Abstract
Bu çalışmanın amacı; kandidemilerde serum IL-23, IL-17, IL-1ß, TNF-a, IL-10, TGF-ß, CRP, PCT, serum amiloid A (SAA) ve mannoz bağlayan lektin (MBL) düzeylerini ölçmek, mannan antijeni ve anti-mannan antikorunun kandidemide tanı değerine bakmak, bakteriyemilerle karşılaştırmaktır. Bu amaçla kandidemili 50 hastanın serum sitokin düzeyleri, 14 polimikrobiyal sepsis, 27 sağlıklı kontrol ve 30 bakteriyemi hastası ile karşılaştırıldı. Ayrıca mannan antijeni ve mannan antikorunun kandidemide tanı değeri araştırıldı. Sitokin düzeyleri ve mannan-antimannan testleri üretici firma protokolüne göre sandviç ELISA yöntemi ile çalışıldı. Serum TGF-ß, IL-23, IL-17 düzeyleri kandidemi grubunda bakteriyemi ve sağlıklı kontrol grubuna göre anlamlı yüksek bulundu. PCT ve SAA düzeyleri ise bakteriyemi grubunda kandidemi ve sağlıklı kontrol grubuna göre yüksek bulundu. Üç grup arasında, IL-1ß, IL-10 ve TNF-a düzeyleri açısından anlamlı fark saptanmadı. Proinflamatuvar sitokin olarak IL-1ß, TNF-a, IL-17 ve IL-23'ün kandidemi tanısında kullanılabilecek yeni bir test olarak kabul edilip edilemeyeceği araştırıldı. IL-17 düzeyleri için eşik değeri >38,79 pg/mL olarak alındığında, duyarlılığı %38 ve özgüllüğü %96,6, IL-23 için eşik değeri >59,97 pg/mL olarak alındığında, duyarlılığı %72 ve özgüllüğü %60 olarak bulundu. IL-1ß ve TNF-a için kandidemi ile bakteriyemiyi ayırmada eşik değer elde edilemedi. Antiinflamatuvar sitokin olarak IL-10 ve TGF-ß?nın kandidemi grubu ile bakteriyemi ayırabilmedeki performansı incelendiğinde IL-10 testi için eşik değer elde edilemedi. Kandidemili olgularda, TGF-ß düzeylerinin eşik değeri >560 pg/mL olarak alındığında duyarlılığı %85,71 ve özgüllüğü %53,33 olarak bulundu. Sonuç olarak, çalışmamız IL-17, IL-23 ve TGF-ß düzeylerinin kandidemi için tanı belirteci olabileceğini ve IL-17, IL-23, TGF-ß ve PCT testleri için kandidemi ve bakteriyemiyi ayırabilecek eşik değerler olabileceğini gösterdi.
This study aims to measure serum levels of IL-23, IL-17, IL-1ß, TNF-a, IL-10, TGF-ß, CRP, PCT, serum amyloid A (SAA) and mannose-binding lectin (MBL) in the cases of candidemia and to compare them with those observed in the cases of bacteremia. For this purpose, serum levels of cytokines of 50 patients with candidemia with those of 14 patients with polymicrobial sepsis, 27 healthy controls and 30 patients with bacteremia. In addition, diagnostic value of mannan antigen and of anti-mannan antibody in candidemia was investigated. Cytokine levels and mannan-anti-mannan tests were studied using sandwich ELISA according to the protocol of the manufacturer. Serum levels of TGF-ß, IL-23 and IL-17 were found to be significantly higher in the group with candidemia compared to the group with bacteremia and healthy controls. PCT and SAA levels were found to be higher in the group with bacteremia compared to the group with candidemia and to healthy controls. IL-1ß, IL-10 and TNF-a levels were not found to be significantly different across three groups. It was investigated whether the proinflammatory cytokines IL-1ß, TNF-a, IL-17 and IL-23 could be considered as a new test that might be used to diagnose candidemia. While the sensitivity and the specificity were 38% and 96.6%, respectively, assuming IL-17 level threshold as >38,79 pg/mL, the corresponding values were found to be 72% and 60%, respectively, with a IL-23 threshold of >59,97 pg/mL. No threshold could be obtained for IL-1ß and TNF-a in the differentiation of candidemia and bacteremia. When the performance of the anti-inflammatory cytokines called IL-10 and TGF-ß in the differentiation of candidemia and bacteremia was examined, no threshold value was obtained for IL-10. In the patients with candidemia, when the threshold of TGF-ß levels was assumed as >560 pg/ml, the sensitivity and the specificity were found to be 85.71% and 53.33%, respectively. Consequently, our study demonstrated that IL-17, IL-23 and TGF-ß levels could be a diagnostic marker for candidemia and that there might be threshold values that could differentiate candidemia and bacteremia in the tests for IL-17, IL-23, TGF-ß and PCT.
