (0–0.9) ay (n:101), 1–3 ay (n:96), 3.1–6 ay (n:111), 6.1–12 ay (n:102), 12.1–24 ay (n: 127), 24.1–36 ay (n:102), 36.1–48 ay (n:106), 48.1–60 ay (n:101). 60,1–72 ay (n: 102) olarak gruplandırıldı. Prematüritelik, kronik hastalık ve mikrogenitalya nedeni olabilecek spesifik bir sendrom ya da endokrinolojik anormalliği olanlar çalışma dışı bırakıldı. Çalışmaya alınan çocukların penis uzunluğu, boy, kilo gibi antropometrik ölçümleri alındı. Penis uzunluğu ölçümü, gerdirilmiş pozisyonda ucu ramus pubise dayanan cetvel yardımı ile ölçüldü. Penis gerdirilmiş durumda iken ramus pubis ile glans penis arası mesafe cm olarak kaydedildi. Persentil eğrileri LMS Chart Maker Pro Version 2.3 kullanılarak oluşturuldu. Penis uzunlukları ve penis uzunluğu/boy oranı tabloları ve persentil eğrileri oluşturuldu. Çocukların ortalama penis uzunlukları ve penis uzunluğu/boy oranı ort±SS ve median, minimum-maksimum değerler olarak verildi. Mikropenis gerdirilmiş penis uzunluğunun yaşa göre ortalama değerden -2 standart sapma daha küçük olması olarak tanımlandı. Çalışmamızda 101 term yenidoğanda ortalama gerdirilmiş penis uzunluğu 2,76±0,42 olarak saptandı. Yenidoğanda mikropenis için sınır değer -2 standart sapmaya göre 1,92 cm olarak belirlendi. Penis uzunluğu ile boy arasında anlamlı bir ilişki mevcuttu. Yenidoğanlarda ortalama penis uzunluğu/boy oranı 0.053±0,008 idi. Bu oran daha büyük çocuklarda 0.042±0.007 ile 0.052±0.008 arasında değişmekteydi. Bu çalışma 0–6 yaş grubu Türk çocuklarının penis uzunlukları için yeni referans değerler ve persentil eğrileri oluşturmamızı sağladı. Ayrıca belirlenen penis uzunluğu/boy oranları bu yaş grubundaki çocuklarda penis uzunluğunun değerlendirilmesine katkıda bulunabilir.
The evaluation and update of mean penile length in children according to ethnic background has recently been performed in various studies. The aim of this study was to determine the mean stretched penile lengths in Turkish children aged 0 to 6 years and to evaluate the relationship between stretched penile length and height.A total of 948 boys without circumcision admitted to Healthy Children Outpatient Follow-up Clinic at Uludag University Faculty of Medicine Hospital and Dörtçelik Children's Hospital between February 1, 2010 and January 31, 2011 were cross-sectionally included. The subjects were grouped according to their ages as follows: newborn (0-0.9 months, n=101), 1-3 months (n=96), 3.1-6 months (n=111), 6.1-12 months (n=102), 12.1-24 months (n=127), 24.1-36 months (n=102), 36.1-48 months (n=106), 48.1-60 months (n=101), 60.1-72 months (n=102).Boys with premature birth, chronic diseases and findings attributable to a specific syndrome or endocrinologic pathology that may cause micropenis were excluded. Height, weight and stretched penile length were measured. Penile length was measured from ramus pubis to the tip of glans penis with a firm ruler while penis was mildly stretched and the measurements were recorded as cm. The mean penile lengths and penile length to height ratios were presented as mean±SD, median, minimum and maximum values. Percentile charts were drawn by using LMS Chart Maker Pro Version 2.3.The penile length and penile length to height ratio charts and percentiles according to ages were presented. Micropenis was defined as a penile length shorter than -2 SD below mean. The mean stretched penile length in 101 newborn was 2.76±0.42 cm. The cut-off level (-2SD) for micropenis in newborn was 1.92 cm. There was a significant correlation between stretched penile length and height. The mean penile length to height ratio in newborns was 0.053±0,008. These ratios in older subjects (1 month to 6 years old) ranged between 0.042±0.007 to 0.052±0.008.This study helped us to construct new normal reference values and percentile charts of penile length in Turkish children aged 0 to 6 years. In addition determined penile length to height ratios may contribute to the evaluation of penile length in children aged 0 to 6 years.
