Abstract
Çalışmamızda %6 HES (130/0,4) solüsyonunun kan şekeri üzerine etkisini incelemeyi amaçladık. Altmış ASA l-ll grubu, non- diabetik, 18-75 yaş aralığında, spinal anestezi uygulanarak elektif cerrahi planlanan olgu çalışmaya dahil edildi. Fakülte etik kurul onayı alındıktan sonra hastalar rasgele 2 gruba ayrıldı: Grup-HES: 500 mİ % 6 HES (130/0,4) Grup-SF: 1000 mİ %0.9 serum fizyolojik Hastalara spinal anestezi öncesi 30 dk. içinde önyükleme amacıyla sıvı infüzyonu uygulandı. İnfüzyondan önce (Zı), infüzyon tamamlandıktan hemen sonra (Z2) ve İnfüzyonun tamamlanmasından sonra sırasıyla 45 dk (Z3), 6 saat (Z4) ve 12 saat sonra (Z5) toplam 5 kez kalibrasyonları düzenli olarak yapılan standart kan şekeri ölçüm cihazıyla, kapiller kan şekeri ölçümleri yapıldı. Buna eş zamanlı olarak ortalama arteriyel kan basıncı, kalp atım hızı ve SpÜ2 kaydedildi. Prehidrasyon uygulanan tüm hastalara, spinal anestezi sol lateral pozisyonda, L3-4 veya L4-5 intervertebral aralıktan, orta hattan 25G Quincke spinal iğne kullanılarak 12.5 mg %0.5 hiperbarik bupivakain uygulandı. Operasyon bitiminde anestezi ve cerrahi süreleri ayrı ayrı kaydedildi. Gruplar arasında cerrahi ve anestezi süreleri, ASA skoru ve cinsiyet açısından fark saptanmadı (p>0.05). Kan şekeri değerleri infüzyondan önceki değerine göre SF grubunda infüzyon tamamlandıktan 6 saat sonra (p<0.05), HES grubunda ise tüm zamanlarda yüksek bulundu (p<0.001, p<0.01, p<0.001, p<0.01). HES grubunda infüzyondan sonraki 6. saatte kan şekeri değerleri açısından iki grup arasında anlamlı fark vardı (p<0.001). Sonuçlarımız, 500 mİ %6 HES (130/0,4) uygulanan grupta kapiller kan şekeri düzeylerinin, diğer gruba göre daha yüksek olduğunu gösterdi. Çalışmamızdan elde ettiğimiz bulgular doğrultusunda %6 HES (130/0,4) uygulamasının kan şekeri düzeylerini etkilediği, özellikle infüzyondan 6 saat sonra bu artışın en yüksek düzeye ulaştığı sonucuna vardık. Bu sonucun kan şekeri izleminin önemli olduğu olgularda dikkate alınması gerektiği düşüncesindeyiz.
In our study, we aimed to investigate the effect of 6% HES (130/0,4) solution on the blood glucose level. Sixty patients ASA l-ll group with 18-75 age range, non diabetic those were planned for elective surgery under spinal anaesthesia were included in the study. After receiving the Faculty Ethics Committee approval, patients were randomly divided into 2 groups: Group-HES: 500 ml 6% HES (130/0,4), Group-SF: 1000 ml 0.9% normal saline. Before the spinal anaesthesia, within 30 minutes, fluid infusion was applied to the patients for preloading. Before infusion (Zi), immediately after the completion of the infusion (Z2), and 45 min (Z3), 6 hours (Z4), and 12 hours (Z5) after the infusion, total of 5 times, capillary blood sugar measurements were done using a regularly calibrated standard blood sugar measurement instrument and average arterial blood pressure, heart rate and Sp02 were recorded simultaneously. Spinal anaesthesia with 12.5 mg 0.5% hyperbaric bupivacaine using 25G Quincke spinal needle was applied to all prehydrated patients at the left lateral position, through midline L3-4 or L4-5 intervertebral space. After the completion of the operation, duration of anaesthesia and surgery were recorded separately. ASA classification, gender, age, duration of surgery and anaesthesia were not significantly different between the groups (p>0.05). Blood sugar levels were found higher in SF group 6 hours after the infusion (p<0.05), and in HES group at all times (p< 0.001, p<0.01, p< 0.001, p< 0.01) as compared to the values before the infusion. At 6th hour after infusion, there was a significant difference in blood sugar between the groups (p<0.001). Our results showed that the blood sugar levels are higher in the group that was applied 500 ml 6% HES (130/0,4) with respect to the other group. According to data that we have found in our study, administration of 6% HES (130/0,4) alters blood sugar levels. Especially, after 6 hours of HES infusion, blood sugar level was at the highest level. These findings should be carefully taken into consideration in patients whose blood sugar level measurement is important.
