dc.description.abstractErken membran rüptürü, (EMR) enfeksiyon gelişmesi nedeniyle önemli maternal ve neonatal morbidite ve mortalite nedenleri arasında sayılmaktadır. Literatürde EMR'de olası bir enfeksiyona karşı antibiyotik profilaksisi konusunda çelişkili görüşler mevcuttur. Bu nedenle çalışmamızda EMR'de antibiyotik profilaksisinin gerekliliği araştırılmıştır. EMR saptanan 10 preterm ve 10 term kadın kliniğimizde doğum öncesi ve sonrası izlendi. Kan ve servikal kültür olmadıktan sonra profilaklik ampisilin tedavisine başlandı ve postpattum 5. güne kadar devam edildi. Yenidoğanlara da kan ve gastrik aspirat kültürü alındıktan sonra 5 gün süreyle uygulandı. Olası bir enfeksiyona karşı annelerden doğum öncesi, yenidoğanlardan ise 1. ve 21. günlerde hematokrit, hemoglobin, beyaz küre, periferik yayma, serum IgG, IgM, IgA, C-reaktif protein, fibronektin ve asit- a1, glikoprotein düzeyleri için kan örnekleri alınarak değerlendirildi. Bir annede koryoamniyonit saptanırken, bebeklerde herhangi bir enfeksiyon bulgusu gözlenmedi. Tüm olgularda kan kültürü negatif bulundu. Çalışma sonuçlarımız, EMR'de enfeksiyon bulguları saptanmadıkça profilaktik antibiyotik uygulanmasının gerekli olmadığını göstermektedir.tr_TR
dc.description.abstractPROM is considered to be one the major causes of maternal and neonatal morbidity and mortality. There is so much conflict in recent studies on antibiotic prophylaxis for the possible infection in PROM. For that reason we studied on the necessity of antibiotic prophylaxis in PROM. We have folloved 10 preterm and term pregnancies with PROM before and after delivery. We have began prophylactic antibiotic ampicillin therapy after obtaining blood and cervical cultures and continued till the fifth postpartum day. Ampicillin has also been administered to the newborns for five days after obtaining blood and gastric aspiration cultures. For the possibility of an infection, blood samples were obtained from the mothers before delivery and the newborns on the first and 21 st. postpartum days for the assesment of hematocrit, hemoglobin, leucocyte serum IgG, IgM, lgA and C-reactive protein, fibronectin and acid- a1, glycoprotein levels. Chorioamnionitis has developed in one case, while no any other sign of infection has been observed on newborns, Blood cultures have found to be negative in all cases. We have concluded that proplıylactic antibiotic therapy had not been indicated in PROM unless signs of infection had been seen.en_US
