Çalışmamızda 24 Romatoid Artritli (RA) vakanın böbrekleri, klinik, laboratuvar ve histopatolojik olarak incelendi. Klinik ve laboratuvar incelemede (idrar analizi, serum kreatinin, glomerüler filtrasyon değeri (GFR), intravenöz pyelografi (lVP), renal ultrasonografi) 12 vakada anomali bulunur ken, renal biyopsilerin ışık mikroskopisi ile incelenmesinde 20 vakada çeşitli histopatolojik bulgular saptandı. Bu bulgular % 41 ormanda tubular atrofi, % 37 oranında mezangial matriks ve hücre artışı, % 29 ormanda arteriolar fibro-hyalen kalınlaşma, % 25 oranda fokal global glomerulosclerosis, % 25 oranında fokal segmental glomeruloskleroz, % 25 oranında tübüler dilatasyon, % 20 oranında interstisyel fibrosis ve % 20 oranında amiloidoz idi. Klinik ve laboratuvar incelemede vakalann % 50'sinde renal patoloji saptanırken, histopatolojik incelemede bu oran % 83 idi. Klinik ve laboratuvar patoloji den daha fazla oranda ışık mikroskobik patoloji saptanması, romatoid artritte böbreğin primer veya sekonder subklinik olarak hastalığa iştirakinin bir delili şeklinde kabul edilebilir.
In 24 rheumatoid arthritis cases (4 male, 20 female), kidneys were evaluated clinically, in laboratory and histopathologically. In 12 cases urine examination, serum Cr, GFR, renal US and JVP revealed normal findings (Group I) and in the other 12 cases laboratory and clinical examination showed some abnomıalities (Group Il). All the cases in group Il (121/12) and only eight cases in group I (8/12) showed renal histopatlıologic abnormalities (We observed in all cases (24) 41 % tubular atrophy, 37 % mesangial matrix and cell proliferation, 29 % interstitial cell proliferation, 29 % focal global sclerosis, 25 % focal segmental sclerosis, 5 25 arterial fibro-hyaline thickening, 25 % tubular dilatation, 20 % interstitial fibrosis and 20 % amyloidosis. While in clinical and laboratory examination of rheumatoid arthritis cases pathology was found in 50 % of cases, tlhs ratio increased to 83 % in histopathological examination. This findings can be considered evidence of extensive involvement of kidneys either prinıarily or secondarily in rheumatoid arthritis.
