Abstract
Bu çalışmada, supratentoryel lokalizasyonlu serebral infarkt ve kanama olgularında, erken dönemde görülen epileplik nöbetlerin tipi ve sıklığı, lezyon lokalizasyonu ve nöbetlerin mortalite ile ilişkisi retrospektif olarak araştırıldı. Bizim serimizde, BDH sonrası erken dönemde serebral infarktlı 416 hastanın % 5.05'inde ve intraserebral hemarajili (İSH) 228 hastanın % 10.5'inde epileptik nöbet görüldü. İSH’ Iardan sonra görülen nöbet sıklığı anlamlı derecede yüksek bulundu (p = 0. 014). Serebral infarkt sonrası nöbet geçiren bulguların % 86'sında lezyon kortikal lokalizasyonlu idi ve kortikal tutulum ile nöbet ilişkisi anlamlı bulundu (p < 05). BDH sonrası nöbet geçiren ve geçirmeyen hastalarda erken dönem mortalite oranları arasında her iki grupta anlamlı fark bulunmadı (infarktlarda % 23.8'e karşın % 22.4; İSH’ larda 37.5'e karşın % 46.5). Sonuç olarak, BDH sonrası epileptik nöbetler mortaliteyi etkilememekle birlikte, kronik dönemde tekrarlama olasılığının yüksek olması ve tedaviye direnç göstermeleri nedeniyle BDH olgularında önemli bir sorun olarak karşımıza çıkmaktadır.
In this study, we investigated CVD patients with supratentorial lesions and retrospectively examined incidence and type of seizures, their relation to nature and site of lesion, and to early stage mortality. In our series, epileptic seizures in early post-stroke period were seen in 5.05 % of cerebral infarctions, and in 10.5 % of intracerebral hemorrhages. Occurrence rate was significantly higher in hemorrhage group (p = 0. 014). Also there was close relation between infractions extended to cerebral cortex and seizures (p < O. 05). In regard of case fatality rate there wasn't significant difference between patients with post-stroke seizures and seizure free patients in both groups (23.8% vs 22.4 % in infarction group and 37.5% vs 47.5% in hemorrhage group). We concluded that early post-stroke seizures outstand as a major problem in stroke patients because of high incidence of recurrences in chronic stage and their resistance to anticonvulsant therapy.
