Gastrointestinal kanser cerrahisi geçirmiş hastaların beslenme durumlarının, gastrointestinal semptomlarının ve yaşam kalitelerinin değerlendirilmesi

Abstract

Bu çalışma, gastrointestinal kanser cerrahisi geçirmiş hastaların beslenme durumu, gastrointestinal semptomları ve yaşam kalitelerinin değerlendirilmesi amacıyla gerçekleştirilmiştir. Tanımlayıcı ve kesitsel nitelikteki araştırma, Ali Osman Sönmez Onkoloji Hastanesi’nde 121 hasta ile yürütülmüştür. Veriler; hasta bilgi formu, Nütrisyonel Risk Skoru, Gastrointestinal Semptom Derecelendirme Ölçeği ve Gastrointestinal Yaşam Kalitesi İndeksi kullanılarak yüz yüze görüşmelerle toplanmış ve SPSS 28.0 programında analiz edilmiştir. Katılımcıların %62,81’i erkek, yaş ortalaması 62,80±10,31 yıl olup, %46,22’sinde en az bir kronik hastalık mevcuttur. En sık uygulanan cerrahi girişim kolektomi ve kolon cerrahisidir (%54,55). Hastaların ortalama Gastrointestinal Yaşam Kalitesi İndeksi puanı 73,97±20,58, Gastrointestinal Semptom Derecelendirme Ölçeği medyan toplam puanı 33,00’dür. Nütrisyonel Risk Skoru’na göre medyan nutrisyonel risk skoru 3,00’dür. Cerrahi gruplar arasında kilo ve beden kitle indeksi açısından anlamlı fark saptanmış, kolektomi uygulanan hastalarda bu değerler daha yüksek bulunmuştur. Gastrointestinal Yaşam Kalitesi İndeksi puanı kolektomi grubunda bazı gruplara göre daha yüksektir. Gastrointestinal Semptom Derecelendirme Ölçeği - hazımsızlık alt ölçek puanı ise cerrahi türüne göre anlamlı farklılık göstermiştir. Bu çalışma, cerrahi türünün, hastaların semptom profili ve yaşam kalitesi üzerinde etkili olduğunu göstermektedir. Bu nedenle, gastrointestinal kanser cerrahisi sonrası hasta takibinde cerrahi türüne özel değerlendirme ve izlem stratejileri geliştirilmesi önerilmektedir.

This study was conducted to evaluate the nutritional status, gastrointestinal symptoms, and quality of life of patients who underwent gastrointestinal cancer surgery. This descriptive and cross-sectional study was carried out with 121 patients at Ali Osman Sönmez Oncology Hospital. Data were collected through face-to-face interviews using a patient information form, the Nutritional Risk Screening, the Gastrointestinal Symptom Rating Scale and the Gastrointestinal Quality of Life Index and analyzed using SPSS 28.0. Of the participants, 62.81% were male, with a mean age of 62.80±10.31 years; 46.22% had at least one chronic disease. The most common surgical procedure was colectomy and colon surgery (54.55%). The patients’ mean Gastrointestinal Quality of Life Index score was 73.97±20.58, and the median Gastrointestinal Symptom Rating Scale total score was 33.00. The median nutritional risk score according to Nutritional Risk Screening was 3.00. There were statistically significant differences between surgical groups in terms of weight and body mass index (BMI), with higher values in patients who underwent colectomy. The Gastrointestinal Quality of Life Index score was also significantly higher in the colectomy group compared to some other surgical groups. The Gastrointestinal Symptom Rating Scale - indigestion subscale score showed significant variation depending on the type of surgery. This study demonstrates that the type of surgery affects patients' symptom profiles and quality of life. Therefore, it is recommended to develop surgery-specific assessment and follow-up strategies for postoperative care in gastrointestinal cancer patients.
