Acil servise kafa travması ile başvuran pediyatrik hastalarda MPV ve nötrofil lenfosit oranının prognoza etkilerinin retrospektif olarak değerlendirilmesi

Abstract

Travmatik beyin hasarı çocuk hasta grubunda acil servis başvurularının en sık nedenidir ve dünya çapında mortalite ve morbiditenin önde gelen sebeplerinden biridir. Bu hasarın şiddetini belirlemek için skorlama sistemleri, görüntüleme yöntemleri ve biyobelirteçlerden faydalanılır. Çalışmamızın amacı çoğu kafa travmalı çocuk hastada ölçülen MPV ve NLO değerlerinin, bu hastaların prognozu konusunda fikir sahibi olunmasını sağlayıp sağlayamayacağını ortaya koymaktır. Çalışmamız Bursa Uludağ Üniversitesi Tıp Fakültesi Acil Servisine 1 Ocak 2019- 1 Ekim 2023 tarihleri arasında kafa travması ile başvuran pediyatrik yaş grubu kafa travması olan 220 hastanın klinik, laboratuvar ve radyolojik sonuçlarının hastane bilgi işlem sistemi üzerinden retrospektif olarak taranması yoluyla yapılmıştır. MPV ve BT’de saptanan patolojiler arasında anlamlı ilişki saptanmamakla birlikte yaş, cinsiyet, NLO ve MPV birlikte değerlendirildiğinde yalnız MPV değerindeki artışla klinik yatış ve taburculuk arasında istatistiksel olarak anlamlı oranda negatif korelasyon izlenmiştir. Yoğun bakıma yatırılma durumunu belirlemede NLO ve MPV değerlerinin istatistiksel açıdan anlamlı seviyede ilişkili olduğu fakat bu iki değerin de duyarlılık ve özgüllüğünün düşük olduğu bulunmuştur. Çalışmamız sonucunda TBH’li pediatrik hastalarda MPV ve NLO değerlerinin birlikte değerlendirilmesinin hastaların prognozunu belirlemede faydalı olabileceği ortaya koyulmuştur.

Traumatic brain injury is the most common reason for emergency department visits in pediatric patients and is one of the leading causes of mortality and morbidity worldwide. Scoring systems, imaging methods, and biomarkers are used to determine the severity of this injury. The aim of our study is to determine whether MPV and NLR values measured in most pediatric patients with head trauma can provide insight into the prognosis of these patients. Our study was conducted by retrospectively reviewing the clinical, laboratory, and radiological results of 220 pediatric patients with head trauma who presented to the Emergency Department of Bursa Uludağ University Faculty of Medicine between January 1, 2019, and October 1, 2023, through the hospital information system. Although no significant relationship was found between MPV and pathologies detected on CT, a statistically significant negative correlation was observed between clinical admission and discharge when age, gender, NLR, and MPV were evaluated together, particularly with the increase in MPV alone. It was found that NLR and MPV values were statistically significantly associated with determining the need for intensive care unit admission, but the sensitivity and specificity of these two values were low. As a result of our study, it was concluded that the combined evaluation of MPV and NLR values in pediatric patients with TBI could be useful in determining the prognosis of the patients.
