Baş ve boyun bölgesindeki kitlelerde; intralezyonel bleomisin tedavisi, hacim azaltmada etkili yöntem olarak cerrahiye alternatif bir tedavi seçeneği olarak giderek daha fazla tercih edilmektedir. Bu tedavi; özellikle cerrahinin riskli veya komplikasyon oranının yüksek olduğu durumlarda, minimal invaziv bir seçenek sunarak hastalara önemli avantajlar sağlamaktadır. Çalışmamızın amacı, baş ve boyun bölgesindeki kitlelerde uygulanan intralezyonel bleomisin tedavisinin etkinliğini ve güvenilirliğini değerlendirmektir. Bu çalışmaya, baş ve boyun bölgesinde kitle bulunan 19 hasta (12 kadın, 7 erkek) dahil edildi. Hastaların yaşları 3-57 arasında değişmektedir. Yaş ortalaması 18,79 ± 13,16 yıl olarak hesaplandı. Tüm işlemler, hastaların klinik durumlarına göre lokal veya genel anestezi altında USG kılavuzluğunda yapıldı. Lezyonların boyutları takip sürecinde MRG veya USG ile değerlendirildi. Takip süresi boyunca hastalarda ciddi komplikasyon gözlenmedi. Tedavi sonucunda kitlelerin hacminde belirgin azalma sağlandı, bu da bası semptomlarının ortadan kalkmasına ve hastaların semptomatik olarak rahatlamasına katkıda bulundu. Ayrıca, kozmetik açıdan da gözle görülür iyileşme elde edildi. Tedaviye verilen yanıt oranları %11,11 ile %98,41 arasında değişmiş olup hastaların %42,11’inde mükemmel, %5,26’sında iyi, %15,79’unda makul ve %36,84’ünde yetersiz sonuçlar elde edildi. Tedavi yanıt oranı ile hastaya uygulanan bleomisin tedavi seans sayısı, toplamda kullanılan ajan miktarı, takip süresi, hasta yaşı arasında anlamlı istatiksel fark saptanmadı. Ayrıca, takip süresi ile seans sayısı arasında da anlamlı fark gözlemlenmedi. Bu sonuçlar, intralezyonel bleomisin tedavisinin özellikle cerrahiye uygun olmayan veya cerrahiyi tercih etmeyen hastalarda etkili ve güvenli seçenek olduğunu göstermektedir. Gelecekte daha geniş hasta gruplarıyla yapılacak çalışmalar, bu tedavinin uzun vadeli etkinliği ve güvenilirliği konusunda daha ayrıntılı veriler sağlayacaktır.
In cases of masses in the head and neck region, intralesional bleomycin therapy is increasingly being preferred as an effective method for volume reduction and an alternative treatment option to surgery. This treatment provides significant advantages to patients, particularly in situations where surgery is risky or has a high complication rate, by offering a minimally invasive option. The aim of our study is to evaluate the efficacy and safety of intralesional bleomycin therapy in the treatment of masses in the head and neck region. This study included 19 patients (12 female, 7 male) with masses in the head and neck region. The ages of the patients ranged from 3 to 57 years, with a mean age of 18.79 ± 13.16 years. All procedures were performed under either local or general anesthesia, depending on the patient's clinical condition, with ultrasound guidance. The sizes of the lesions were evaluated during the follow-up period using MRI or ultrasound. No serious complications were observed during the follow-up period. Significant reduction in the volume of the masses was achieved, contributing to the relief of pressure symptoms and symptomatic improvement in the patients. Additionally, noticeable cosmetic improvement was observed. The response rates to treatment ranged from 11.11% to 98.41%, with 42.11% of patients showing excellent results, 5.26% showing good results, 15.79% showing reasonable results, and 36.84% showing insufficient results. No statistically significant differences were found between the treatment response rate and the number of bleomycin treatment sessions, the total amount of agent used, the follow-up period, or the patient's age. Furthermore, no significant difference was observed between the follow-up period and the number of sessions. These results demonstrate that intralesional bleomycin therapy is an effective and safe option, particularly for patients who are not suitable for surgery or who prefer not to undergo surgery. Future studies with larger patient groups will provide more detailed data on the long-term efficacy and safety of this treatment.
