Patients, using (n=16) and non-using (n=14) interferon, were compared with each other in all parameters; ESS and PSG data were compared with a control group consisting of 19 healthy people. Short form-36 (SF-36) data were also compared with the society norms.Results: Central fatigue was observed in 86.7% of the patients. PSG data revealed that stage N2 sleep duration of those who did not use interferon was significantly longer than those who used it (p<0.001). According to the PSG, total sleep time, sleep efficiency, stage N3 and rapid eye movement time, mean respiratory disturbance index, sleep latency and the mean value of total leg movements were significantly higher in the patient group than in the control group (p<0.001). All parameters of SF-36 were significantly lower in patient group (p<0.001). The stage N3 sleep time length was found related with physical component summary of SF-36 (p<0.001).Conclusion: MS patients have high level of fatigue and additionally there are weighty disturbances in objective and subjective sleep parameters. Our findings were revealed that all the components of quality of life decreased significantly in these patients. The secondary outcomes were subjective outcomes, which were assessed using the quality-of-life scales. Forty-seven women who underwent the procedure were followed up for a minimum of 3 years. Forty-four of the 47 patients had stage 0 or 1 prolapse at the end of the second year, according to the primary outcomes. The anatomic success rate was 93.6%. In the secondary outcomes, 2 patients were not satisfied with their pelvic floor after the second year. The subjective cure rate was 95.7%. There was a statistically significant improvement in both pelvic organ prolapse quantification and quality-of-life scores in the third postoperative year. The mean operating time was 84.6 minutes, and the mean estimated blood loss was 21.3 mL. There were no bladder or bowel complications in the perioperative or postoperative period.Conclusion: Laparoscopic sacrohysteropexy can be performed safely with this meshless modification in uterine prolapse as an alternative. This study aims to measure the serum levels of interleukin (IL)-23, IL-17, IL-1 beta, tumor necrosis factor (TNF)-alpha, IL-10, transforming growth factor (TGF)-beta, C-reactive protein (CRP), procalcitonin (PCT), and serum amyloid A (SAA) in cases of candidemia and to compare them with those observed in cases of bacteremia. For this purpose, the serum cytokine levels from 50 patients with candidemia were compared with those of 14 patients with polymicrobial sepsis, 30 patients with bacteremia, and 27 healthy control subjects. The cytokine levels were studied using sandwich ELISAs according to the manufacturer protocol. The serum levels of TGF-beta, IL-23, and IL-17 were found to be significantly higher in the candidemia group in comparison with the samples from those with bacteremia and healthy controls. The PCT and SAA levels were higher in samples from the group with bacteremia those from individuals with candidemia and the healthy control group. Assuming an IL-17 level threshold of > 38.79 pg/ml, the sensitivity and specificity were 38% and 96.6%, respectively but considering an IL-23 threshold of > 59.97 pg/ml, the sensitivity and specificity valueswere found to be 72% and 60%, respectively. The sensitivity and the specificity of the TGF-beta levels were found to be 85.71% and 53.33%, respectively, when the TGF-beta threshold is > 560 pg/ml. PCT and SAA demonstrated a superior performance for the differentiation of candidemia and bacteremia. Tabanus bromius L. is common species worldwide, which shows considerable variation in the size of the body due to its adaptation to different habitats. In this paper, thirteen populations of this species, which are collected from different habitats and ecological regions (forests, sea sides and alpines) at altitudes between 5-2200 meters in Anatolia and Aegean were compared using geometric morphometries. Recent studies have demonstrated a relationship between inflammation and obstructive sleep apnea syndrome (OSAS). The aim of this study was to investigate the association between TNF-alpha G308A and C857T gene polymorphisms and OSAS in Turkish patients. Material and Methods: Sixty-nine patients who were diagnosed with OSAS and 42 control subjects were included in the study. The polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method was used to detect TNF-alpha G308A and C857T gene polymorphisms. The level of significance for statistical analysis was set at p<0.05. Results: The distribution of genotypes was not significantly different between subjects with a definite diagnosis of OSAS and the control group (p> 0.05). However, the mean body mass index of the OSAS group was significantly different from that of the control group (p<0.05). Conclusion: To our knowledge, this study is the first to analyze the relationship between OSAS and INF-alpha G308A and C857T gene polymorphisms in Turkish patients. We employed 23 questions from an earlier study related to MTX concerns. These questions were entered into the LLMs, and the responses generated by each model were evaluated by two reviewers using Likert scales to assess accuracy and completeness. The GPT models achieved a 100% correct answer rate, while BARD and Bing scored 73.91%. In terms of accuracy of the outputs (completely correct responses), GPT-4 achieved a score of 100%, GPT 3.5 secured 86.96%, and BARD and Bing each scored 60.87%. BARD produced 17.39% incorrect responses and 8.7% non-responses, while Bing recorded 13.04% incorrect and 13.04% non-responses. The ChatGPT models produced significantly more accurate responses than Bing for the "mechanism of action" category, and GPT-4 model showed significantly higher accuracy than BARD in the "side effects" category. There were no statistically significant differences among the models for the "lifestyle" category. GPT-4 achieved a comprehensive output of 100%, followed by GPT-3.5 at 86.96%, BARD at 60.86%, and Bing at 0%. In the "mechanism of action" category, both ChatGPT models and BARD produced significantly more comprehensive outputs than Bing. For the "side effects" and "lifestyle" categories, the ChatGPT models showed significantly higher completeness than Bing. The GPT models, particularly GPT 4, demonstrated superior performance in providing accurate and comprehensive patient information about MTX use. Identification of this membrane is difficult with conventional MRI techniques. The purpose of this retrospective study is to determine the impact of three-dimensional sampling perfection with application-optimized contrasts using different flip-angle evolutions (3D-SPACE) sequence with variant flip-angle mode (VFAM) in the assessment of LM at 3-T MRI devices. 3D-SPACE with VFAM images were obtained in 445 patients. LM visibility and integrity were scored as 0 (good), 1 (moderate), and 2 (poor) on these images for each parts (sellar, diencephalic, and mesencephalic) and overall of the membrane. According to the LM overall integrity scores, 11 % (48 cases) of the patients had perforated membrane. According to subsegmental integrity scores, sellar part was completely intact in 63 % of patients, diencephalic segment was completely intact in 60 % of the patients, and mesencephalic segment was completely intact in 95 % of the patients. Visibility scores of the third ventricle inferior wall were significantly higher in the patients with intact LM (p = 0.001). There was not any statistically significant relationship between LM pattern and overall integrity (p = 0.352). LM attachment sites could be detected easier in the patients who had better visibility of third ventricle inferior wall or intact LM (p < 0.001 for both). 3D-SPACE technique is a useful alternative for the evaluation of morphology, integrity, individual variations, topographic relationships, and visibility of LM since it has some advantages including lower SAR values, fewer artifacts, and high-resolution images.Item Assessment of submitted manuscripts in medical sciences according to statistical errors(Ortadogu Yayınları, 2012-10) Ercan, İlker; Ocakoğlu, Gökhan; Sığırlı, Deniz; Özkaya, Güven; Uludağ Üniversitesi/Tıp Fakültesi/Biyoistatistik Anabilim Dalı.; 0000-0003-0297-846X; 0000-0002-2382-290X; AAH-5180-2021; AAA-7472-2021; A-4421-2016; 6603789069; 15832295800; 24482063400; 16316866500Objective: The aim of the present study was to list errors in statistical analysis and presentation detected during the first review of submitted manuscripts, compared to the literature reviewing published articles. Material and Methods: Statistical errors determined during the first review by the biostatistics reviewer in manuscripts included in the study were presented. All studies were assessed by the same biostatistics specialist. The defined statistical errors were classified by 4 biostatistics specialist respectively. Results: No statistical errors were found in 4.42% of 181 original manuscripts; 73.99% of 173 original manuscripts with statistical errors were accepted following the correction of the statistical errors. The most common statistical errors were errors related to test in 34.59%, p-values in 17.36%, summarizing data in 15.60% and incorrect and insufficient demonstration of descriptive statistics in 15.60%. Conclusion: Considering that the errors in scientific papers are reviewed and corrected by researchers according to the suggestions of the reviewer, researchers with lack of statistical solid background are found to make errors in the process of utilizing statistics in their studies. In some cases, although researchers get statistical counseling, they make errors in the presentation while writing the papers. It is important to investigate the effect of health literacy on medication adherence in this patient population. Health literacy can be summarized as an individual's ability to understand and interpret the provided medical information and to behave appropriately based on this information.Aim: The purpose of this study was to investigate the association between health literacy and medication adherence in older people with chronic disease.Methods: A total of 175 patients admitted to the family health center clinic in Bursa, Turkey, who were older than 65 years old, were enrolled in this cross-sectional study using the convenience sampling method. A priori power analysis was conducted to determine the required sample size to reach 90% power. The Turkish version of the 8-item Morisky Medication Adherence Scale (MMAS-8) was used to assess medication adherence. The European Health Literacy Survey Questionnaire (HLS-EU-Q47) was used to evaluate health literacy. Disability associated with dyspnea was assessed using the Medical Research Council (MRC) dyspnea scale.Results: The data showed that, according to dyspnea status and diagnosis, medication adherence varied. In this sample, medication adherence in elderly patients was not associated with health literacy. Instead, medication adherence was associated with the patient's disability and the course of the disease.Discussion and conclusions: Improving health literacy may enhance the medication adherence of older people with chronic disease. Patients were randomly divided into 4 groups (n=40, each): i) the standard analgesia (SA) group received the block and port-site infiltration with normal saline (NS); ii) the local anesthetic group received the block with NS and port-site infiltration of bupivacaine; iii) the unilateral laparoscopic-assisted transversus abdominis plane (ULTAP) block group received 2-quadrant block with bupivacaine on the right and NS on the left and port-site NS infiltration; and iv) the BLTAP block group received bilateral bupivacaine and port-site NS infiltration. Postoperative 1-, 3-, 6-, 12-, and 24-hours visual analog scale (VAS) pain scores at rest and during cough, opioid requirement, presence of nausea and vomiting, and satisfaction scores were recorded.Results: The one-hour VAS score at rest was lower in the BLTAP block group than in the SA and ULTAP block groups. The change in VAS score was higher in the SA group than in the BLTAP block group. During cough, the one-hour VAS score was lower in the BLTAP block group than in the SA group. There were no differences among groups in other parameters. Methods: In a retrospective study, data were obtained for singleton live births that occurred at a hospital in Bursa, Turkey, between June 1 and December 31, 2015. All Syrian refugees were eligible for inclusion; one Turkish citizen was included for each refugee. Results: Overall, 545 Syrian refugees and 545 Turkish citizens were included. Cesarean delivery was undertaken for 176 (32.3%) Syrians and 235 (43.1%) Turks (P<0.001). Median neonatal birth weight was higher among Turkish citizens (3300 g, range 970-4720) than among Syrian refugees (3110 g, range 540-4790; P<0.001). Gestational diabetes was recorded for 42 (7.7%) Turkish citizens and 9 (1.7%) Syrian refugees (P<0.001). Pre-eclampsia affected 21 (3.9%) Turkish citizens and 8 (1.5%) Syrians (P=0.014). Conclusion: Cesarean delivery is more common among pregnant Turkish citizens than among Syrian refugees. Kliniğimizde Ocak 2004-Ocak 2010 tarihleri arasında 1257 izole çalışan kalpte koroner bypass, 494 izole konvansiyonel koroner bypass uygulandı. 66 hastada (%5.4) acil kardiyopulmoner bypassa konversiyon gereksinimi oldu. Kardiyopulmoner bypassa konversiyon olan hastalar, konvansiyonel cerrahi ile karşılaştırıldığında mortalitenin (p=0.003), intraaortik balon pompa kullanımının (p=0.007), ventilasyon süresinin (p=0.020), kardiopulmoner bypass süresinin (p=0.002), operasyon süresinin (p=0.001), inotrop ihtiyacının (p<0.001), aritmi insidansının (p<0.001), hastanede kalış süresinin (p<0.001), göğüs tüpü drenajınını (p=0.023) önemli derecede yüksek olduğu görüldü. Off-pump koroner bypass cerrahisi esnasında acil kardiyopulmoner bypassa konversiyon olması yüksek mortalite ve morbidite ile sonuçlanır. METHODS Patient information material on prostate cancer was used as a reference source from the website of the European Association of Urology Patient Information. This was used to generate 59 queries. The accuracy of the model's content was determined with F1, precision, and recall scores. The similarity was assessed with cosine similarity, and the quality was evaluated using a 5RESULTS ChatGPT was able to respond to all prostate cancer-related queries. The average F1 score was 0.426 (range: 0-1), precision score was 0.349 (range: 0-1), recall score was 0.549 (range: 0-1), and cosine similarity was 0.609 (range: 0-1). The average GQS was 3.62 +/- 0.49 (range: 1-5), with no answers achieving the maximum GQS of 5. While ChatGPT produced a larger amount of information compared to the reference, the accuracy and quality of the content were not optimal, with all scores indicating need for improvement in the model's performance. CONCLUSION Caution should be exercised when using ChatGPT as a patient information source for prostate cancer due to limitations in its performance, which may lead to inaccuracies and potential misunderstandings. ADC maps were generated automatically. Signal intensity and ADC values were measured. Following MRI, pleural fluid of 10-15 ml was obtained and analyzed. AUC values were compared for different diffusion levels of ADC and SI measurements. The relationship between ADC values and pleural effusion LDH and total protein levels was examined.Results: The cut-off values obtained from signal intensity and ADC measurements to differentiate exudates with malignant pathology were not found to be statistically significant. In the inflammatory-infectious group, a significant negative correlation was observed between ADC values and pleural fluid LDH measurements in all b values. The patients were divided into two groups, as those who demonstrated a decrease in PTH levels measured after completion of hemithyroidectomy of the first side (Group 1, n = 43) and those who did not demonstrate a decrease in PTH levels (Group 2, n = 24). The serum PTH levels were measured just before the incision (PTHi), when the hemithyroidectomy stage had been completed (PTHht), at the end of the operation (PTHtt), and at the postoperative 24th hour (PTH24hr). The serum total calcium (Ca2+) levels were also measured. The median percentage differences in PTHtt levels based on basal PTHi levels of Groups 1 and 2 were -60.6 and -15.7 per cent, respectively, P = 0.001. The frequency of postoperative biochemical hypocalcemia was higher in Group 1, P < 0.05. It was determined that a 10 per cent or higher decrease in PTHht levels in Group 1 could predict biochemical hypocalcemia at the postoperative 24th hour. In conclusions, postoperative hypocalcemia is seen more frequent in patients with a decrease of PTHht during total thyroidectomy. Methods: Patients operated on for PHPT in the university hospitals of Turkiye were included in the study. The demographic, clinical, and laboratory findings and the operational data of the patients were investigated according to the whole country and to different geographical regions. Comparisons were performed according to whole country and regions. Results: A total of 1,162 cases were included in the study from different regions and 20 university hospitals. The mean age of patients was 52.4 +/- 0.38 (mean +/- standard error) in the general population of Turkiye. The rates of hypertension, urolithiasis, bone disease and 25-hydroxyvitamin D insufficiency were 35%, 18.6%, 67.6%, and 63%, respectively. The median parathormone (PTH), serum total calcium (Ca+2) and phosphorus value were 220 pg/mL (range, 70-2,500 pg/mL), 11.2 mg/dL (range, 9.5-11.2 mg/dL), and 2.4 mg/dL (range, 1-4.7 mg/dL, respectively. The median size of the adenomas resected was 16 mm (range, 4-70 mm). Significant differences were observed in the clinical and laboratory findings of the patients operated on due to PHPT between different geographical regions of Turkiye (P < 0.05). Conclusion: The clinical and laboratory characteristics of the patients with PHPT in different geographical regions of Turkiye differ. Our study was carried out between June 2004 and September 2005 among adults aged 40 years or more in Orhangazi, Bursa, Turkey. This population-based study was planned in 3 phases. In phase 1, face-to-face home interviews with 1124 subjects were carried out by residents of the departments of neurology and family medicine, using a short questionnaire; 247 persons had symptoms indicative of tremor. Of the persons with tremor symptoms in phase 1, 48 did not continue to phase 2. In phase 2, participants were videotaped and evaluated by movement disorders specialists by using scales specific to tremor. In phase 3, video recordings of all identified patients were reviewed and final diagnoses were made through a consensus of all three specialists. In the study population, the prevalence rate of ET was found to be 3.34%; of PT, 4.14%; and all types of tremor, 9.00%. There was no significant variability according to gender and age groups in prevalence rate of ET, PT. Most patients with amyloid A amyloidosis present with nephropathy and it leads to renal failure and death. We studied clinical characteristics and survival in patients with amyloid A amyloidosis. Methods: A total of 81 patients (51 males, 30 females) with renal biopsy proven amyloid A amyloidosis were analyzed retrospectively. The patients were divided into good and poor outcomes groups according to survival results. Results: Most of the patients (55.6%) had nephrotic range proteinuria at diagnosis. Most frequent underlying disorders were familial Mediterranean fever (21.2%) and rheumatoid arthritis (10.6%) in the good outcome group and malignancy (20%) in the poor outcome group. Only diastolic blood pressure in the good outcome group and phosphorus level in the poor outcome group was higher. Serum creatinine levels increased after treatment in both groups, while proteinuria in the good outcome group decreased. Increase in serum creatinine and decrease in estimated glomerular filtration rate of the poor outcome group were more significant in the good outcome group. At the time of diagnosis 18.5% and 27.2% of all patients had advanced chronic kidney disease (stage 4 and 5, respectively). Median duration of renal survival was 65 +/- 3.54 months. Among all patients, 27.1% were started dialysis treatment during the follow-up period and 7.4% of all patients underwent kidney transplantation. Higher levels of systolic blood pressure [hazard ratios 1.03, 95% confidence interval: 1-1.06, p = 0.036], serum creatinine (hazard ratios 1.25, 95% confidence interval: 1.07-1.46, p = 0.006) and urinary protein excretion (hazard ratios 1.08, 95% confidence interval: 1.01-1.16, p = 0.027) were predictors of end-stage renal disease. Median survival of patients with organ involvement was 50.3 +/- 16 months. Conclusion: Our study indicated that familial Mediterranean fever constituted a large proportion of cases and increased number of patients with idiopathic amyloid A amyloidosis. Additionally, it was observed that patient survival was not affected by different etiological causes in amyloid A amyloidosis.
