Çalışmamızda, Bursa Uludağ Üniversitesi Tıp Fakültesi Çocuk Endokrinoloji bölümüne 1 Ocak 2019 – 31 Aralık 2022 tarihleri arasında başvuran ve obezite nedeniyle metformin tedavisi alan hastaların kayıtları incelenmiş, tedavi öncesi ve tedavinin 6. ayında antropometrik ve laboratuvar parametreleri karşılaştırılmıştır. Olguların ortalama yaşı 13,7 ve %57,4'ü kadındı. En sık başvuru şekli (%82,9) kilo fazlalığıydı. Fizik muayenelerinde hastaların %87,2'sinde akantozis nigrikans (AN) mevcuttu. Çalışmamızda; en az 6 ay süreyle metformin tedavisi kullanan hastaların, 6. ayın sonunda tedavi başlangıcına göre; vücut ağırlığı standart deviasyon skoru (SDS), vücut kitle indeksi (VKİ) ve VKİ SDS değerlerinde istatistiksel olarak anlamlı (p<0,001) düşüş saptanmış ve önemli bir yan etki gözlenmemiştir. Çalışmada yine; ast, alt, Homeostasis Model Assestment of Insülin Resistance (HOMA-IR) değerlerinde anlamlı değişiklikler saptanmıştır. (sırasıyla p değerleri : 0,012, 0,008, 0,004) Sonuç olarak, insülin direnci (İR) olup metformin tedavisi kullanan hastaların 6. ayında metabolik parametreleri iyileşmiş, antropemetrik ölçümlerinde anlamlı sonuçlar alınmıştır. Ancak metforminin hastalar üzerindeki uzun dönemdeki etkilerini incelemelek için, hastalar daha uzun süre incelenmeli, daha fazla çalışma yapılmalıdır.
In our study, records of patients who applied to the Department of Pediatric Endocrinology of Bursa Uludağ University Faculty of Medicine between January 1, 2019, and December 31, 2022, due to obesity and received metformin treatment were examined, and anthropometric and laboratory parameters were compared before treatment and at the 6th month of treatment. The mean age of the cases was 13.7 and 57.4% of them were female. The most common reason for application (82.9%) was excess weight. Acanthosis was present in 87.2% of the patients on physical examination. In our study; statistically significant (p<0.001) decreases were detected in the body weight standard deviation score (SDS), body mass index (BMI), and BMI SDS values of patients using metformin treatment for at least 6 months compared to the start of treatment, and no significant side effects were observed. Significant changes were also observed in AST, ALT and Homeostasis Model Assestment of Insülin Resistance (HOMA-IR) values in the study (p values: 0.012, 0.008, 0.004 respectively). In conclusion, metabolic parameters improved in patients with insulin resistance (IR) using metformin treatment at the 6th month, and significant results were obtained in anthropometric measurements. However, in order to examine the long-term effects of metformin, patients should be examined for a longer period, and more studies should be conducted.
