For the first time in 2002 Fujii and his colleagues published 25 gauge (G) and in 2005 Eckart published 23 G transconjunctival sutureless vitrectomy surgical technique. The complications of these surgical techniques can be collected into two groups as intraoperative and postoperative. All of the serious complications in both groups are associated with leakage of the sclerotomies. Main complications are fluid or tamponade material leakage from sclerotomies, postoperative hypotonia, need for peroperative or postoperative suturization of sclerotomies, serous choroidal detachment, vitreus hemorrhage and endophthalmitis. As foreseen theoretically studies including large case series have shown that small incisional transconjunctival sutureless vitrectomy endophthalmitis complication is not much more than classical 20 G vitrectomy surgery. Even though this surgery is named as sutureless vitrectomy surgery, suturization of sclerotomies should not be avoided if needed.
