Preoperatif laboratuar testlerinin yararlılığı

Abstract

Giriş: Günümüzde laboratuar testlerinin yaklaşık %10'unu sağlık harcamalarının çoğunu kapsayan preoperatif rutin testler oluşturmaktadır. Bu testlerin çoğu faydasızdır. Metod ve Hastalar: 1 Haziran 2001 - 15 Şubat 2002 tarihi arasında 134 genel cerrahi hastasına ilişkin bilgiler prospektif olarak incelenmiştir. Hastanın anamnez ve fizik muayenesine göre istenecek preoperatif testler planlanmıştır. Bu hastalara küçük ve orta grup çeşitli ameliyatlar yapılmıştır. Bulgular: 134 hastanın 113'ünde yandaş hastalık yoktur ve en sık görülen yandaş hastalık kardiak nedenlerle olanlardır. 134 hasta için hasta çalışılan 317 testin 47 tanesinde anormal sonuç saptandı. Fakat bu anormal sonuçların hiçbiri hasta için planlanan cerrahi yaklaşımı etkilemedi. Sonuç: Bu çalışma sonucunda literatürdeki diğer çalışmalarda da belirtildiği gibi preoperatif tarama testlerinin muayene sonuçlarına göre istenmesi gerektiğini ve gereksiz testlerin dikkatli bir anamnez ve iyi bir fizik muayene ile engellenebileceğini düşünmekteyiz.Introduction: Recently about 10% of the laboratory tests are preoperative routine tests which consist of the great amount of health payments. Most of these tests are useless. Method and Patients: 134 general surgey patients'data are examined between 01st Jun. 2001-15th feb. 2002 in a prospective manner. According to the story and physial examination of the patient preoperative tests that are to be prescriped were organizet. Those patients underwent smoll and medium size operation of various kind. Results: Of the 134 patients 113 did not have a concominant disease. The most common coexisting disease was caused by cardiac reasons. None of the 47 abnormal resuls of 317 tests ordered for 134 patients did affect the surgical management of the patients. Coclusion: In this study, as it is in the literature, we conclude that preoperative screening tests should be ordered just on the finding at medical examination. Unnecessary tests must be avoided by taking a careful story and a good physical examination.
