| dc.description.abstract | Bu çalışmanın amacı; meme kanseri tanısıyla radyoterapi uygulanan 70 yaş üstü meme kanserli olgularda genel sağkalım, hastalıksız ve hastalığa özgü sağkalım üzerine etkili prognostik faktörleri belirlemektir.Aralık 1995- Aralık 2006 tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi (UUTF) Muammer Ağım Radyoterapi Merkezi'ne başvuran 70 yaş üstü meme kanseri tanılı olgulardan dosya bilgilerine ulaşabildiğimiz 64 olgu retrospektif olarak değerlendirildi. Olguların ortalama yaşı 76 (aralık: 70-85) idi.Sağkalım üzerine etkili prognostik faktörler kişiyle, tümörle ve tedaviyle ilgili parametreler olarak analiz edildi. İstatistiksel hesaplamalarda tek değişkenli analizde Kaplan- Meier Log Rank testleri kullanıldı. Hasta sayısı yeterli olmadığı için çok değişkenli analiz yapılmadı. İstatistiksel anlamlı değer olarak p ? 0.05 kabul edildi.Medyan 50 aylık (aralık: 6-143 ay) izlem süresi içinde olguların 3'ünde lokal yineleme, 13'ünde uzak metastaz saptanmış olup, 11 olgu meme kanseri, 15 olgu hastalık dışı nedenlere bağlı olarak kaybedildi. Sırasıyla; beş ve on yıllık hastalıksız sağkalım (HSK) %74, %63, genel sağkalım (GSK) %53, % 37 ve hastalığa özgü sağkalım (HÖSK) %80, %63, ve olarak bulundu.Sonuç olarak çalışmamızda HSK üzerine etkili prognostik faktörler, lateralite (p=0.032 ), hastalık evresi (p<0.000), T evresi (p=0.007), N evresi (p<0.000), pozitif nod sayısı (p=0.001), metastatik lenf nodu çapı (p=0.047), ER (p=0.001), PR (p=0.045), lenf damarı invazyonu (p=0.022), cilt invazyonu (p<0.000), pectoral kas invazyonu (p<0.000), neoadjuvan KT (p=0.005), preRTKT (p=0.039 ) ve toplam KT (p=0.037) idi. GSK üzerine etkili faktörler, neoadjuvan KT (p=0.020), cilt invazyonu (p=0.041) ve metastaz (p=0.001) olarak saptandı. HÖSK üzerine etkili prognostik faktörler ise, hastalık evresi (p<0.000), tümör evresi (p=0.001), ER (p=0.028), neoadjuvan KT (p=0.026), N evresi (p=0.003), lenf damarı invazyonu (p=0.005), cilt invazyonu (p<0.000) ve metastaz (p<0.000) olarak bulundu. Sonuç olarak, ilgili literatürün ışığında çalışmamız değerlendirildiğinde, daha genç yaş grubuna özgü prognostik etmenlerin 70 yaş üstü olgular için de geçerli olduğu ve ko-morbid faktörler göz önüne alınarak optimum tedavinin verilmesi gerektiği sonucuna ulaştık. | tr_TR |
| dc.description.abstract | The aim of this study is to evaluate the prognostic factors that affect overall survival, disease- free and disease specific survival in the cases with breast cancer over 70 years who received radiotherapy.Sixty-four cases with breast cancer older than 70 years that attended to the Uludag University Faculty of Medicine (Muammer Agim Radiotherapy Center), beetween January 1995- January 2006, were evaluated retrospectively. The median age of the cases was 76 years (range, 70-85).The prognostic factors that effect survival were analysed as personal, tumor and treatment related parameters. In univariate analysis the statistical analysis of Kaplan-Meier curves performed by log rank method. As the number of the cases were not sufficient, multivariate analysis couldn?t be performed. A p? 0,05 value was considered as statistically significant.During the median 50 months? follow ?up time (range, 6-143) in three cases local recurrence and in 13 cases distant metastases were observed. Eleven of the cases died of breast cancer- related, and 15 cases died of disease- free medical problems. The 5 and 10- year disease- free survival (DFS) rates were %74 and %63, the overall survival (OS) rates were %53 and %37 and the disease- specific survival (DSS) rates were %80 and %63, respectively.The outcomes of our study; the prognostic factors effective on DFS had been laterality (p=0.032), disease stage (p<0.000), T stage (p=0.007), N stage (p<0.000), positive node (p=0.001), the diameter of involved node (p=0.047), estrogen receptor (p=0.001), progesteron receptor (p=0.045), presence of lenfovasculer invasion (p=0.022), skin invasion (p<0.000), pectoralis muscle invasion (p<0.000), neoadjuvant KT (p=0.005), pre RTKT (p=0.039) and the number of the chemotherapy cycles ( p=0.037). The prognostic factors efective on OS were observed as neoadjuvant KT (p=0.020), skin invasion (p<0.041) and metastases (p=0.001). The prognostic factors found to be effective on DSS were, disease stage (p<0.000), T stage (p=0.001), estrogen receptor (p=0.028), neoadjuvant KT (p=0.026), N stage (p=0.003), presence of lenfovasculer invasion (p=0.005), skin invasion (p<0.000) and metastases (p<0.000). As a result, when our study is evaluated according to the related literatures the prognostic factors specific to the younger cases is similar to the cases older than 70 years and when we pay attention to the comorbid factors we conclude that the optimal treatment modality should be exposed even to the cases older than 70 years. | en_US |
