| dc.description.abstract | Cinsel sağlık ve üreme sağlığı hizmetleri (CSÜS) kişilerin özel ve cinsel yaşamının bir parçası olması nedeniyle hem heteroseksüeller hem de LGBTİ+’lar için ‘özellikli’ sağlık hizmetlerindendir. LGBTİ+’lar yaşamın pek çok alanında olduğu gibi bu özellikli hizmetlere erişim ve sonrası bağlamında da olumsuz tutum ve davranışlara maruz kalabilmektedir. LGBTİ+’ların CSÜS hizmetlerine erişim sırasında yaşadığı sorunlar hem hastalar hem de sağlık çalışanları için Tıp Etiği açısından yanıtlanması gereken özgün etik sorunları ve soruları doğurmaktadır. Tezin amacı bu tür sorunların neler olduğunu saptamak, etik açısından analiz etmek ve hak ihlali ve ikilem yaratabilen durumları önlemek için öneriler geliştirmektir. Bu amaçla PRISMA kılavuzundan yararlanılarak bir sistematik derleme çalışması yürütülmüştür. Belirlenen anahtar kelimeler ile Web of Science, Cochrane, Ovid, Proquest, EBSCO, Scopus, PubMed, ULAKBİM TR Dizin, Dergi Park, Türkiye Klinikleri veri tabanları taranmış, erişilen 1.985 makale belli ölçütlere göre değerlendirilmiş, sonuçta uygunluk gösteren 28 tanesi araştırmaya dâhil edilmiştir. Çalışmaya dâhil edilen araştırmaların kalitesi New Castle – Ottawa Quality Assessment Scale, Critical Review Form – Qualitative Studies ve A Measurement Tool to Assess Systematic Reviews kalite değerlendirme ölçekleri ile değerlendirilmiştir. Sistematik derleme ile LGBTİ+’ların özel yaşamın gizliliğine ilişkin sorunlar, damgalanma, ötekileştirilme kaygısı, homofobik/transfobik tutumlar, sağlık politikalarının yetersizliği ve sağlık çalışanlarının LGBTİ+ sağlığı hakkında eğitim yetersizliklerinin olması sebebi ile CSÜS hizmetlerine erişimde önemli ölçüde sorun yaşadıkları saptanmıştır. Saptanan sorunlar olgu örnekleriyle de tartışılmış ve ilgili düzenlemelerin, haklara saygılı olacak yönde iyileştirilmesi, sağlık politikalarının ve buna bağlı olarak sağlık hizmetlerinin örgütlenme biçiminin yeniden düzenlenmesi, sağlık çalışanlarının eğitimi ve etik danışmanlığının kurumsallaştırılması gibi sorunlara yönelik çözüm önerileri geliştirilmiştir. | tr_TR |
| dc.description.abstract | Sexual and reproductive health services (SRH) are 'special' health services for both heterosexuals and LGBTI + individuals, as they are a part of their private and sexual life. LGBTI + 's may be exposed to negative attitudes and behaviors in the context of access to these special services and afterwards, as in many areas of life. Problems experienced by LGBTI +’s during access to SRH services raise unique ethical problems and questions that need to be answered in terms of Medical Ethics for both patients and healthcare professionals. The aim of the thesis is to determine what such problems are, to analyze them in terms of ethics and to develop suggestions to prevent abuse of rights and situations that may create dilemmas. For this purpose, a systematic review study was conducted using the PRISMA guideline. Web of Science, Cochrane, Ovid, Proquest, EBSCO, Scopus, PubMed, ULAKBİM TR Index, Journal Park, Turkey Clinics databases were scanned, the accessed 1,985 articles were evaluated according to certain criteria, 28 of them were included in the study. The quality of the studies included in the study was evaluated with the New Castle - Ottawa Quality Assessment Scale, Critical Review Form - Qualitative Studies and A Measurement Tool to Assess Systematic Reviews quality assessment scales. With the systematic review, it has been determined that LGBTI +’s have significant problems in accessing SRH services due to problems related to privacy of private life, stigmatization, anxiety of being marginalized, homophobic / transphobic attitudes, insufficiency of health policies and inadequacy of healthcare workers about LGBTI + health. The identified problems were also discussed with case examples, and solutions were developed for problems such as improving the relevant regulations in a way that respects the rights, reorganizing health policies and the way health services are organized accordingly, training healthcare professionals and institutionalizing ethical counseling. | en_US |
