Özet
Amaç: Bu çalışma, Bursa Uludağ Üniversitesi Tıp Fakültesi Çocuk Beslenme ve Metabolizma Bilim Dalı’nda tanı alan ya da takibe giren fenilketonüri (FKU) hastalarının, klinik özellikleri ve beyin volümetrik ölçümleri arasındaki ilişkinin değerlendirilmesi amaçmıştır. Gereç ve yöntem: Çalışma 72 FKU tanılı hasta üzerinde kesitsel, retrospektif olarak planlandı. Hastaların demografik verileri, fizik muayene bulguları, genetik mutasyonları, izlem süreleri, izlem süreleri boyunca ortalama fenilalanin düzeyleri veri olarak alındı. Hastaların görsel uyarılmış potansiyel (VEP=Visual evoked potential) ve beyin sapı işitsel uyarılmış potansiyel (BAEP=Brainstem auditory evoked potential) verileri dosyalarından elde edildi. Daha öncesinde Kranial MRG (Manyetik Rezonans Görüntüleme) çekilen 27 hastanın ve kontrol grubu olarak 35 hastanın beyin volümetrik ölçümleri karşılaştırıldı. Bulgular: Hastaların ortalama fenilalanin değeri 408,32±183,87 μmol/L, ortalama FA/Tirozin değeri 25,63±13,27 ve ortalama albumin değeri 45,64±3,39 gr/L olarak bulunmuştur. Hastaların %26,40’ının nörolojik bulgusunun olduğu saptanmıştır. Sağ ve sol hemisfer ve total beyin volümü açısından çalışma grupları arasında anlamlı farklılık olmadığı belirlenmiştir. MRG beyin volümleri ile fenilalanin ve FA/tirozin oranı arasında anlamlı bir ilişki bulunamamıştır. VEP testi sonucu anormal olan hasta grubunda medyan fenilalanin düzeyinin daha yüksek olduğu saptanmıştır. Sonuç: FKU hastalarında fenilalanin düzeyi beyin volümünden bağımsız olarak VEP testi anormalliklerine yol açabilir.
Objective: This study aimed to evaluate the relationship between clinical features and brain volumetric measurements of PKU patients diagnosed or followed up at Bursa Uludağ University, Faculty of Medicine, Department of Pediatric Nutrition and Metabolism. Materials and methods: The study was planned as a cross-sectional retrospective study on 72 patients diagnosed with PKU. Demographic data of the patients, physical examination, genetic changes, follow-up programs, and mean phenylalanine/tyrosine ratios during follow-up programs were taken as data. Visual evoked potential (VEP) and brainstem auditory evoked potentials (BAEP) were performed on the patients. Brain volumetric measurements of 27 patients who had previously undergone Cranial MRI (Magnetic Resonance Imaging) and 35 patients as a control group were compared. Results: The mean phenylalanine value of the patients was found to be 408.32±183.87 μmol/L, the mean FA/Tyrosine value was found to be 25.63±13.27 and the mean albumin value was found to be 45.64±3.39 gr/L. It was determined that 26.40% of the patients had neurological findings. There was no significant difference between the study groups in terms of right and left hemisphere and total brain volume. No significant relationship was found between MRI brain volumes and phenylalanine and FA/tyrosine ratio elevation. The median phenylalanine level was higher in the patient group with abnormal VEP test results. Conclusion: In patients with PKU, phenylalanine levels may lead to VEP test abnormalities independent of brain volume.
