Hemodiyaliz hastalarında tedaviye uyumun kaşıntı dermatolojik yaşam kalitesi ve konfora etkisinin incelenmesi

Abstract

Bu çalışma hemodiyaliz tedavisi alan hastalarda tedaviye uyumun kaşıntı, dermatolojik yaşam kalitesi ve konfora etkisinin incelenmesi amacıyla yapılmıştır. Araştırmanın evrenini iki ayrı merkezde 13 Aralık 2023-13 Mayıs 2024 tarihleri arasında hemodiyalize giren 146 hasta, örneklemi ise dahil edilme kriterlerine uyan ve araştırmaya katılmayı kabul eden 102 hasta oluşturmuştur. Araştırmanın verilerinin toplanmasında Tanıtıcı Bilgi Formu, Türkçe Modifiye Morisky Ölçeği (TMMÖ), Dermatolojik Yaşam Kalitesi İndeksi (DYKI), Hemodiyaliz Konfor Ölçeği (HKÖ) ve 5-D Kaşıntı Ölçeği (KÖ) kullanılmıştır. Veriler SPSS 21 programında, ortalamalar, yüzdelikler, Kruskal Wallis, Spearman kolerasyon ve Man-Whitney U testleri ile değerlendirilmiştir. Hastaların %59,8’isinin erkek olduğu ve %59,8’inin ekonomik durumunu geliri giderinden az olarak tanımladığı bulunmuştur. Hastaların KBY tanı süresi 7,47±6,16 yıl, hemodiyaliz tedavi süresi ortalama 4,36±3,73 olup %88,2’si haftanın üç günü diyalize girmektedir. Hastaların kullandığı ilaç çeşidi sayısı 4,06±1,92 olup, %86.3’ünde ek bir kronik bir hastalık vardır. Hastaların ilaç tedavisine uyum motivasyon puanı 2,54±0,73 olup, %14,7’sinde motivasyon düzeyi düşük; bilgi puanı 1,93±0,37 olup, %8,8’inde bilgi düzeyi düşüktür. Hastaların dermatolojik yaşam kalitesi ölçeği puanı 2,23±2,82 olup, %48’inde dermatolojk yaşam kalitesinin iyi düzeyde olduğu belirlenmiştir. Hastaların kaşıntı ölçeği puanı 14,16±4,42 olup, orta düzeyde kaşıntı yaşadıkları belirlenmiştir. Hastaların HKÖ puan ortalamaları 32,59±5,752’dır. Hastaların sosyodemografik özelliklerinin tedaviye uyum düzeyini etkilemediği (p >0,05), hastalıkla ilişkili özelliklerinden sadece kendisinde ek hastalık olma durumunun tedaviye uyumun bilgi boyutunu etkilediği saptanmıştır (p <0,05). Hastaların ilaç tedavisine uyum ölçeği bilgi ve motivasyon alt boyutları ile dermatolojik yaşam kalitesi, kaşıntı ve konfor ölçeği puanları arasında anlamlı bir ilişki olmadığı saptanmıştır (p >0,05). Bu sonuçlar doğrultusunda tedaviye uyumun yaşam kalitesi ve konforu olumlu yönde etkilediği düşünülmektedir.

This study was conducted to examine the effect of treatment adherence on itching, dermatological quality of life, and comfort in patients undergoing hemodialysis treatment. The population of the research consisted of 146 patients who underwent hemodialysis in two different centers between December 13, 2023, and May 13, 2024. The sample consisted of 102 patients who met the inclusion criteria and agreed to participate in the study. The data of the research were collected using the Introductory Information Form, the Turkish Modified Morisky Scale (TMMS), the Dermatological Quality of Life Index (DQLI), the Hemodialysis Comfort Scale (HCS), and the 5-D Itch Scale (IS). Written permission and ethical approval were obtained from the institutions. The data were analyzed using the SPSS 21 program with means, percentages, Kruskal-Wallis, Spearman correlation, and Mann-Whitney U tests. It was found that 59.8% of the patients were male and 59.8% defined their economic situation as their income being less than their expenses. The duration of CKD diagnosis for the patients is 7.47±6.16 years, the average duration of hemodialysis treatment is 4.36±3.73 years, and 88.2% of them undergo dialysis three days a week. The number of types of medications used by the patients is 4.06±1.92, and 86.3% have an additional chronic disease. The patients' medication adherence motivation score is 2.54±0.73, with 14.7% having a low motivation level; the knowledge score is 1.93±0.37, with 8.8% having a low knowledge level. The patients' Dermatological Quality Of Life Scale score was 2.23±2.82, and it was determined that 48% of the patients had a good level of dermatological quality of life. The patients' pruritus scale score is 14.16±4.42, indicating a moderate level of pruritus. The patients' average HRQoL score is 32.59±5.752. It was found that the sociodemographic characteristics of the patients did not affect the level of treatment adherence (p >0.05), and among the disease-related characteristics, only the presence of an additional disease in the patients themselves affected the knowledge dimension of treatment adherence (p <0.05). It was found that there was no significant relationship between the patients' medication adherence scale sub-dimensions of knowledge and motivation and the scores of dermatological quality of life, pruritus, and comfort (p >0.05). In accordance with these results, adherence to treatment is considered to positively impact quality of life and comfort.
