Abstract
Adli vaka, bir kişinin fiziksel ya da ruhsal olarak hasta diyebileceğimiz bir duruma gelmesinde başka kişi veya kişilerin kasıt, ihmal, tedbirsizlik veya dikkatsizliğinin etken olmasıdır ve acil servislerde en sık karşılaşılan olgular arasında yer almaktadır. Bu çalışmada, adli olgularda üniversitemiz acil servisinde tutulan tıbbi belge ve kayıtlardaki eksikliklerin ortaya konulması amaçlanmaktadır. Araştırma; retrospektif olarak yapılmış; adli olgu olarak başvuran olguların demografik özellikleri ve kayıt esnasında yaşanan eksikliklerin ortaya çıkarılması hedeflenmiştir. Her hasta için; ad, soyad, protokol numarası, yaş aralığı, cinsiyet, başvuru saati ve tarihi, geliş şekli, olay türü, lezyon tanımı, kalıcı iz veya sakatlık varlığı, alkol durumu, hayati tehlike ve prognoz kayıt edilmiştir. Adli olay türlerine göre yaş değişkeni bakımından anlamlı farklılıklar görülmektedir. İş kazaları olgularında yaş medyan değeri diğer olay türlerindeki yaş medyan değerinden anlamlı düzeyde daha yüksek saptanmıştır. Alkollü olan olguların ve hayati tehlikesi olan olguların saat aralıklarına göre dağılımlarında anlamlı farklılıklar görülmektedir. Adli olgularla sık karşılaşan ve mesleki hata riskleri yüksek olan hekimlere özellikle acil hekimlerine düzenli eğitim verilmesi önemlidir. Bu açıdan adli olguların özellikle raporlama aşamasının daha özenli yapılması için mezuniyet sonrası anabilim dalları ile ortak yürütülecek adli tıp eğitim programları ve adli tıp uzmanları ile koordine çalışma gereklidir.
A forensic case is the fact that one person is physically or mentally ill and is caused by intention, neglect, imprudence or carelessness of another person or persons and is one of the most common cases in emergency services. In this study, it is aimed to reveal the deficiencies in medical records and records held in the emergency room of our university during forensic cases. The research was carried out retrospectively; it was aimed to reveal the demographic characteristics of the cases referred to as forensic cases and the deficiencies experienced during the registration. For each patient; name, surname, protocol number, age range,gender, time and date of admission, arrival mode,type of event, definition of lesion, presence of permanent trauma or disability, alcohol status, life threatening and prognosis were recorded. There are significant differences in terms of age variable according to the types of forensic cases. In the case of work accidents, the age median value was found to be significantly higher than the median value of age in the other incident types. There are significant differences in the distribution of alcoholic cases and life-threatening cases according to time intervals. It is important to give regular training to physicians especially emergency physicians who are frequently encountered in forensic cases and who have a high risk of professional errors. In this respect, coordination with forensic medicine specialists and forensic medicine training programs are necessary after graduation in order to make forensic cases, especially the reporting phase, more attentive.
