Coronary artery anomalies (CAAs) are a group of congenital disorders resulting in a highly variable clinical phenotype (2). There is a growing interest in the incidence of these anomalies and in their role in the manifestations of ischemic heart disease (3). To our knowledge, however, the role of structural cardiovascular abnormalities in the pathogenesis of syndrome X remains unclear. In the present study, we sought to investigate the presence and frequency of CAAs in patients with CSX. We reviewed the records of 50 patients with CSX who had undergone exercise electrocardiogram test and coronary angiography at the Uluda¤ University Cardiovascular Laboratories between 2003 and 2005. The following variables were collected: time of 1-mm ST depression, onset of angina, maximum ST segment shift, number of leads (derivations) with ST depression observed at peak exercise, and exercise duration in multiples of resting O2 consumption (METS). PDA has been reported as a most common congenital heart defect in dogs and cats, however there is limited information on diagnostic procedure of PDA by use of three-dimensional computed tomography angiography (3D-CTA) in cats[1,2]. Thus, we report here the use of 3D-CTA to confirm the clinical diagnosis of PDA in a kitten.Item Hypothyroidism leading to a syncopal episode: A reminder to check drug interactions/A case of iatrogenic hypothyroidism presented with cardioinhibitory syncope and resolved by thyroxine supplementation Reply(Türk Kardiyoloji Derneği, 2012-08) Uçar, Birsen; Kılıç, Zübeyir; Kırel, Birgül; Evim, Melike Sezgin; Uludağ Üniversitesi/Tıp Fakültesi/Pediatrik Hematoloji Anabilim Dalı.; AAH-1452-2021; 36337796600We read with great interest the article: “A case of iatrogenic hypothyroidism presented with cardio-inhibitory syncope and resolved with levothyroxine supplementation” (1). Similarly we report a case of cardio-inhibitory syncope as a result of a drug interaction from an over the counter medication. A 65-year-old female with a past medical history significant for hypothyroidism, hypertension and obesity was brought into the emergency room after experiencing a syncopal episode. Electrocardiogram revealed sinus bradycardia with heart rate-40 beats/min and findings of a new first-degree atrioventricular block (PR interval 0.24 s). Head computed tomography (CT) did not reveal any evidence of an acute hemorrhagic stroke. Carotid Doppler examinations were negative for evidence of stenosis. A complete metabolic profile and complete blood count were unrevealing as well. Thyroid function studies revealed a thyroid stimulating hormone (TSH) level of 125 mIU/mL with free thyroxine (T4) of 0.19 ng/DL. Three months prior to this episode, the patients TSH was 2.2 mIU/mL. Patient had been taking levothyroxine 100 mcg every day for the past 5 years without any prior abnormalities in her thyroid function tests. Upon further questioning it was elicited that the patient had been also taking orlistat, an over the counter medication, to help lose weight under the guidance of a primary care physician. Orlistat was held immediately. Endocrinology was consulted and the patient was restarted on her home dose of levothyroxine as she had been therapeutic on prior to the initiation of levothyroxine. At a 6 month f/up, patient’s bradycardia and first degree AV block had resolved. Her repeat thyroid functions revealed TSH of 5 miu/mL and free T4 of 0.56 ng/dL. Orlistat is a gastrointestinal lipase inhibitor that reduces absorption of dietary fat by thirty percent (2). A few cases have been described in the literature to suggest that orlistat may prevent the absorption of levothyroxine. (1) published in the Anatolia Journal of Cardiology in late 2016 with great interest. We commend the authors for their contribution to improving our understanding of sudden cardiac death mechanisms and suggesting potential reasons for occurrence of the condition of genetic origin. We do, however, have a number of thoughts about the study, which are outlined below. The authors mentioned de novo mutation in the sarcoglycan (SGCD) and titin (TTN) genes. The article fails to mention, however, the parent-based variant approach to analysis. In human genetic diseases, the term “de novo mutation” by definition refers to an alteration in a gene that is present for the first time in one family member as a result of a mutation in a germ cell of one of the parents or in the zygote itself. However, the site of stent implantation usually becomes vulnerable for endothelial damage and disturbed vasomotion that can lead to stent thrombosis. In the very interesting report published in this journal by Terzioğlu et al. (1) entitled "Kounis syndrome not induced but prevented by the implantation of a drug eluting stent: a case report.", a 65-year-old male patient with hypertension and diabetes who underwent bypass graft surgery developed an acute allergic reaction with dyspnea, itching, hand swelling, retrosternal chest pain, and syncope following paracetamol/ propyphenazone tablet administration. Type II Kounis syndrome was diagnosed, which manifested as non-STEMI. Coronary arteriography revealed a severe midportion circumflex artery stenosis, and a paclitaxel-eluting stent was implanted, which gave favorable results. However, after 10 months, the patient again inadvertently received paracetamol/propyphenazone combination and developed urticaria, angioedema, and dyspnea within 10 min. His symptoms disappeared with an antiallergic treatment.
