Lokal ileri evre prostat kanserli hastalarda akut ve subakut yan etkiler açısından konformal radyoterapi ve IMRT sonuçlarının degerlendirilmesi

Abstract

Bu çalışmada lokal ileri evre prostat kanserli hastalarda yan etkiler açısından konformal radyoterapi ve IMRT (Intensity Modulated Radiotherapy) sonuçlarının değerlendirilmesi amaçlanmıştır. 01.01.2007-30.06.2013 tarihler arasında Uludağ Üniversitesi Tıp Fakültesi'ne başvuran ve küratif radyoterapi uygulanan 32 olgu retrospektif olarak değerlendirildi. Olguların hepsi erkekti, ortanca yaş 75 yıl (aralık 48-82) idi. Hastaların tamamına uzun dönem maksimum androjen blokaji (MAB) ile birlikte pelvik lenf nodlarına ve prostata radyoterapi uygulanmıştı. 3BKRT (Üç boyutlu konformal radyoterapi)'de 1,8-2,2 Gy'lik fraksiyonlar ile haftada beş gün ilk alan ve boost şeklinde toplam 75 Gy'ye ulaşılmıştır. İMRT tekniği ile radyoterapi uygulanan hastalara 2,2-2,4 Gy'lik fraksiyonlar ile haftada beş gün toplam 75 (sınırlar 72,6-78,8) Gy'ye uygulanmıştır. Ortanca izlem süresi 16 ay (sınırlar 6-30) idi. Tedavi surecinde akut ve subakut yüksek gradlı toksisite görülmedi. İMRT uygulanan hastalarda konformal radyoterapiye göre mesane V65%, V72-75% (p=0,001) ve rektum V65% (p=0,001), V73-75% (p=0,004) yüksek doz alan volümleri ve genitoüriner sistem yan etkileri istatiksel anlamlı olarak daha az bulundu (p=0,03). Takiplerde hastaların hiç birinde biyokimyasal rekürrens görülmedi. Sonuçta, IMRT uyguladığımız hastalarda uygulanan yüksek doz radyoterapiye rağmen daha az akut yan etki gözlendi.

Evalution of IMRT and Conformal Radiotherapy Results in Terms of Acute and Subacute Side Effects in Patients With Locally Advance Prostate Cancer In this study, the aim is to evaluate side effects in patients with locally advanced prostate cancer, in terms of the results of conformal radiotherapy and IMRT (Intensity Modulated Radiotherapy). In between 01/01/2007 - 30/06/2013, 32 cases who applied to Uludag University Medical Faculty and underwent curative radiotherapy were retrospectively evaluated. All cases are male and the median age (interval 48-82) was 75. All patients had radiotherapy to the prostate and pelvic lymph nodes with long term maximum androgen blockade (MAB). In 3D CRT (3D Confrmal Radiotherapy), patients treated total RT (radiotherapy) dose of 75 Gy (limits 72-75) with the fraction doses of 1,8-2,2 Gy at five days a week, starting first treatment after plus boost. At IMRT technique, total of median 75 Gy (limits 72.6–78.8) radiotherapy dose given five days a week with the fraction doses of 2,2-2,4 Gy. The median follow-up time was 16 (limits 6-30) months. In the process of treatment high-grade acute and subacute toxicity was not observed. In IMRT patients, high dose regions V65%, V72-75% (p=0.001) of the bladder and V65 (p=0.001), V73-75% (p=0.004) volume of rectum and genitourinary side effects were found to be statistically (p=0.03) less significant compared to conformal radiotherapy. Biochemical recurrence was not observed in any of the patients at follow up. As a result, despite the high dose radiotherapy, less side effects were observed in patients treated with IMRT.
