Özet
Sağlık çalışanları ve hekimler yoğun stres maruziyeti, artmış iş yükü ve mobbing sebebiyle yüksek depresyon riskiyle karşı karşıyadır. Yüksek stres maruziyeti, özellikle de kronik iş stresi sağlık çalışanları ve hekimlerde tükenmişlik sendromunu beraberinde getirmektedir. Çalışmamız bu sebeple tıpta uzmanlık öğrencisi araştırma görevlisi hekimlerde depresyon düzeyini, depresyon ve tükenmişlik ile ilişkili faktörleri ortaya koymayı amaçlamaktadır. Kesitsel tipteki bu araştırma, Bursa Uludağ Üniversitesi Tıp Fakültesi Sağlık Uygulama ve Araştırma Merkezi'nde yürütülmüştür. Çalışma, araştırmaya katılmayı kabul eden 508 (%78,2) araştırma görevlisi hekim ile gerçekleştirilmiştir. Veri toplama aracı olarak Beck Depresyon Envanteri ve Maslach Tükenmişlik Ölçeği ile araştırmacı tarafından oluşturulan 41 sorunun yer aldığı anket formu kullanılmıştır. Verilerin analizinde tanımlayıcı istatistikler, Ki-kare testi, Bağımsız Örneklem T-Testi, ANOVA Testi, Kruskal-Wallis ve Man-Whitney U testleri kullanılmıştır. Analiz, SPSS 28.0 programı kullanılarak gerçekleştirilmiş ve anlamlılık düzeyi p<0,05 olarak kabul edilmiştir. Katılımcıların %60,63'ü kadın, yaş ortalamaları 29,25'tir. Çalışmada katılımcıların sadece %49,90'ının düzenli fiziksel aktivitede bulunduğu, %28,91'inin kronik/psikiyatrik hastalığa sahip olduğu tespit edilmiştir. Katılımcıların %71,69'u haftada kırk saatin üzerinde çalışmakta, %40,75'i fiziksel ve %73,23'ü ise zihinsel iş yükünü ağır olarak tanımlamaktadır. Katılımcıların %49,50'si nöbet ertesinde mesailerine devam etmektedir. Katılımcıların %24,02'si eğitim görevlilerinden destek görmediklerini, %23,03'ü eğitim kurumunun mesleki gelişim için yeterli olanakları sağlamadığını, %48,03'ü çalışma ortamının sağlık ve güvenlik açısından yetersiz olduğunu, %60,83'ü son bir senede hasta ve yakınlarının şiddetine, %29,72'si ise mobbinge maruz kaldığını beyan etmiştir. Araştırmada katılımcıların depresyon yaygınlığının ve tükenmişlik alt boyutu puan ortalamalarının mesleki deneyim değişkeni grupları arasında anlamlı farklılıklar gösterdiği tespit edilmiştir.
Healthcare workers and physicians face a high risk of depression due to intense stress exposure, increased workload and mobbing. High stress exposure, especially chronic work stress causes burnout syndrome in healthcare workers and physicians. For this reason, our study aims to reveal the level of depression in resident doctors and the factors associated with depression and burnout. Cross-sectional study was conducted at Bursa Uludağ University Faculty of Medicine Health Application and Research Center. The study was conducted with 508 (78.2%) resident doctors who agreed to participate in the study. Beck Depression Inventory, Maslach Burnout Inventory and a survey form containing 41 questions created by the researcher were used as data collection tools in the study. Descriptive statistics, Chi-square test, Independent Sample T-Test, ANOVA Test, Kruskal-Wallis and Man-Whitney U tests were used to analyze the data. The analysis was performed using SPSS 28.0 program and the significance level was accepted as p<0.05. In the study, it was found that 60.63% of the participants were women and participants' average age was 29.25. The study found that only 49.90% of the participants engaged in regular physical activity, and 28.91% had a chronic/psychiatric disease. In the study, it was found that 71.69% of the participants work more than forty hours a week, 40.75% describe the physical and 73.23% mental workload as heavy. 49.50% of the participants continue their work after 24-hour shift. In the study, 24.02% of the participants stated that they did not receive support from professors and other educational staff, 23.03% stated that the educational institution did not provide sufficient opportunities for professional development, 48.03% stated that the working environment was inadequate in terms of health and safety, and 60.83% of them have been exposed to violence by patients and their relatives in the last year and 29.72% declared that they were exposed to mobbing. In the study, it was determined that the depression prevalence and burnout subscale score averages of the participants showed significant differences between professional and workplace experience variable groups.
