| dc.description.abstract | Amaç: Çalışmamızın amacı boyun diseksiyonları sonrası ortaya çıkan omuz ağrısı ile servikal pleksus'un hasarı arasındaki ilişkiyi araştırmaktır. Gereç ve yöntem: Çalışmamız Aralık 2003 ile Ekim 2004 tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi Kulak Burun Boğaz Anabilim Dalında Larenks kanseri tanısıyla total veya parsiyel larenjektomi + bilateral lateral (Zon 2-3-4) veya anterolateral (Zon 2-3-4-6) boyun diseksiyonu uygulanan 17 hastanın 34 boynu üzerinde prospektif olarak yapılmıştır. Bir taraf boyunda servikal pleksus korunurken diğer tarafta korunmayarak preoperatif, postoperatif 2. hafta, 1. ay, 3. ay ve 6. aylarda boyunda duyu algılamala düzeyleri ve omuz ağrısı değerlendirilmiştir. Boyunda duyu algılama düzeylerine bakılarak servikal pleksus'un hasarı ortaya konulmuştur. Omuz ağrısı değerlendirilerek her iki tarafta karşılaştırma yapılmıştır. Bulgular: Çalışma kapsamına alınan 17 hastanın 34 boynunda preoperatif duyu kaybına rastlanmadı. Hiçbir hastada preoperatif dönemde omuz hareketsiz haldeyken ya da hareketleri sırasında ağrı tespit edilmedi. Postoperatif 2. hafta, 1. ay, 3. ay ve 6. aylarda servikal pleksus'un korunduğu boyunlarda korunmayanlara göre, duyu algılama değerleri yüksekti. Servikal pleksus'un korunduğu ve korunmadığı boyun taraflarındaki omuz ağrıları karşılaştırıldı ve postoperatif 2. hafta, 1. ay, 3. ay ve 6. aylarda aralarında bir fark olmadığı tespit edildi. Sonuç: Servikal pleksus'un hasarlanmasının boyunda duyusal değişikliklere sebep olduğu görülmüştür. Seçici Boyun Diseksiyonu uygulanan hastalarda, servikal pleksus hasarının, omuz ağrıları üzerine etkisinin olmadığı tespit edilmiştir. | tr_TR |
| dc.description.abstract | Objective: The aim of our study is to investigate the relationship between shoulder pain that come out after neck dissections and the damage of the plexus cervicalis. Material and Method: Our study is done prospectively on the 34 necks of the 17 patients to whom total partial larengectomy + bilateral lateral (zone 2-3-4) or anterolateral (zone 2-3-4-6) selective neck dissection were applied with the diagnosis of laryngeal carcinoma in Uludag University Medical School, Otorhinolaryngology Department in between December 2003 and October 2004. Evaluations are made preoperative; postoperative second week, first month and sixth month, in the patients with the plexus cervicalis protected on one side and sacrified on the other side of the neck, during dissection. The damage of the cervical plexus is revealed with looking at sense perception levels in the neck. A comparison is made in both sides of the neck based on the shoulder pain. Results: Loss of sensitivity is not encountered preoperatively in the 34 necks of the 17 patients who are accepted in the study. Pain is not determined at any patient while the shoulder is mobile and immobile. Sense perception levels are high in the necks that plexus cervicalis are protected than the sacrified ones in postoperative second week, first month, third month and sixth month. The shoulder pain in the protected and the sacrified side is compared and it is found that there is no difference between postoperative second week, first month, thirdmonth and sixth month. conclusion: It is seen that the damage of the plexus cervicalis causes sensory changes in the neck. It is determined that the damage of the plexus cervicalis has no effect on the shoulder pain in the patients to whom selective neck dissection were applied. | en_US |
