Abstract
Flor-18 fluorodeoksiglukoz pozitron emisyon tomografisi/bilgisayarlı tomografi (FDG PET/BT) NHL (Non Hodgkin Lenfoma) tanılı olguların evrelendirilmesinde en yararlı metot olarak kabul edilmektedir. F18-FDG akümülasyonunun yoğunluğu standardize uptake değeri (SUV) olarak ölçülebilmektedir. Bu çalışma yeni tanı alan ve tedavi uygulanmamış NHL'lı olgularda F18-FDG PET/BT görüntülemede ölçülen SUVmax değerinin önemini ve bununla birlikte hastanın tanı anındaki serum LDH (Laktat dehidrogenaz) düzeyi ve Ki-67 proliferasyon indeksleri arasındaki ilişkiyi belirlemeyi amaçlamaktadır.Çalışmaya Ocak 2012 - Ekim 2015 tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi Nükleer Tıp A.D.'da evreleme amacıyla PET/BT görüntülemesi yapılmış 31'i kadın, 51'i erkek 82 hasta dâhil edilmiştir. Hasta grubu içerisinde 9 adet histopatolojik subtip bulunmakta olup bunların 5'i agresif lenfoma, 4'ü ise indolent lenfoma alt grubuna dahildir. En çok hasta sayısına sahip grup 39 hasta ile Diffüz Büyük B Hücreli Lenfoma (DBBHL) ve en az hasta sayısına sahip grup 2'şer hasta ile Anaplastik Büyük Hücreli, Küçük Lenfositik Lenfoma ve MALT lenfomadır.Çalışmamızda histopatolojik subtiplere göre SUVmax dağılımına baktığımızda en yüksek ortalamanın Diffüz Büyük B hücreli lenfoma alt tipinde, en düşük ortalamanın Küçük Lenfositik lenfoma alt tipinde olduğu görüldü. Agresif lenfoma grubundaki SUVmax değerlerinin indolent lenfoma grubundaki değerlerden anlamlı olarak yüksek olduğu görüldü.Çalışmamızda SUVmax değerleri ile tanı anındaki serum LDH değerleri arasında korelasyon olmadığı izlendi.Tümör içerisinde prolifere hücreleri gösteren bir antikor olan Kİ-67 ve proliferasyon ile ilişkili olarak artan Ki-67 proliferasyon indeksi ile SUVmax değerleri arasında yapılan çalışmalardan farklı olarak korelasyon olmadığı saptandı.Sonuç olarak SUV değerleri literatürle uyumlu olarak agresif tip NHL hastalarında indolent tip NHL'ye göre anlamlı olarak yüksek bulunmuş ve farklılık arz etmiştir, ancak birçok NHL'lı olguda kesin tanıyı belirlemede faydalı bulunamamıştır.
Flor-18 fluorodeoxyglucose positron emission tomography/computerized tomography (FDG PET/CT) is regarded as the most efficient method in staging of NHL (Non Hodgkin Lymphoma) diagnosed cases. Density of F18-FDG accumulation can be measured as standardised uptake value (SUV). This study aims to determine the significance of SUVmax value measured with F18-FDG PET/CT imaging of new diagnosed and untreated NHL cases as well as the relation between the serum LDH (Lactate dehydrogenase) value of the patient at the time of diagnosis and Ki67 proliferation indexes.A total of 82 patients consisting of 31 women and 51 men exposed to PET/CT imaging for stating purposes in University of Uludağ Faculty of Medicine Department of Nuclear Medicine between January 2012 to October 2015.The group of patients included 9 histopathological subtypes and 5 patients out of this group were included in aggressive lymphoma and 4 patients were included in indolent lymphoma subgroup. The group with highest number of patients ie: 39 patients was Diffuse Large B-Cell Lymphoma (DLBCL) whereas the groups with lowest number of patients were Anaplastic Large Cell Lymphoma, Small Lymphocytic Lymphoma and MALT lymphoma, each with 2 patients.When, in our study, SUVmax distribution according to histopathological subtypes are considered, it was seen that Diffuse Large Bcell lymphoma subtype had the highest average whereas Small Lymphocytic Lymphoma subtype had the lowest average.It was observed that SUVmax values in aggressive lymphoma group were significantly higher than that of the values in indolent lymphoma group.In our study, it was observed that there was no correlation between the SUVmax values and LDH values at the time of diagnosis.Dissimilarly to the studies conducted, it was ascertained that there was no correlation between Kİ-67 which is an antibody indicating the proliferated cells inside the tumour and Ki-67 proliferation index which increases associated with proliferation.As a consequence, SUV values were, compatibly with the literature, found to be significantly higher and differed in aggressive type NHL patients compared to indolent type NHL patients however this was not considered as useful to define the definitive diagnosis in many of NHL cases.
