Abstract
Tip 2 Diyabetes Mellitus insülin salgılanmasındaki problem ile karakterize hiperglisemi ile seyreden kronik ve sinsi bir hastalıktır. Tedavi edilmediği ve düzenlenen tedavilere uyulmadığı durumlarda yaşam kalitesinde ciddi düşüşlere, organ kayıplarına ve hatta ölümlere yol açan komplikasyonlar doğurabilmektedir. Uzun yıllar boyunca ilaç kullanması gereken bu hastalar için tedavi uyumu hayati önem arz etmektedir. Bu çalışmada, Tip 2 Diyabetes Mellitus tanılı hastalarda mizaç ve karakter özelliklerinin tedavi uyumu üzerindeki etkisinin araştırılması amaçlandı. Çalışmamıza Bursa Uludağ Üniversitesi Endokrinoloji Poliklinik ve Kliniği'nde takip edilen 66 Tip 2 Diyabetes Mellitus hastası dahil edildi. Katılımcılar bir defa kesitsel olarak görüldü; Sosoyodemografik Veri Formu, Mizaç ve Karakter Envanteri, Tip 2 Diabetes Mellitus Tedavisine Hasta Uyum Ölçeği ile değerlendirildi. Çalışmamızda Tip 2 Diyabetes Mellitus tedavisinde, mizaç özellikleri içerisinde kötü uyum ile ilişkili olarak Yenilik Arayışı ölçeğinin alt ölçekleri olan Düzensizlik ve Savurganlık bulunmuşken; iyi uyum ile Ödül Bağımlılığı ölçeği ve onun Bağlanma alt ölçeği ilişkili bulunmuştur. Karakter özellikleri içerisinde kötü uyum ile ilişkili herhangi bir ölçek bulunmazken; iyi uyum ile Kendini Yönetme ve onun tüm alt ölçekleri, İş Birliği Yapma ve onun Sosyal onaylanma ve Erdemlilik-Vicdanlılık alt ölçekleri ilişkili bulunmuştur. Düşük sosyoekonomik düzey, akrabalarla birlikte yaşamak, polifarmasi ve alkol kullanımı kötü tedavi uyumu ile ilişkili bulunmuşken; 20 yıl ve üzerinde hastalık öyküsü iyi tedavi uyumu ile ilişkili bulunmuştur. Tip 2 Diyabetes Mellitus hastalarının tedaviye uyumlarının büyük önem arz ettiği gerçeği göz önünde bulundurularak, hastaların rutin takiplerinde mizaç ve karakter özelliklerinin dikkate alınması önerilmektedir.
Type 2 Diabetes Mellitus is a chronic and insidious disease characterized by hyperglycemia due to problems in insulin secretion. If left untreated or if treatments are not followed, it can lead to significant decreases in quality of life, organ loss, and even complications that result in death. Compliance with treatment is of great importance for individuals with diabetes who have to use medication for many years. The aim of this study is to investigate the effect of temperament and character traits on treatment compliance in patients diagnosed with Type 2 Diabetes Mellitus. 66 Type 2 Diabetes Mellitus patients followed at Bursa Uludağ University Endocrinology Clinic were included in our study. Participants were evaluated cross-sectionally once using the Sociodemographic Data Form, Temperament and Character Inventory, and Type 2 Diabetes Mellitus Treatment Compliance Scale. In our study, while the Novelty Seeking scale and especially its subscales Disorganization and Extravagance, which are temperament characteristics in the treatment of Type 2 Diabetes Mellitus, were associated with poor compliance, the Reward Dependency scale and the Attachment subscale were associated with good compliance. Although no specific character trait was associated with poor commitment, good commitment was related to the Self-Direction scale and all its subscales, as well as the Cooperation Scale and the Social Approval and Conscientiousness subscales. Low socioeconomic status, living with relatives, polypharmacy, and alcohol use were associated with poor treatment compliance, while a disease history of 20 years or more was associated with good treatment compliance. Considering the importance of compliance with treatment in Type 2 Diabetes Mellitus patients, it is recommended that temperament and character characteristics of patients be taken into account in routine follow-up.
