Total tiroidektomi sonrası erken dönem kalsiyum infüzyonunun kan parathormon seviyelerine etkisi

Abstract

Tiroidektomi sonrası hipokalsemi ve hipoparatiroidizm, sık karşılaşılan komplikasyonlardandır. Bu durumun hangi hastalarda gelişeceği ise cerrahların tartışma konularından biri olmuştur. Bu nedenle; hastalara ameliyat sonrası dönemde, hipokalsemi gelişmesini önlemek amacıyla profilaktik kalsiyum infüzyonları yapılmaktadır. Çalışmamızda total tiroidektomi sonrası erken dönem kalsiyum infüzyonunun, hipokalsemi gelişmeyen olgularda, kan parathormon seviyelerine etkisini araştırdık. Bu amaçla; çalışma için belirlenmiş kriterlere sahip, ardışık tiroidektomi yapılan ve hipokalsemi gelişmeyen 45 hastanın verileri incelendi. Hastalar, ameliyat sonrası kalsiyum infüzyonu yapılmayanlar (Grup 1), ilk 2 saat içinde profilaktik kalsiyum infüzyonu yapılanlar (Grup 2), 12 saat içinde profilaktik kalsiyum infüzyonu yapılanlar (Grup 3) olmak üzere 3 gruba ayrıldı. İnfüzyonda 2 ampul kalsiyum (Kalsiyum Glukonat Monohidrat 225 mg/10ml, Kalsiyum Levulinat Dihitrat 572 mg/10ml) kullanıldı. Ameliyat sonrası ilk 2 saat, 6-12. saat arası, 12-24. saat arası ve 1. ayda ölçülen kan kalsiyum, fosfor ve parathormon (PTH) değerleri incelendi. Bu ölçümlerin, ameliyat öncesi ölçümlere göre yüzde değişim (YD) değerleri, gruplar arası karşılaştırıldığında anlamlı farklılık olmadığı görüldü (p>0,05). Parathormon ve kalsiyum için hesaplanan YD değerleri grup içi karşılaştırıldığında, ameliyat sonrası ilk 2 saat ve 6-12. saat arasında azalma olmakla birlikte, anlamlı farklılık saptanmadı (p>0,05). Grup 1 ve 3' te; parathormon ve kalsiyumun; 12-24. saatteki YD değerleri arasında anlamlı ilişki saptandı (sırasıyla r=0,58, r=0,52 ve p<0,05). Yaptığımız çalışmada, total tiroidektomi yapılan ve hipokalsemi gelişmeyen olgularda ameliyat sonrası erken dönem profilaktik kalsiyum infüzyonunun, PTH' nun YD değerine anlamlı etkisi olmadığını, PTH' nun 12-24. saat ve 1. aydaki değerlerinde artış yönünde anlamlılık saptandığını gördük.

The Effects of Early Calcium Infusion After Total Thyroidectomy on Blood Levels of Parathormone Hypocalcemia and hypoparathyroidism are common complications after thyroidectomy. There are debates among surgeons about which patients will suffer from these conditions. Therefore, prophylactic calcium infusions are given to patients in postoperative period for preventing development of hypocalcemia. In this study, we evaluated the effects of early calcium infusion after total thyroidectomy on the blood parathormone (PTH) levels in cases without hypocalcemia development. For this purpose; data of 45 consecutive patients who were undergone thyroidectomy, had established criteria for study and did not develop hypocalcemia were evaluated. Patients were divided into following 3 groups: patients who did not received postoperative calcium infusion (Group 1), patients who received prophylactic calcium in first two postoperative hours (Group 2), patients who received prophylactic calcium in first 12 postoperative hours (Group 3). 2 ampoule of calcium (Calcium Gluconate Monohydrate 225 mg/10 ml, Calcium Levulinate Dihydrate 572 mg/10 ml) were used in infusion. Blood calcium, phosporus and parathormone levels were measured at postoperative 2nd hours, between 6-12th hours, between 12-24th hours and 1st month. When postoperative measurements in different hours were compared with preoperative measurements, there were no significant differences between groups in terms of percentage change (PC) values (p>0,05). When calculated PC values of parathormone and calcium were compared within the group, there were no significant differences although there were reductions in postoperative 2nd hour and between 6-12th hours (p>0,05). There were significant relationships in PC values of parathormone and calcium at between v postoperative 6-12th hours in Group 1 and Group 3 (r=0,58, r=0,52 and p<0,05; respectively). In our study, we observed that early prophylactic calcium infusion in postoperative period to patients undergone total thyroidectomy and did not develop hypocalcemia has no significant effect on PC value of PTH and there was significance towards increase in PTH values at 12-24th hours and 1st month.
