Abstract
B hücreli akut lenfoblastik lösemi’de (ALL) Her-2/neu ekspresyonunun olabileceği az sayıdaki çalısmalarda gösterilmis olmakla birlikte, löseminin diğer tiplerinde Her-2/neu ekspresyonu ve biyolojik önemi henüz yeterince arastırılmamıstır. Prospektif olarak yürütülen çalısmamızda, gerek lenfoid gerekse miyeloid lösemi tiplerinde Her-2/neu ekspresyon durumunun saptanması amaçlandı. Çalısmamıza, 2006 Mayıs ve 2007 Subat tarihleri arasında hasta alımı yapıldı. Akım sitometri aracılığı ile Her-2/neu spesifik monoklonal antikor (Becton Dickinson, CA, USA) kullanılarak, 38’i tanı ve 11’i nüks anında olmak üzere toplam 49 akut lösemi hastasının lösemik blastlarında Her-2/neu ekspresyonu analiz edildi. Her-2/neu ekspresyonun analizi için kemik iliği aspirasyon örnekleri kullanıldı. Olguların 41’i (%83,67) akut miyeloid lösemi (AML), 7’si (%14,29) BALL ve 1’i (%2,04) T-ALL olgusu idi. 41 AML olgusunun 32’si (%78,05) de novo AML ve 9’u (%21,95) sekonder AML olgusu idi. Yedi ALL olgusunun tümü de novo akut lösemiydi. De novo ya da sekonder AML tanısına sahip 41 olguda ve T-ALL tanısına sahip 1 olguda Her-2/neu ekspresyonu saptanmadı. Diğer taraftan 7 B-ALL olgusunun 1’inde (%14,29) Her-2/neu ekspresyonu tespit edildi. Sonuç olarak, 49 akut lösemi olgusu içeren bu prospektif çalısmada, bazı ALL alt tiplerinde Her-2/neu’nun eksprese olabileceği gösterildi. Her- 2/neu ekspresyonunun, akut lösemilerde prognostik öneme sahip olup olmadığının ve Her-2/neu odaklı tedavi stratejileri için potansiyel bir hedef olup olmadığının yeni çalısmalar ile arastırılması gerektiği kanaatine varıldı.
Although, it has been demonstrated in a few studies that Her-2/neu expression can be detected on the blast cells from patients with subset of Bacute lymphoblastic leukemia (ALL), its expression and biological importance on blasts cells from other type of leukemias was not well studied yet. In this prospective study, we aimed to evaluate the incidence of Her-2/neu expression on both lymphoid and myeloid type leukemias. The period of enrollment was between May 2006 and Febuary 2007. We analyzed the expression of Her-2/neu on the leukemic blasts from 49 patients with acute leukemia, including 38 at diagnosis and 11 at relapse, by flow cytometry using a Her-2/neu specific monoclonal antibody (Becton Dickinson, CA, USA). Bone marrow aspiration samples were used to analyze Her-2/neu expression. Fourty one patients (%83,67) had acute myeloid leukemia (AML), 7 patients (%14,29) had B-ALL and 1 patient (%2,04) had T-ALL. Among 41 AML patients, 32 (%78,05) had de nova AML and 9 (%21,95) had secondary AML. All of the 7 ALL patients had a de nova acute leukemia. Her-2/neu expression was not detected in none of the 41 patients with de nova or secondary AML and 1 patient with T-ALL. On the other hand, Her-2/neu expression was detected in 1 (%14,29) of 7 B-ALL patients. In conclusion, in this prospective study including 49 patients with acute leukemia, we showed that Her-2/neu may express on the leukemic blasts of the subset of ALL. We think that further studies are needed to evaluate whether Her-2/neu has a prognostic importance and could be used as potential target for the application of Her-2/neu directed treatment strategies in acute leukemias.
