Abstract
Yaşlı nüfusundaki artışa paralel olarak adli nedenli otopsiler içerisindeki yaşlı oranları da artış göstermektedir. Adli ölü muayenesi ve otopsi ani, beklenmedik, şüpheli ve doğal olmayan tüm adli ölümlerde uygulanmaktadır. Bu çalışmada Bursa ’da adli otopsisi yapılmış 65 yaş ve üzeri ölüm olgularına ait sosyodemografik özelliklerin ve ölüm nedenlerinin ortaya konması, elde edilen verileri literatür ile karşılaştırılması, yaşlı ölüm olgularında adli otopsinin önemi ile yaşlı nüfusun sağlık ve sosyal sorunlarına dikkat çekmek amaçlanmıştır. Adli Tıp Kurumu Bursa Grup Başkanlığı’nda 2019-2021 yılları arasındaki 3 yıllık süre içerisinde otopsisi yapılan 4247 olgu retrospektif olarak taranmış ve 65 yaş üzeri olan 877 olgu çalışmamıza dahil edilmiştir. Otopsisi yapılan 4247 olgudan %20,6’sının 65 yaş ve üstü olduğu, olguların yaş ortalaması 74,70±7,3 olup, %74,6’ının erkek olduğu saptanmıştır. Her iki cinsiyette 65-74 yaş grubundaki olgu sayısının diğer gruplara göre daha fazla olduğu saptanmıştır. En fazla olgunun yaz (%30,6), en az olgunun ise ilkbahar (%20) mevsiminde olduğu görülmüştür. Olguların en sık ölüm orijini doğal ölüm (%51,4) olup kardiyovasküler hastalıkların en sık doğal ölüm nedeni olduğu saptanmıştır. Kazaların erkeklerde en sık trafik kazası kadınlarda ise suda boğulma olduğu tespit edilmiştir. İntihar olgularının %81,3’ünün ası olduğu ve asının her iki cinsiyette de en sık intihar yöntemi olduğu saptanmıştır. Ülkemizde tıbbi otopsi sayısının az olduğu göz önüne alındığında adli otopsilerin yaşlı ölümleri ile ilgili çok sayıda veri sağlayabileceği görülmektedir. Bu tür çalışmalardan elde edilecek bilgiler ile ölüm nedenleri ve orijinlerinin tespit edilmesi önlenebilir nedenlerin ortadan kaldırılabilmesi için yaşlı nüfusa yönelik gerekli sağlık hizmeti ve sosyal bakım konusundaki önceliklerin belirlenmesine yardımcı olacağını düşünmekteyiz.
In parallel with the increase in the elderly population, the proportion of elderly people in autopsies for forensic reasons is also increasing. Forensic death examination and autopsy are performed in all sudden, unexpected, suspicious and unnatural forensic deaths. In this study, it was aimed to reveal the sociodemographic characteristics and causes of death of deaths of people aged 65 and over who had forensic autopsies performed in Bursa, to compare the obtained data with the literature, to draw attention to the importance of forensic autopsy in elderly death cases and the health and social problems of the elderly population. 4247 cases whose autopsies were performed in the 3-year period between 2019-2021 at the Bursa Group Chairman of Council of Forensic Medicine were retrospectively scanned and 877 cases over the age of 65 were examined. It was determined that 20.6% of the 4247 cases who were autopsied were 65 years or older, the average age of the cases was 74.70±7.3, and 74.6% were men. It was found that the number of cases in the 65-74 age group was higher in both genders than in other groups. It was observed that the most cases were in summer (30.6%) and the least in spring (20%). The most common cause of death was natural death (51.4%), and cardiovascular diseases were found to be the most common cause of natural death. It has been determined that the most common accidents are traffic accidents in men and drowning in women. It was determined that 81.3% of suicide cases were by hanging and hanging was the most common suicide method in both genders. Considering that the number of medical autopsies in our country is low, it can be seen that forensic autopsies can provide a lot of data regarding elderly deaths. We think that the information obtained from such studies and determining the causes and origins of death will help determine the priorities for the necessary health services and social care for the elderly population in order to eliminate preventable causes.
