dc.description.abstractLaparoskopik Nissen fundoplikasyonu (LNF) gastroözofageal reflü (GÖR) semptomlarının tedavisindeki etkinliği pek çok çalışmada kanıtlanmış ve günümüzde birçok merkezde yaygın olarak kullanılan bir tedavi prosedürüdür. LNF sonrası etkinlik; hastaların şikayetleri, laboratuar incelemeleri ve sağlıkla ilişkili yaşam kalitesi ölçekleri ile değerlendirilir. Literatürde bu ölçeklerin çocukluk çağında birebir uygulaması yayınlanmamıştır. Bu tezin amacı, gastroözofageal reflü (GÖRH) nedeniyle yapılan LNF uzun dönem sonuçlarını değerlendirmede; sübjektif kriterlere dayanan yaşam kalitesi ölçeğinin çocuk hastalarda da uygulanabilirliğini araştırmaktır. Çalışmaya Mart 2003-Ekim 2009 tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi Çocuk Cerrahisi Anabilim Dalı'nda GÖRH nedeniyle LNF yapılan 29 hasta ve kontrol grubu olarak 30 hasta dahil edildi. Hasta ve ebeveynlerine klinik doktoru tarafından "GÖRH'na spesifik yaşam kalitesi değerlendirme ölçeği" (GERD-HRQL) ve "gastrointestinal sistem yaşam kalitesi değerlendirme ölçeği" (GIÖLI) olmak üzere 2 anketin soruları yöneltildi. Hasta ve ebeveynleri anket formlarını hastanede doldurdular. Çalışmanın veri analizinde SPSS 13.0 (Statistical Package for Social Sciences, Chicago, İL) istatistik programı kullanıldı. P < 0.05 istatistiksel olarak anlamlı kabul edildi. Elde ettiğimiz sonuçlara göre GERD-HRQL ölçeği ve GIÖLl'nin çocukluk çağı LNF'nin etkinliğini değerlendirmede; yapılacak bazı modifikasyonlar ile uygun olacağı düşünülmektedir. Bu yüzden bu anketlerin çocuklara uyumluluğun tamamlanması için daha fazla çalışmalara ihtiyaç vardır.tr_TR
dc.description.abstractThe efficacy of Laparoscopic Nissen fudoplication (LNF) in treatment of gastroesophageal symotoms has been proven in many studies and today LNF is a treatment procedure which is widely used in many centers. The efficacy of LNF is evaluated by patients complaints, laboratory examinations and health-related quality of life index (HRQL). The application of these scales for children has not been presented yet. The aim of this thesis is to evaluate the applicability of HRQL which is based on subjective criteria in pediatric patients, for long-term results of LNF performed in treatment of gastroesophageal reflux disease (GERD). In this study 29 children with GERD who were operated upon by LNF in Uludag Universty Medical Faculty Pediatric Surgery Department in the period between March 2003-0ctober 2009 and 30 children as contol group were included. The questions of both Gastroesophageal Reflux Disease-Health Releated Quality of life (GERD-HRQL) and Gastrointestinal System Quality of Life questionares (GIQLI) were asked to patients and their parents by the clinic doctor. Patients and their parents replied the questionare in the hospital. The SPSS 13.0 (Statistical Package for Social Sciences, Chicago, IL) statistical program was used in the data analysis of the study (p<0.05 was accepted as statistically significant). According to our out comes; in the evaluation of efficacy of LNF in childhood GERD-HRQL and GIQLI questionares are considered to be convenient with some modifications. Therefore, to achieve a questionare with 100% convinience for children, we need further studies.en_US
