Özet
Amaç: Mikrobiyal keratitli hastalarda standart antimikrobiyal tedaviye ek amniyotik membran transplantasyonunun (AMT) etkinliğinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Bu retrospektif çalışma, Ocak 2022 - Ocak 2025 tarihleri arasında Bursa Uludağ Üniversitesi Göz Hastalıkları Kliniği’nde yatarak takip edilen ve mikrobiyal keratit tanısı alan 33 hastanın dosya kayıtları üzerinden yürütüldü. Enfeksiyonu kontrol altına almak için antimikrobiyal tedaviye ek olarak erken dönemde AMT uygulanan 14 hasta çalışma grubunu, yalnızca antimikrobiyal tedavi ile izlenen 19 hasta kontrol grubunu oluşturdu. Tüm hastaların demografik verileri, risk faktörleri, kültür sonuçları, yatarak takip süresi, topikal antibiyotik kullanım süresi, görme keskinlikleri, ön segment görüntüleme ile elde edilen fotoğraflar üzerinden kornea opaklığı, kornea neovasküarizasyon alanı ve neovasküler damar büyüklüğü değerlendirildi. Elde edilen veriler, iki grup arasında istatistiksel olarak karşılaştırıldı ve p<0,05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Çalışma ve kontrol grupları arasında yaş, cinsiyet, tutulan göz tarafı ve ilk başvuru anındaki keratit tutulum oranı istatistiksel olarak benzerdi (p>0,05). En sık saptanan risk faktörleri kontakt lens kullanımı ve organik travmaydı. Kültürlerin %54,5’inde etken izole edildi. Her iki grupta da tedavi sonrası görme keskinliğinde anlamlı iyileşme izlense de iki grup arasında son görme keskinlikleri açısından anlamlı bir fark gözlenmedi. Neovaskülarizasyon alanı, AMT uygulanan grupta anlamlı şekilde daha düşük bulundu (p=0.038); diğer parametrelerde iki grup arasında anlamlı fark saptanmadı. Ayrıca çalışma grubunda ön segment optik koherens tomografi (ÖS-OKT) ölçümlerde keratit tedavi sürecinde 1.hafta ve 3.ayda lezyon bölgesindeki kornea kalınlığı (LBKK) ve infiltrasyon derinliği (İD) anlamlı düzeyde azaldı. iii Sonuç: Elde edilen bulgular, mikrobiyal keratit tedavisinde antimikrobiyal tedaviye ek olarak AMT’nin keratit sonrası gelişen neovaskülarizasyonu sınırlamada katkı sağladığını göstermektedir. ÖS-OKT cihazı, mikrobiyal keratitte stromal ödemi, korneal infiltratlarının boyutunu ve derinliğini belirleyerek, hastalığın seyrinin objektif olarak izlenmesine ve nicel veriler elde edilmesine olanak sağlayabilir.
Objective: This study aims to investigate the efficacy of adding amniotic membrane transplantation (AMT) to standard antimicrobial therapy in patients with microbial keratitis. Materials and Methods: This retrospective study was conducted using the medical records of 33 patients diagnosed with microbial keratitis who were hospitalized at the Department of Ophthalmology, Bursa Uludağ University, between January 2022 and January 2025. Fourteen patients who received early AMT in addition to antimicrobial therapy comprised the study group, while 19 patients treated with antimicrobial therapy alone formed the control group. Demographic data, risk factors, culture results, duration of hospitalization and topical antibiotic use, visual acuity, and anterior segment images were evaluated. From the images, corneal opacity, corneal neovascularization area, and neovascular vessel diameter were assessed. The data were statistically compared between the two groups, and a p-value of <0.05 was considered statistically significant. Results: The study and control groups were statistically similar in terms of age, sex, affected eye laterality, and initial keratitis involvement rate (p>0.05). The most commonly identified risk factors were contact lens use and organic trauma. A causative organism was isolated in 54.5% of cultures. Although both groups showed significant improvement in visual acuity after treatment, there was no statistically significant difference in final visual acuity between the two groups. The area of neovascularization was found to be v significantly lower in the AMT group (p=0.038), while no significant differences were observed between the groups for other parameters. Additionally, in the study group, anterior segment optical coherence tomography (AS-OCT) measurements showed a significant reduction in lesion-based corneal thickness (LBCT) and infiltration depth (ID) at both the 1st week and 3rd month of the treatment period. Conclusion: The findings indicate that adding AMT to antimicrobial therapy contributes to limiting post-keratitis neovascularization. The AS-OCT device provides objective monitoring of disease progression in microbial keratitis by assessing stromal edema, as well as the size and depth of corneal infiltrates, thus enabling the collection of quantitative data.
