Özet
Amaç: MikroRNA’ların hücresel süreçlerin düzenlenmesine katıldığı, deregülasyonu halinde kanser dahil olmak üzere çeşitli hastalıklarla ilişkili olduğu bilinmektedir. Çalışmamızda pankreas kanserinde lokal nüks, uzak organ metastazı ve sağkalım dahil klinikopatolojik parametreler ile ilişkili olabilecek miRNA’ların tanımlanması amaçlanmaktadır. Gereç ve Yöntem: Çalışmada, 2012-2022 yılları arasında Bursa Uludağ Üniversitesi Tıp Fakültesi Tıbbi Onkoloji Anabilim Dalı’na başvuran, adjuvan kemoterapi almış, pankreas kanseri 50 hastaya ait tümör doku örnekleri ve ilgili hastaların 21’inin bitişik normal doku örneklerinde 10 mikroRNA'nın (miRNA) RT-qPCR yöntemiyle ekspresyon analizleri gerçekleştirildi. Ekspresyon düzeylerinin değerlendirilmesinde katlık artış değerleri kullanıldı. Verilerin değerlendirilmesinde Ki-Kare, Student T-Testi, One-Way Anova Testi, Mann Whitney U Testi, binary lojistik regresyon analizi, kaplan meier testi ve Cox regresyon analizi kullanıldı. Bulgular: Pankreas kanseri dokusunda miRNA’lardan let-7c, miR-27a, miR29b, miR-31, miR-34a, miR-143, miR-200c’nın aşağı regüle olduğu, miR10b’nin ise yukarı regüle olduğu saptandı (p<0,05). Perinöral invazyon pozitifliğinin [OR:0,12; %95GA (0,019-0,763)] ve miR-143 yüksek ekspresyonlu grupta olmanın [OR:4,1; %95GA (1,094-15,674)] lokal nüksü etkilediği saptandı (p<0,05). Tümör tipinin adenoskuamöz olması [HR: 4,107; %95GA (1,612-10,763)] ve miR-29b yüksek ekspresyon seviyelerine sahip grupta olmanın [HR: 1,992; %95GA (1,083-3,663)] düşük hastalıksız sağkalımla ilişkili olduğu saptandı. Tümör tipinin adenoskuamöz olması [HR: 5,102 kat (1,820-14,302)] ise düşük genel sağkalımla ilişkili olduğu saptandı. Sonuç: Bu bulguların ışığında, miR-143’nın pankreas kanseri hastalarında gelişebilecek lokal nüks süreci değerlendirmede yol gösterici olabileceği düşünülmektedir. Ayrıca miR-29b’nin ekspresyon seviyelerinin artmasının hastalıksız sağkalımı olumsuz etkilemektedir. Sonuç olarak, miRNA’ların iv ekspresyon farklılıklarının pankreas kanseri prognozunda etkili olabileceği düşünülmekle birlikte bu konuda daha fazla sayıda ve uzun takipli çalışmalara da ihtiyaç duyulmaktadır.
Purpose: It is known that microRNAs participate in the regulation of cellular processes and that their deregulation is associated with various diseases, including cancer. Our study aims to identify miRNAs that may be associated with clinicopathological parameters, including local recurrence, distant organ metastasis, and survival in pancreatic cancer. Materials and Methods: In this study, expression analyses of 10 miRNAs were performed using the RT-qPCR method on tumor tissue samples from 50 pancreatic cancer patients who received adjuvant chemotherapy and were admitted to the Department of Medical Oncology at Bursa Uludağ University Faculty of Medicine between 2012 and 2022, as well as on adjacent normal tissue samples from 21 of these patients. Fold change values were used to evaluate expression levels. For data analysis, Chi-Square Test, Student’s ttest, One-Way ANOVA, Mann-Whitney U Test, binary logistic regression analysis, Kaplan-Meier test, and Cox regression analysis were utilized. Results: In pancreatic cancer tissue, microRNAs let-7c, miR-27a, miR-29b, miR-31, miR-34a, miR-143, miR-200c were down-regulated, while miR-10b was up-regulated (p<0.05). Perineural invasion positivity [OR: 0.12; 95% CI (0.019-0.763)] and being in the miR-143 high expression group [OR: 4.1; 95% CI (1.094-15.674)] were found to affect local recurrence (p<0.05). Tumor type was adenosquamous [HR: 4.107; [95%CI (1.612-10.763)] and being in the group with high expression levels of miR-29b [HR: 1.992; 95%CI (1.083- 3.663)] were found to be associated with lower disease-free survival. And It was found that the tumor type being adenosquamous [HR: 5.102 times (1.820- 14.302)] was associated with lower overall survival. vi Conclusion: In light of these findings, it is thought that miR-143 may be a guide in evaluating the local recurrence process that may develop in pancreatic cancer patients. In addition, increased expression levels of miR29b negatively affect disease-free survival. In conclusion, it is thought that the expression differences of miRNAs may be effective in pancreatic cancer prognosis, and more studies with longer follow-up are needed on this subject.
