Intraserebral hematoma eşlik edip komputerize tomografi ile tesbit edilemeyen subdural hematomlar

Abstract

1985-1986 yılları arasında kliniğimizde 34 intraserebral hematom olgusu komputerize tomografi ile teşhis edilip ameliyat edildi. intraserebral hemorajinin altında yatan vasküler patoloji yi araştırmak için travmatik olgular haricinde bütün olgulara anjiografi yapıldı. İki olguda anjiografide subdural hematom görüldü. Diğer bir vakada ise ameliyat sırasında subdural hematom tespit edildi. Her üç olguda KT subdural hematom için negatif idi. Nadir olmasına rağmen intracerebral hematoma eşlik eden subdural hematom görülmesi olasılığı unutulmamalı, KT negatif olduğunda bile diğer nöroradyolojik tetkikler yapılmalıdır.Between January 1985 and June 1986 in our clinic, 34 cases of intracerebral hematomas were diagnosed with computerized tomography and operated on. Angiography was performed in all of the cases except traumatic cases for investigation of the vascular pathology underlining the intracerebral hematoma. In two of them, angiography showed subdural hematoma, and in one of the other case subdural hematoma was diagnosed during the surgical intervention. CT was false negative for subdural hematoma in those three cases. Occurence of the subdural hematomas accompanying the intracerebral hematomas, although very rare, should not be overlooked. When the CT is negative, other neuro radiological investigations should be performed.
