Abstract
Ksantogranülomatöz kolesistit safra kesesinde düzensiz duvar kalınlaşması ile karakterize kronik bir hastalıktır. Kese duvarında ciddi proliferatif fibrozis, perikolesistik infiltrasyon ve enflamasyon mevcuttur. Genellikle safra kesesi taşları ile birliktelik gösterir. En sık 60-70 yaşlarda ve çoğunlukla da kadınlarda görülür. Taşa bağlı tekrarlayan iltihabi atakların ve bilier stazın etyolojide rol oynadığı düşünülmektedir. Ksantogranülomatöz kolesistit, yapısı ve makroskopik görüntüsü ile safra kesesi malignitelerini taklit edebilir. Ameliyat öncesi veya ameliyat sırasında doğru tanının konulması oldukça önemlidir. Bu durum, hastayı gereksiz bir majör cerrahiden ve bunun sonucunda oluşabilecek komplikasyonlardan koruyacaktır. Kesin tanı histopatolojik inceleme ile konur. Bu yazıda, taşlı kolesistit ön tanısıyla ameliyata alınan, intraoperatif ayırıcı tanıda güçlük çekilen ve postoperatif histopatolojik inceleme ile kesin tanı alan bir olgu, literatür eşliğinde irdelendi.
Xanthogranulomatous cholecystitis is a chronic disease characterized with irregular wall thickening of the gallbladder. Severe proliferative fibrosis, pericholecystic infiltration and inflammation are seen on the gallbladder. This disease usually appears together with cholelithiasis, mostly at the age of 60-70 and mainly seen on female patients. Recurrent inflammatory attacks caused by stones and biliar stasis may take place in the etiology. Due to its presence and macroscopic look xanthogranulomatous cholecystitis can be mixed easily with gallbladder malignancies. Before or during surgery it is of high importance to make the right diagnosis so that the patient is saved from unnecessary major surgeries and their complications afterwards. Final diagnosis is established with histopathological results. This article reports along with literature about a case with the prediagnosis cholelithiasis, but difficulty in the differential diagnosis intraoperative, and after this the postoperative final diagnosis with histopathological examination.
