AMAÇ: Preterm erken membran rüptüründe antibiyotik profilaksisinin latent peryod ve enfeksiyöz morbiditeye etkisini araştırmak. GEREÇ VE YÖNTEM: Çalışmaya 1996-2000 yılları arasında takip edilen 24 - 34 haftalık preterm EMR' li 80 gebe alındı. 39 gebeye profilaktik ampisilin verilirken, 41 gebeye antibiyotik verilmedi. BULGULAR: Antibiyotik alan grupta latent peryod daha uzun olmakla beraber, latent peryod üzerine etkili olan diğer faktörlerle beraber değerlendirildiğinde gebelik haftası, servikal açıklık, preterm erken membran rüptürü ile ilk vajinal tuşe arasındaki süre ve tuşe sayısının, latent peryodun süresi üzerine etkili olduğu, antibiyotik kullanımının ise bağımsız etkisinin olmadığı görüldü. Antibiyotik kullanımı servikal açıklığı 2 cm ve daha fazla olan gebelerde latent peryodu uzatırken, servikal açıklığı 2 cm'den az olan gebelerde etkili değildi. Klinik koryoamnionit, postpartum endometrit, yenidoğan sepsisi ve mortalitesi açısından iki grup arasında farklılık bulunmadı. SONUÇ: Preterm erken membran rüptürü olgularında ampisilin profilaksisi ile latent peryod ve enfeksiyöz morbidite insidansında azalma tespit edilemedi.
OBJECTIVE: Evaluation of the effect of antibiotic prophylaxis on the latent period and infectious morbidity in preterm premature rupture of the membranes. MATERIALS & METHODS: Eighty patients with preterm premature rupture of the membranes at 24-34 weeks of gestation who were followed up at the Uludag University Obstetrics&Gynecology Clinic during 1996-2000 were analysed. Ampicilin prophylaxis was given to the treatment group. The control group was followed without antibiotherapy. RESULT: While the treatment group had a longer mean latent period than the control group, controlling for confounding factors indicated that gestational age, cervical dilatation, the time of the first pelvic examination after rupture of the membranes, the number of vaginal examinations perfomed had some effect on latency, but antibiotic prophylaxis had no independent effect. In the treatment group, antibiotic use was effective in extending the latent period in the group of patients with a cervical dilatation of 2 cm or more, but was ineffective in patients with a dilatation of less than 2 cm. There was no significant differences between the groups with regard to chorioamnionitis, postpartum endometritis or newborn sepsis or mortality. CONCLUSION: There is no effect of ampicillin prophylaxis on the latent period or infectious morbidity, in preterm premature rupture of the membranes.
