Perianal fistüllerin tedavisinde sfinkterotomi'nin yeri

Abstract

1975-1978 tarihleri arasında 69 perianal fistüllü hastaya tek ya da çok seanslı sfinkterotomi uyguladık . Komplikasyonsuz basit fistüllerde (alçak fistüller) setonla veya doğrudan bistüri keserek fistülotomi uyguladık. Yüksek fistüllerde ise Parks metodunu uyguladık. Toplam olarak 52 olguda (% 72,46) tam iyileşme sağladık. 14 olguda (% 20) ise çeşitli komplikasyonlara rastladık ve 3 olguda da (% 2,89) nüks gördük. Alçak fistüllerin tedavisinde seton uygulaması ile çok zamanlı sfinkterotominin, yüksek fistüllerde ise Parks metodunun tatminkar sonuç verdiği kanaatine vardık.Sphincterotomies in one or more stages were performed in 69 oase. of perianal fistula during the period of 1975- 1978 in our clinic. In uncomplicated cases of simpk (lower) fistula direct uciaion or seton were applied. In higher fistulas the Parks method was performed. As a result, we attained complete healing in 52 cases aut of 69 (% 72,46) complications were noted in 14 cases (% 20) and recurrence in 3 cases (% 2,89). According to our experience, seton application and multi~toged sphincterotomy are satis factory procedures in the treatment of lower fistulas and the Parks method is superior in higher fistulas.
