    Tobacco smoking is a significant risk factor for a variety of chronic disorders, such as chronic obstructive pulmonary disease (COPD), lung cancer, cardiovascular diseases, and other forms of malignancies. In this study, we investigated the comorbid medical conditions and sociodemographic features that contribute to disparities in tobacco smoking prevalence in the adult population. We induded volunteers who participated in events for the "World COPD day" in 4 consecutive years (i.e., 2014, 2015, 2016, and 2017) held in Bursa, Turkey. Sociodemographic features, smoking characteristics, and comorbidities were recorded. A total of 747 volunteers who were 47.1 +/- 14.5 [16-84] years of age and smoked 24.1 +/- 20.91 pack-years were included. The study population consisted of 405 (54.2%) males, 36.3% of which were current smokers, 23.6% exsmokers, and 40.2% nonsmokers. Moreover, 15.7% of participants with asthma were nonsmokers and 9.4% ever-smoker (p=0.011), while 2% of participants with coronary artery disease (CAD) were nonsmoker and 6.7% were ever-smoker (p=0.003). Current smoker participants exhibited more symptoms such as cough. sputum, and wheezing (p<0.001, in both cases). In this study, we found that the factors associated with smoking in the general population are male gender and higher educational level.

    Attitudes and beliefs about copd: Data from the breathe study

    Although COPD is a debilitating pulmonary condition, many studies have shown awareness of the disease to be low. This article presents data on attitudes and beliefs about COPD in subjects with respiratory symptoms participating in the BREATHE study in the Middle East and North Africa region. This study was a large general population survey of COPD conducted in ten countries of the region (Algeria, Egypt, Jordan, Lebanon, Morocco, Saudi Arabia, Syria, Tunisia, Turkey and United Arab Emirates), together with Pakistan, using a standardised methodology. A total of 62,086 subjects were screened, of whom 2,187 fulfilled the "epidemiological" definition of COPD. A detailed questionnaire was administered to these subjects, which documented knowledge about the disease, attitudes to care, beliefs about COPD and satisfaction with treatment. 1,392 subjects were analysable. Overall, 58.6% of subjects claimed to be very well or adequately informed about their respiratory condition. Two-thirds of subjects reported receiving information about COPD from their physician and 10.6% from television; the internet was cited by 6% and other health professionals or patient associations by <1%. Several inappropriate beliefs were identified, with 38.9% of respondents believing that there were no truly effective treatments, 73.7% believing that their respiratory condition would get progressively worse regardless of treatment and 29.6% being unsure what had caused their respiratory problems. Although 81% of respondents believed that smoking was the cause of most cases of COPD in general, only 51% accepted that it was the cause of their own respiratory problems. Treatment satisfaction was relatively high, with 83.2% of respondents somewhat or very satisfied with their physician's management, in spite of the fact that only 47.5% considered that their physician's advice had helped them manage their respiratory symptoms a lot. In conclusion, awareness of COPD in the region is suboptimal and treatment expectations are undervalued.     Prognostic value of serum d-dimer levels in patients with community acquired pneumonia

    Aim: D-dimer results from the fibrin breakdown after fibrinolytic system activation. Although the potential use of serum D-dimer levels has been assessed as a screening test for venous thromboembolism, its role in other disorders has not been as well defined. Little is known about the relationship between D-dimer levels and the clinical outcomes of patients with community-acquired pneumonia (CAP). The aim of this study was to investigate the prognostic value of plasma D-dimer levels in patients with CAP.Material and methods: This prospective study was conducted between January 2006 to September 2006 including 64 cases diagnosed as CAP, 45 cases as pulmonary emboli and 20 cases for control group. The demographic characteristics, physical examination findings and laboratory test results of cases were recorded. The severety of cases in pneumonia group were assessed with Pneumonia Severity Index (PSI). Among three groups levels of D-dimer in serum were measured with latex buttressed turbidimetric method for quantitative determination and compared.Results: Serum D-dimer levels were higher in patients with pulmonary embolism (748.3 +/- 769.6 mu g/L) when compared other two groups and it was higher in CAP (357.8 +/- 294.7 mu g/L) group when compared to controls (149.7 +/- 99.6 mu g/L) (p<0.05). A significant relationship was found between the presence of elevated D-dimer levels and radiologic pneumonia extension and PSI.Conclusion: We concluded that D-dimer levels could be useful for predicting clinical outcome in patients with CAP.

    Asthma control and adherence in newly diagnosed young and elderly adult patients with asthma in Turkey

    Objective: This study aimed to evaluate the factors that affect asthma control and adherence to treatment in newly diagnosed elderly asthmatics in Turkey compared with younger patients. Methods: This real-life prospective observational cohort study was conducted at 136 centers. A web-based questionnaire was administered to the patients who were followed up for 12 months. Results: Analysis included 1037 young adult asthma patients (age <65 years) and 79 elderly asthma patients (age >= 65 years). The percentage of patients with total control in the elderly and young groups were 33.9% and 37.1% at visit 1, 20.0% and 42.1% (p = 0.012) at visit 2, and 50.0% and 49.8% at visit 3, respectively. Adherence to treatment was similar for both groups. Visit compliance was better in the elderly group than in the young group at visit 1 (72.2% vs. 60.8%, p = 0.045), visit 2 (51.9% vs. 34.9%, p = 0.002), and visit 3 (32.9% vs. 19.4%, p = 0.004). Adherence to treatment increased with asthma control in both groups (both p < 0.001) but decreased with the presence of gastritis/ulcer, gastroesophageal reflux, and coronary artery disease in the elderly. Conclusions: Asthma control and adherence to treatment were similar for the elderly and young asthma patients, though the follow-up rate was lower in young patients. The presence of gastritis/ulcer, gastroesophageal reflux and coronary artery disease had negative impacts on the adherence to treatment in elderly adult patients.

    Amyloidosis is a syndrome characterized by the deposition of an insoluble proteinaceous material in the extracellular matrix of one or several organs. Respiratory tract involvement with amyloid is rare and deposition of lower respiratory tract has been recognized in a variety of situations with different presentations. Primary idiopathic amyloidosis may be a diagnostic problem because of its low incidence and its variable manifestations. We report herein a case with multiple myeloma presenting diffuse interstitial infiltration, in which pulmonary AL type amyloidosis was diagnosed through transbronchial lung biopsy.

    Sleep disorders and occupational accident in shift work nurses

    Objective: This study aimed to investigate the prevalence of symptoms of sleep related breathing disorders in shiftwork nurses and to evaluate relationship between occupational accident and the symptoms.Material and Method: Data of 410 nurses was evaluated. A questionnaire was administered to get information on their socio-demographic data, presence of witnessed apnea during sleep, the prevalence of snoring, excessive daytime sleepiness and frequency of other symptoms and having any occupational accident.Results: The prevalence of habitual snoring, witnessed apnea, excessive daytime sleepiness was 5.4%, 22.4%, 47.1% respectively. Snoring (p<0.001) and witnessed apnea (p<0.001) prevalence was higher in overweight nurses. Mean values of the body mass index (p<0.001), neck circumflex (p=0.03), co-morbid disease (p<0.001) and the prevalence of witnessed apnea (p<0.001) among the nurses with habitual snoring were statistically higher than the nurses without habitual snoring. 51 nurses (12.4%) had been involved in an occupational accident. The prevalence of snoring (p=0.008) and mean Epworth Sleepiness Score (ESS) were higher than the group without accidents. The ESS was higher than 10 points in 63 (15.4%) of the interviews. In this group occupational accident prevalence was 4.1 times higher than the group with less than 10 points ESS.Conclusion: We come to the conclusion that interrogating the sleep related disorders symptoms in adults and also performing diagnostic procedures and treatment in case of suspicion may decrease the morbidity of sleep related breathing disorders.

    Introduction: In a resource-constrained situation, a clinical risk stratification system can assist in identifying individuals who are at higher risk and should be tested for COVID-19. This study aims to find a predictive scoring model to estimate the COVID-19 diagnosis.Materials and Methods: Patients who applied to the emergency pandemic clinic between April 2020 and March 2021 were enrolled in this retrospective study. At admission, demographic characteristics, symptoms, comorbid diseases, chest computed tomography (CT), and laboratory findings were all recorded. Development and validation datasets were created. The scoring system was performed using the coefficients of the odds ratios obtained from the multivariable logistic regression analysis.Results: Among 1187 patients admitted to the hospital, the median age was 58 years old (22-96), and 52.7% were male. In a multivariable analysis, typical radiological findings (OR= 8.47, CI= 5.48-13.10, p< 0.001) and dyspnea (OR= 2.85, CI= 1.71-4.74, p< 0.001) were found to be the two important risk factors for COVID-19 diagnosis, followed by myalgia (OR= 1.80, CI= 1.082.99, p= 0.023), cough (OR= 1.65, CI= 1.16-2.26, p= 0.006) and fatigue symptoms (OR= 1.57, CI= 1.06-2.30, p= 0.023). In our scoring system, dyspnea was scored as 2 points, cough as 1 point, fatigue as 1 point, myalgia as 1 point, and typical radiological findings were scored as 5 points. This scoring system had a sensitivity of 71% and a specificity of 76.3% for a cut-off value of >2, with a total score of 10 (p< 0.001).Conclusion: The predictive scoring system could accurately predict the diagnosis of COVID-19 infection, which gave clinicians a theoretical basis for devising immediate treatment options.
