The aim was to perform a multicenter, retrospective study assessing clinical characteristics and treatment results. Methods: A standard questionnaire was uploaded to an online national database system to collect data on children with hypercalcemia (serum calcium level > 10.5 mg/dL) due to vitamin D intoxication [serum 25-hydroxyvitamin D (25(OH)D) level > 150 ng/mL] who were treated in pediatric endocrinology clinics. Results: Seventy-four children [median (range) age 1.06 (0.65-1.60) years, 45 males (61 %) from II centers] were included. High-dose vitamin D intake was evident in 77% of the cases. At diagnosis, serum calcium, phosphorus, alkaline phosphatase, 25(OH)D and parathyroid hormone concentrations were 15 +/- 3.2 mg/dl., 5.2 +/- 1.2 mg/dL, 268 +/- 132 IU/L, 322 (236-454) ng/ml, and 5.5 (3-10.5) pg/mL, respectively. Calcium levels showed moderate correlation with 25(OH)D levels (r(s) = 0.402, p <0.001). Patients were designated into five groups according to the initial specific treatment regimens (hydration-only, prednisolone, alendronate, pamidronate, and combination). Need for another type of specific drug treatment was higher in children who initially received prednisolone (p <0.000). Recurrence rate of hypercalcemia was significantly lower in children who were treated with pamidronate (p=0.02). The exact mechanism is not fully understood; however, it has been proposed that estrogen receptors and aromatase enzyme activity may play important roles in the pathogenesis of gynecomastia. While many studies have reported that aromatase enzyme (CYP19) gene polymorphism is associated with gynecomastia, only one study has shown a relationship between estrogen receptor (ER) alpha and beta gene polymorphism and gynecomastia. Thus, the aim of this study was to evaluate the relationships between CYP19 (rs2414096), ER alpha (rs2234693), ER beta (rs4986938), leptin (rs7799039), and leptin receptor (rs1137101) gene polymorphisms and gynecomastia. Methods: This study included 107 male adolescents with gynecomastia and 97 controls. Total serum testosterone (T) and estradiol (E2) levels were measured, and DNA was extracted from whole blood using the PCR-RFLP technique. The polymorphic distributions of CYP19, ER alpha, ER beta, leptin and leptin receptor genes were compared. Results: The median E2 level was 12.41 (5.00-65.40) pg/ml in the control group and 16.86(2.58-78.47) pg/ml in the study group (p < 0.001). The median T level was 2.19 (0.04-7.04) ng/ml in the control group and 1.46 (0.13-12.02) ng/ml in the study group (p = 0.714). There was a significant relationship between gynecomastia and leptin receptor rs1137101 (p = 0.002) and ER beta receptor rs4986938 gene polymorphisms (p = 0.002). Conclusions: According to our results, increased E2 level and ER beta gene rs4986938 polymorphism might explain why some adolescents have gynecomastia. Leptin receptor gene rs1137101 polymorphism might affect susceptibility to gynecomastia.Item Konjenital hiperinsülinemik hipoglisemi tanılı hastalarda klinik ve genetik özellikler ile tedavi sonuçları: tek merkez deneyimi(Bursa Uludağ Üniversitesi, 2020-10-15) Erbaş, İbrahim Mert; Çatlı, Gönül; Paketçi, Ahu; Anık, Ahmet; Demir, Korcan; Böber, Ece; Abacı, AyhanKonjenital hiperinsülinemik hipoglisemi, pankreastan kontrolsüz insülin salınımı sonucu ortaya çıkan nadir bir hastalıktır. Bu çalışmada, kliniğimizde konjenital hiperinsülinemik hipoglisemi tanısı ile takip edilen hastaların klinik ve genetik özellikleri ile prognozlarının değerlendirilmesi amaçlanmıştır. Çalışmaya, 2011–2020 yılları arasında kliniğimizde konjenital hiperinsülinemik hipoglisemi tanısı ile takip edilen olgular dahil edildi. Olguların dosya kayıtlarından, klinik, laboratuvar ve genetik özellikleri, görüntüleme sonuçları, tedavi süreçleri ve prognozları ile ilgili bilgiler elde edildi. Çalışmaya dahil edilen 10 olgunun (altı kız, dört erkek) ortanca tanı yaşı 25 (3-183) gündü. En sık başvuru nedeni konvülziyon (altı olgu) iken, olguların üçü semptomatik hale gelmeden rutin tarama sırasında saptanan hipoglisemi nedeniyle tanı aldı. Yedi hastaya genetik analiz yapıldı. Analiz yapılan genler (ABCC8, KCNJ11, GLUD1, HNF4A) arasında dört hastada ABCC8 geninde varyant saptandı (iki hasta heterozigot, bir hasta homozigot, bir hasta ise bileşik heterozigot). Yedi hastada diazoksite yanıt alınırken, diazoksit tedavisine yanıt alınamayan üç hastanın tedavisi oktreotid olarak değiştirildi. Bu hastalardan birisinde ABCC8 geninde heterozigot, bir diğerinde ise homozigot varyant saptandı. Altı hasta izlemde (ortanca süre 1,5 yıl) spontan remisyona girdi ve bu olgular geçici konjenital hiperinsülinemik hipoglisemi olarak kabul edildi. TS’nede spontan menarş olasılığı %10 iken, fertilite olasılıkları ise çok daha düşüktür. 45,X/47,XXX mozaisizmi TS’li olguların %1-%4’ünü oluşturmaktadır. Bu karyotipe sahip bireylerde spontan menarş ve fertilite olasılığı daha yüksek olup, boy kısalığı ve renal anomali sıklığı ise diğer tiplerden daha nadirdir. On üç yaşında kız olgu iki yıldır uzamada yavaşlama ve yaşıtlarından kısa olması nedeniyle polikliniğe başvurdu. Fizik muayenesinde boyu 135 cm (-3,3 SDS), ağırlığı 32 kg (-2,3 SDS), Tanner evrelemesine göre meme gelişimi ve pubik kıllanması evre 3, dirsekte geniş taşıma açısı ve düşük ense saç çizgisi mevcuttu. Diğer sistem muayneleri normaldi. Tetkiklerinde tam kan sayımı, karaciğer ve böbrek fonksiyon testleri, tiroit fonksiyon testleri normaldi. Kemik yaşı 11 yaş ile uyumlu bulundu. FSH 5,99 mIU/mL, LH 2,94 mIU/mL ve E2 <20 pg/mL olarak saptandı. Karyotip analizi 45,X/47,XXX olarak raporlandı. Renal anomalisi olmayan olgunun ekokardiyografisinde minimal mitral yetmezlik dışında patolojik bulgu saptanmadı. Ders başarısı düşük olan olgunun yapılan WISC-R testinde IQ puanı 68 olarak saptandı. Rekombinan insan büyüme hormonu (RhGH) tedavisi başlanan olgunun 13 yaş 6 aylık iken spontan menarşı oldu. We aimed to investigate mutational frequencies of these genes and genotype/phenotype correlations in consanguineous families with CHNG. Design Because consanguinity in individuals with a presumptive genetic condition is often an indicator of an autosomal recessive inheritance and allows firmer correlations to be established between genotype and phenotype, we planned to execute our study in consanguineous families. Patients Hundred and thirty-nine children with CHNG phenotype born to consanguineous families. Measurements First, we investigated cases for evidence of linkage to the four known CHNG genes by microsatellite marker analysis. Mutation analysis by direct sequencing was then performed in those cases in whom linkage to the relevant candidate gene could not be excluded. In addition, in silico analysis of the predicted structural effects of TSHR mutations was performed and related to the mutation-specific disease phenotype. Results Homozygous germline TSHR mutations were detected in six families (5%), but no mutations were detected in PAX8, TSHB and NKX2-5. Four of TSHR mutations had not previously been described. Genotype-phenotype correlations were established and found to be related to the predicted structural effects of the mutations. Although congenital adrenal hyperplasia occurs commonly, several other important molecular causes have been reported, often with overlapping clinical and biochemical features. The relative prevalence of these conditions is not known, but making a specific diagnosis can have important implications for management. Objective: The objective of the study was to investigate the clinical and molecular genetic characteristics of a nationwide cohort of children with PAI of unknown etiology. Design: A structured questionnaire was used to evaluate clinical, biochemical, and imaging data. Genetic analysis was performed using Haloplex capture and next-generation sequencing. Patients with congenital adrenal hyperplasia, adrenoleukodystrophy, autoimmune adrenal insufficiency, or obvious syndromic PAI were excluded. Setting: The study was conducted in 19 tertiary pediatric endocrinology clinics. Patients: Ninety-five children (48 females, aged 0-18 y, eight familial) with PAI of unknown etiology participated in the study. Results: A genetic diagnosis was obtained in 77 patients (81%). The range of etiologies was as follows: MC2R (n = 25), NR0B1 (n = 12), STAR (n = 11), CYP11A1 (n = 9), MRAP (n = 9), NNT (n = 7), ABCD1 (n = 2), NR5A1 (n = 1), and AAAS (n = 1). Recurrent mutations occurred in several genes, such as c.560delT in MC2R, p.R451W in CYP11A1, and c. IVS3ds + 1delG in MRAP. Several important clinical and molecular insights emerged. Conclusion: This is the largest nationwide study of the molecular genetics of childhood PAI undertaken. Context Thyroid dyshormonogenesis comprises 10-15% of all cases of congenital hypothyroidism (CH), which is the most common neonatal endocrine disorder, and might result from disruptions at any stage of thyroid hormone biosynthesis. Currently seven genes (NIS, TPO, PDS, TG, IYD, DUOX2 and DUOXA2) have been implicated in the aetiology of the disease. Design As TDH is mostly inherited in an autosomal recessive manner, we planned to conduct the study in consanguineous/multi-case families. Patients One hundred and four patients with congenital TDH all coming from consanguineous and/or multi-case families. Measurements Initially, we performed potential linkage analysis of cases to all seven causative-TDH loci as well as direct sequencing of the TPO gene in cases we could not exclude linkage to this locus. In addition, in silico analyses of novel missense mutations were carried out. Results TPO had the highest potential for linkage and we identified 21 TPO mutations in 28 TDH cases showing potential linkage to this locus. Four of 10 distinct TPO mutations detected in this study were novel (A5T, Y55X, E596X, D633N). Conclusions This study underlines the importance of molecular genetic studies in diagnosis, classification and prognosis of CH and proposes a comprehensive mutation screening by new sequencing technology in all newly diagnosed primary CH cases.Item Tip 1 diyabetli çocuklarda tanıda diyabetik ketoasidoz sıklığı ve ilişkili faktörler(Uludağ Üniversitesi, 2010-06-22) Demir, Korcan; Büyükinan, Muammer; Dizdarer, Ceyhun; Şimşek, Damla Gökşen; Özen, Samim; Asar, Gülgün; Can, Şule; Altıncık, Ayça; Özhan, Bayram; Ersoy, Betül; Böber, Ece; Darcan, ŞükranGiriş: Bu çalışmada İzmir ve Manisa illerindeki üçüncü basamak hastanelerde bulunan çocuk endokrinolojisi kliniklerine getirilen yeni tanı tip 1 diyabet hastalarındaki başvuru anında diyabetik ketoasidoz (DKA) sıklığı ve bununla ilişkili faktörlerin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Hastaların dosyaları geriye yönelik olarak incelendi. Olguların cinsiyet, doğum tarihi, ailede diyabet öyküsü ve sağlık güvenceleri hakkındaki bilgiler kaydedilip başvuru şekli ile olan ilişkileri araştırıldı. Bulgular: Yapılan değerlendirmede 2008 yılında 139 hastanın (E/K:74/65, ortalama yaş 8,7±3,9) tanı aldığı görüldü. Hastaların büyük çoğunluğunda başvuru esnasında ketoz (n=58, %41,7) veya DKA (n=57, %41) tablosu mevcuttu. Hiçbir hastada ölüm ya da ciddi morbidite gelişmedi. Ailede tip 1 diyabet öyküsünün olmaması ve tanı anında 5 yaşından küçük olma durumunun tanıda DKA tablosu ile ilişkili olduğu saptandı. Risk analizi amacıyla lojistik regresyon uygulandığında sadece 5 yaşından küçük olmanın DKA açısından bir risk faktörü olduğu görüldü (p=0,008, Odds Oranı 3,3, %95 güven aralığı 1,4-8,1). Sonuç: Bu sonuçlar, toplumu çocukluk yaş grubunda diyabet konusunda bilinçlendirerek tanı anında DKA oranını azaltmak için geniş kapsamlı kampanyalar/çalışmalar yapılması gerektiğini düşündürmüştür.
