Abstract
1206 meme kanseri olgusu içinde saptanan 29 bilateral meme tümörü geriye dönük olarak incelendi. Bilateral meme kanseri sıklığı % 2.4 olarak bulundu. İlk tam sırasındaki yaş ortalaması 46 idi. Olguların çoğunluğu invazif duktal karsinom olarak belirlendi (% 86). Elli sekiz tümörün 30'u Evre II (% 52), 10'u Evre IV (% 17), 10'u Evre IV (% 17) ve 8'i Evre III (% 14) olarak evlendirildi. 0lguların beşi eş zamanlı bilateral hastalık olarak bulundular (% 17). Diğerlerinin 7'si senkron (% 24, ilkinden sonra ilk yıl içinde gelişen) 17'si de asenkron (% 59, aradaki süre 1 yıldan fazla) olarak gelişmişlerdi. İki kanser arasındaki ortalama süre 2.6 ± 0.6 yıldı (medyan 1.2, sınırlar 0-16.7). Ortalama sağkalım 4.8 ± 0.7 yıl (nıedyan 4.4, sınırlar 0.6-18.2) olarak bulundu. İkinci kanser gelişiminden sonraki sağkalım ise 2.2 ± 0.4 yıldı (medyan 1.4, sınırlar 0.2-8.9). 5 yıllık düzeltilmiş sağkalım % 51 olarak bulundu. Yaş, menaposal durum ve tümör büyüklüğü iki kanser arasındaki süre ile; yaş, tümör büyüklüğü, ilk kanser sırasındaki nodal durum ve iki kanser arasında geçen sürede sağkalım ile uyum gösterdi.
Twenty-nine bilateral breası carcinonıa (58 breası cancers) cases among 1.206 patients with carcinonıa of the breast were analyzed retrospectively.The incidence of bilateral carcinoma of the breast among unilateral breast cancer patients was approximately 2.4 %. Median age was 46 years. The majority of the lesions were invasive ductal carcinoma (86 %). Of 58 tumors, 30 were staged as Stage II (52 %), 10 as Stage I (17 %), 10 as Stage IV ( 17 %) and 8 as Stage III (14 %). Of 29 patients with bilateral breası cancer, 5 presented with simultaneous bilateral disease ( a second primary within first month of initial cancer, 17 % ), 7 (24 %) with synchronous tumors (a second primary diagnosed within 1 year) where as 17 (59 % ) developed asynchronous tumors (a second primary diagnosed after 1 year of the first primary). The mean interval between two catıcers was 2.6 ± 0.6 years. Overall survival was 4.8 ± 0.7 years. Mean survival after the second cancer was 2.2 ± 0.4 years. Overall Syear actuarial survival was calculated to be 51 %. Age, menopausal status and tumor size at the time of initial cancer co"elated with the time interval between two cancers. Age, tumor size and nodal status at the time of initial cancer and the time interval between two cancers correlated with the overall survival.
