The aim of this study was to determine the relationship among postoperative pulmonary complication, snoring and STOP questionnaire in patients with ortophaedic surgery. Methods: 1,406 consecutive records of patients who had undergone elective ortophaedic surgery during the period January 2005-December 2008 were investigated retrospectively. Demographic information, sleep symptoms, STOP questionnaire, comorbidities and outcome data were collected. Results: There were 289 (20.5%) snorers and 1,117 (79.5%) non-snorers in the study group. There was no significant difference between snorer and non-snorer patients (p > 0.05) in the prevalence of pneumonia and respiratory failure. But in snorer patients the rate of postoperative atelectasis was significantly higher than in non-snorer group (p < 0.0001). The STOP Questionnaire was given to 1,406 patients and 147 (10.4%) out of them were classified at high risk of OSA. There was no significant difference in the prevalence of pneumonia and respiratory failure between low and high risk group (p > 0.05). However, in high risk patients the occurrence of postoperative atelectasis was significantly higher than in low risk group (p < 0.0001). We conducted this study to evaluate the benefits of preperitoneal catheter analgesia in managing pain after abdominal colon and rectal resections. A total of 50 patients undergoing colon and rectal resections for benign and malignant diseases received analgesic medicines via an epidural catheter placed just prior to surgery and a preperitoneal catheter placed at the end of the surgical procedure. Patients were instructed to use the epidural patient-controlled analgesia (PCA) device freely and were randomized into two groups after obtaining the approval of the Institutional Review Board: Group A received 10 ml of levobupivacaine twice a day postoperatively via preperitoneal catheter and group B received only 10 ml of saline. Demographics, surgical characteristics, pain scores recorded four days following surgery, analgesic volume used from the epidural PCA, clinical outcomes (length of stay, time to first bowel movement, time to first passage of gas or stool, time to first oral intake) and respiratory function test results (preoperative vs. postoperative) were compared. There were no significant differences in demographics or surgical characteristics between both groups. Pain scores were similar. Clinical outcomes and respiratory functions were comparable. The use of analgesic volume via epidural catheter was significantly lower in group A than in group B (P = 0.032). It is predominant in young women. To our knowledge, only 7 cases of tuberculous mastitis in men have been reported in the English literature since 1945. Furthermore, there are no male patients who have breast and osteoarticular tuberculosis in the literature. We presented a 4 1 -year-old man who was admitted with a fixed tender mass in the right retromammary region and pain in the right hip. Mycobacterium tuberculosis colonies were isolated from the semisolid mass of breast. Histopathologic examination revealed caseous granulomatous infection in the right hip synovial tissue. The standard approach to management of MPE has remained controversial. In this article, we present the case of an 18-year-old female who was admitted to our clinic with pericardial tamponade. We aimed to evaluate the efficacy and safety of CDT in the management of acute PE with right ventricular dysfunction (RVD). The primary outcomes were mortality, clinical success, and complications. Secondary outcomes were change in hemodynamic parameters in the first 24 hours following the procedure. Material/Methods: Medical records of consecutive patients diagnosed as having acute massive or submassive PE with accompanying RVD treated by immediate CDT at our institution from January 2007 to January 2014 were reviewed. Patient characteristics, mortality, achievement of clinical success, and minor and major bleeding complications were analyzed in the overall study group, as well as massive vs. submassive PE subgroups. Change in hemodynamic parameters in the second, eighth, and 24th hours after the CDT procedure were also analyzed. Results: The study included 15 consecutive patients (M/F=10/5) with a mean age of 54.2 +/- 16.6 years who underwent immediate CDT. Nine of the patients had submassive PE, and 6 had massive PE. In-hospital mortality rate was 13.3% (95% CI, 0.04-0.38). One major, but not life-threatening, bleeding episode was evident in the whole group. Hemodynamic parameters were stabilized and clinical success was achieved in 14/15 (93.3%; 95% CI, 70.2-98.8) of the patients in the first 24 hours. Notably, the hemodynamic recovery was significantly evident in the first 8 hours after the procedure. Conclusions: CDT is a promising treatment option for patients with acute PE with RVD with no fatal bleeding complication. Several mechanisms may be involved in mortality. One of these is coagulopathy and disseminated intravascular coagulation caused by SARS-CoV-2. When patients have coagulation disorders, treatment becomes more challenging and mortality rate increases accordingly. We investigated the relationship of DH with breathing pattern and ventilation ((V)over dot(E)) in COPD and normal subjects (NS). In 35 subjects with moderate COPD and 17 younger healthy volunteers we measured inspiratory capacity (IC), breathing frequency (f(R)), expiratory time (T-E), ventilation ((V)over dot(E)) and end-tidal carbon dioxide tension (PETCO2) at baseline and after 30 s of MPT at 40 breaths/min with metronome-defined I:E ratios of 1:1 and 1:2: A reduction in IC (Delta IC) was taken to indicate DH. In COPD subjects, DH correlated with T-E but not with (V)over dot(E) or PETCO2, and was best predicted by total lung capacity. NS also showed DH (although less than in COPD), which correlated with PETCO2 but not with f(R), T-E or (V)over dot(E). We conclude that MPT evokes DH in both NS and patients with COPD. Characteristics of asthma related factors and BMI of patients analyzed retrospectively from the data obtained from the clinic files of 160 hospitalized patients (41male,119female, mean age 46.5±1.1years)who were followed in our department.Body mass index was calculated as body weight/height (kg/m2).Patients were divided and evaluated in three groups according to their BMI; Group1: BMI<22.5 thin, Group2: 22.5≤ BMI<30 normal or overweight,Group3: BMI≥30 obese.The mean BMI of the whole patients was 28.3±0.5, whilst the mean BMI of patients in Group 1 was 32.9±2.9,in Group 2 was 46.6 ± 1.6,and in Group 3 was 49.2 ± 1.5. The 20% of patients in Group 1, 47% of patients in Group2 and 33% of patients in Group3 were atopic The 33%of the asthmatic patients was also diagnosed to have allergic rhinites.The 8.4% of patients (n:13)had a history of childhood asthma (54% in Group1, 31% in Group2 and 15% in Group3).The percentage of patients with the history of childhood asthma was significantly higher in Group1 than in Group2 and Group 3.Although there was not any significant difference in the mean eosinophiles counts and mean serum ECP levels of patients, those values were lower in Group3 than in other groups.There was not any significant difference in mean outdoor and indoor allergen sensitivity scores of patients among groups. Mean medication score (number of controller drugs) of patients in Group1 was significantly lower than in Group2 and Group3.The lung function parameters of FEV1 and PEF were not significantly different among groups, but predicted value of FVC was significantly higher in Group1 than Group2 and Group3 According these results we came to the conclusion that obesity was a severity factor in hospitalized asthmatic patients. Characteristics of asthma related factors and body mass index (BMI) analyzed from the data obtained out-patients files of 900 female patients, who were followed in our department between the years of 1992-2004.Patients were divided and evaluated in four groups according to their BMI; Group1: BMI<18.5 thin,Group2:18.5≤BMI<25 normal, Group3: 25≤BMI<30 overweight, Group4: BMI ≥30 obese The number of patients in Group1 was 23, was 336 in Group 2, was 302 in Group3, and was 239 in Group4. The mean BMI of patients was 26.9±0.2. The mean age of patients was 37.4± 0.4 and the patients in Group4 was significantly older than patients in other groups. The 42.6% of patients were atopic and 50.9% was also diagnosed to have allergic rhinites. The 53.8% of patients in Group1, 72.9% of patients in Group2, 63.5% of patients in Group3, 58.2% of patients in Group4 were atopic . The mean serum IgE levels of patients in Group4 was significantly lower than others. Although there was not any significant difference in the mean eosinophiles count of patients, those values were lower in Group4 than in other groups. The mean asthmatic years of patients in Group4 was significantly longer than others. The mean and predicted values of lung function parameters of Group4 were generally lower than other groups, especially the means FEV1 and FVC values was significantly lower in Group4 than Group1, Group2 and Group3. The mean medication score (number of controller drugs) of patients in Group4 was significantly higher than in patients of other groups.The hospitalization rate of patients in Group4 (19%) was significantly higher than patients in patients Group2 (9%) and Group3 (12) (p<0.05). In conclusion, obesity was a severity factor for asthmatic female patients. Genetic and environmental factors are held accountable for the etiology. Based on the general opinion, sarcoidosis develops after exposure to a specific environmental agent by genetically susceptible individuals. The present study aimed to evaluate the disease susceptibility of the GSTT1 and GSTM1 gene polymorphisms in the patients with sarcoidosis. Method: The present study included 78 patients; 38 patients with histopathologically verified sarcoidosis and 40 control subjects. Multiplex PCR method was used to determine the GSTT1 and GSTM1 gene polymorphisms. The genotype was determined based on the bands formed in the agarose gel electrophoresis. The statistical analysis was done using the chi-square test. Results: The positive/negative genotype rates were 79%/21% and 53%/47%, respectively in the case group for the GSTT1 and GSTM1 gene polymorphisms, whereas the positive/negative genotype rates were 77%/23% and 55%/45% in the control group. There was no statistically significant difference in the positive and negative genotypes compared with the case group and the control group for the GSTT1 and GSTM1 gene polymorphisms (p > 0.05). Discussion: The results from the present study suggest that there is not any association with the control group for the disease susceptibility of the GSTT1 and GSTM1 gene polymorphisms in patients with sarcoidosis, and this result should be supported by large-scale studies because of the limited number of cases in the present study.Item Hafif/orta şiddette alt veya üst ekstremite direnç egzersizlerinin akciğer işlevleri ve yaşam kalitesi üzerine etkisi(Bursa Uludağ Üniversitesi, 2019-05-16) Ünal, Hacı Osman; Coşkun, Funda; Dilektaşlı, Aslı Görek; Gökalp, Yusuf Emin; Özyener, Fadıl; Bursa Uludağ Üniversitesi/Sağlık Bilimleri Enstitüsü/Fizyoloji Anabilim Dalı.; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Göğüs Hastalıkları Anabilim Dalı.; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Fizyoloji Anabilim Dalı.Çalışmamızda, hafif/orta şiddette yapılan alt veya üst ekstremite direnç egzersizlerinin kronik solunum sistemi sorunları olan kişilerde akciğer işlevleri ve yaşam kalitesi üzerine etkisinin incelenmesi amaçlandı. 35-80 yaş arası 20 kronik akciğer hastası ve 15 sağlıklı birey çalışmaya gönüllü oldu. Bu 2 grup kendi içlerinde alt ve üst ekstremite direnç egzersizlerini yapmak üzere randomize olarak 2 alt gruba bölünerek toplam 4 grup oluşturuldu. Egzersiz programı günde 2-3 kere, haftada 3 gün ev programı şeklinde planlandı ve uygulamalı olarak katılımcılara gösterildi. Antrenmanın toplam süresi en az 2, en fazla 4 ay olmak üzere planlandı. Katılımcıların solunum fonksiyonları (FEV1, FVC, FEV1/FVC, PEF, MEF75, MEF50, MEF25) spirometrik ölçümlerle, yaşam kaliteleri ise St. George Solunum Anketi (SGRQ) ile antrenman öncesi ve 2-4 ay sonrası değerlendirildi. İstatistiksel anlamlılık düzeyi p<0,05 olarak kabul edilerek gruplar birbirleriyle ve kendi içlerinde karşılaştırıldı. Katılımcıların egzersiz programı öncesi ve sonrasında solunum fonksiyon testi (SFT) parametreleri karşılaştırıldığında anlamlı bir fark bulunamadı (p>0,05). Egzersiz programı sonrasında hasta grubun yaşam kalitesi anket skorları anlamlı düzeyde azaldı (p<0,001). Sonuç olarak, kronik akciğer sorunu olan hastalarda hafif/orta şiddette alt veya üst ekstremite direnç egzersizlerinin de ev ortamında düzenli olarak yapılması halinde yaşam kalitesinde iyileşme sağlayabileceği görüldü.
