Abstract
BCR-ABL pozitif hücrelerin tespitinde, Floresan In Situ Hibridizasyon (FISH) ve Kantitatif Revers Transkriptaz Polimeraz Zincir Reaksiyonunu (QRT-PCR) içeren moleküler tanısal yöntemler, klasik sitogenetik analize göre daha sensitiftir. QRT-PCR, üstün sensitivitesinden dolayı, İmatinib ile tedavi edilen Kronik Miyelositer Lösemilerin (KML) takibinde altın standart olarak düşünülmektedir. Bizim çalışmamızın amacı, KML hastalarında farklı zamanlarda FISH ve QRT-PCR'ın diagnostik ve klinik faydalılığının karşılaştırılmasıdır.Gereç ve Yöntem: Bu çalışmada, retrospektif olarak analiz edilen 78 KML hastasının 135 sonucu Philadelphia (Ph) translokasyonu için hem FISH hem de QRT-PCR çalışmaları ile test ettik ve bu iki farklı tekniğin avantaj ve dezavantajlarını değerlendirdik.Bulgular: Bizim çalışmamız, yeni tanı konmuş vakaların tümünün her iki metotla pozitif olduğunu gösterdi. Bir vakada, FISH pozitifti, QRT-PCR negatifti; 61 vakada QRT-PCR pozitifken FISH negatifti. FISH ve QRT-PCR'ın tam uyumu (her iki testte negatif veya pozitif olma) 73 vakada bulundu (%54,1) ve 62 vakada (%45,9) farklılık tanımlandı. Aynı zamanda QRT-PCR değerleri ile sitogenetik yanıt kategorileri arasında iyi bir uyum vardı.Sonuç: QRT-PCR'ın düşük seviyelerdeki BCR-ABL transkriptlerini tam olarak ölçmeye izin verdiğini ve minimal rezidüel hastalığın (MRH) hassas br göstergesi olarak hizmet verdiğini doğruladık. Ek olarak, yeni tanı konmuş KML hastaları arasında BCR-ABL füzyon genini tespit etmede FISH ile QRT-PCR `ın %100 korele olduğunu gösterdik. Aynı zamanda FISH'in minimal rezidüel hastalığın takibinde uygun olmadığı sonucuna vardık.
The molecular diagnostic methods including fluorescence in situ hybridization (FISH) and quantitative reverse transcription-polymerase chain reaction (QRT-PCR) are more sensitive than classical cytogenetic analysis for the detection of BCR-ABL positive cells. QRT-PCR, due to its superior sensitivity, is considered the gold standard for the follow-up of chronic myelogenous leukemia (CML) treated with imatinib. The objective of our study was to compare the diagnostic and clinical usefulness of FISH and QRT-PCR at different timepoints for CML patients.Material and Methods: In this study, we retrospectively analyzed 135 results of 78 patients with CML tested by both FISH and QRT-PCR studies for Philadelphia (Ph) translocation and evaluated the advantages and disadvantages of these two different techniques.Results: Our study demonstrated that all newly diagnosed cases were positive with both methods. In one case, FISH was positive QRT-PCR was negative; in 61 cases, FISH was negative while QRT-PCR was positive. An overall concordance of FISH and QRT-PCR (being either negative or positive in both tests) was found in 73 cases (54.1%) and a discrepancy identified in 62 cases (45.9 %). Also there was good concordance between QC-PCR values and cytogenetic response categories.Conclusions: We confirm that QRT-PCR allows precise measurement of low levels of BCR-ABL transcripts and can serve as a sensitive indicator for minimal residual disease. In addition, we demonstrate 100% correlation of QRT-PCR with FISH in detecting the BCR-ABL fusion gene among patients with newly diagnosed CML. FISH is not suitable for monitoring minimal residual disease.
