Abstract
Açık kalp ameliyatından sonra solunum fonksiyon testlerindeki düşüş ve bu azalmanın hastaların postoperafuar seyrine etkisini araştırmak amacıyla 39 aorta-koroner bypass ameliyatı olmuş, hasta prospektif olarak incelendi. Ayrıca internal mammary arterinin (IMA) kullanımının ilave bir kısıtlayıcı etken olup olmadığı araştırıldı. Tüm hastalara ameliyattan bir gün önce ve taburcu olmadan bir gün sonra solunum fonksiyon testleri yaptırıldı. Ayrıca perop. anestezi almadan önce ve taburcu edilmeden bir gün önce arteriyel kan gazları incelendi. 25 olguda preop. dönemde hafif ve orta derecede obstrüktif ve restriktif tarzda solunum yetersizliği vardı. Diğer hastalarda bir özellik yoktu. Postoperalif 8-10. günlerde solunum fonksiyonları (FVC, FEVI, MMV) . % 30 civarında ve arteriyel PaO2 % 20 civarında azaldı. IMA kullanılan hastalarda sadece PaO2 de daha fazla düşüş görülmesine rağmen anlamlı sonuç çıkmadı. Hastalarımız 9-11. günlerde taburcu oldular. Sonuç olarak ameliyat öncesi devrede solunum fonksiyon testleri ileri derecede bozuk olmayan hastalar, koroner bypass cerrahisinden sonra ortaya çıkan solunum fonksiyonlarındaki azalmayı rahatlıkla tolere edebilmekledirler ve IMA kullanımı kısıtlayıcı bir faktör değildir.
39 patients who had undergone aorto-coronary bypass operations were studied prospectively to investigate the decrease in respiratory functional tesıs. After open heart surgery and its effect on its postoperative progression. Furthermore, the use of IMA was examined whether it was a restrictive factor or not. Respiratory function tests and arterial blood gases were examined in all patients preop. and before day were discharged from the hospital. In 25 of all, mild and moderate obstructive and restrictive respiratory distress were present. The others had no specularite post operatively, in the 8 th and 10 th. Days a decrease of 30 % in respiratory function and a decrease of 20 % arterial P02 were estimated. In the patients who had IMA constructed, only P02 decreases were proven but the results were not significant. As a result, the patients who had respiratory failure pre op. and who had ACB surgery tolerated this respiratory functional decrease easily and IMA was not a restrictive factor.
