Abstract
Bu çalışmada, cranial cruciate ligament transeksiyonu ile oluşturulan diz osteoartritis modelinde, tıbbi ozon, hiyaluronik asit ve ozon-hiyaluronik asit kombinasyonunun intraartiküler uygulamaları ile sistemik ozon tedavisinin karşılaştırmalı etkileri araştırıldı. Çalışmada, 60 dişi Sprague Dawley ırkı ratın sağ dizine cranial cruciate ligament transeksiyonu uygulandı. Altı gruba ayrılan deneklere cerrahi işlem sonrası 2. haftadan başlayarak haftada bir kez, toplam beş hafta süren tedavi protokolü uygulandı. Radyolojik değerlendirme amacıyla 2. ve 8. haftalarda bilgisayarlı tomografi görüntüleri alındı. Histopatolojik olarak Modifiye Mankin skoru, kıkırdak matriks kaybı ve kıkırdak dejenerasyon skoru, ayrıca immünohistokimyasal olarak kollajen tip II ve matriks metalloproteinaz-13 ekspresyonu incelendi. Serolojik değerlendirmede TNF-α düzeyleri ELISA ile ölçüldü. Sekizinci haftada, intraartiküler ozon ve ozon-hiyaluronik asit kombinasyonu gruplarında Mankin skorlarının anlamlı şekilde düşük olduğu belirlendi (p< 0,05). Kıkırdak matriks kaybı ve kıkırdak dejenerasyon skorları da ozon uygulanan gruplarda daha düşük bulundu. İntraartiküler ozon uygulamasının, sistemik uygulamaya kıyasla daha etkili sonuçlar verdiği belirlendi. Kollajen tip II düzeyleri de intraperitoneal kontrol grubuna kıyasla intraartiküler ozon ve ozon- hiyaluronik asit kombinasyonu gruplarında anlamlı şekilde arttı (p= 0,010). Elde edilen bulgular, özellikle hiyaluronik asit ile kombine edilen ozon tedavisinin osteoartritis’te belirgin iyileştirici etkiler sağladığını göstermektedir. Bu kombine tedavi şekli, CrCL transeksiyonu ile indüklenen OA modelinde kapsamlı olarak değerlendirilen ilk çalışmalardan biri olup, ozon temelli yaklaşımların terapötik potansiyelini ortaya koymaktadır.
In this study, the comparative effects of intra-articular administration of medical ozone, hyaluronic acid, and ozon-hyaluronic acid combination, as well as systemic ozone treatment, were investigated in a rat model of knee osteoarthritis induced by cranial cruciate ligament transection. Cranial cruciate ligament transection was performed on the right knee of 60 female Sprague Dawley rats. The animals were divided into six groups and received weekly treatments for five weeks, starting from the second week after surgery. Computed tomography images were obtained at Weeks 2 and 8 for radiological evaluation. Histopathological analyses included Modified Mankin scoring, cartilage matrix loss, and cartilage degeneration scoring. Immunohistochemical assessments were conducted for type II collagen and matrix metalloproteinase-13 expression. Serum TNF-α levels were measured by ELISA for serological evaluation. At week 8, significantly lower Mankin scores were observed in the intra-articular ozone and ozone-hyaluronic acid combination groups (p< 0.05). Cartilage matrix loss and zonal degeneration scores were found to be lower in the ozone-treated groups. Intra-articular ozone therapy yielded more effective outcomes compared to systemic administration. Type II collagen expression was also significantly increased in the intra-articular ozone and ozone-HA combination groups compared to the intraperitoneal control group (p= 0.010). The findings indicate that ozone therapy, particularly when combined with hyaluronic acid, provides significant therapeutic benefits in osteoarthritis. This combined treatment approach represents one of the first comprehensive evaluations in a CrCL transection-induced OA model, highlighting the therapeutic potential of ozone-based strategies.
