Abstract

Çalışmanın amacı kronik hipoksinin endokrinolojik yan etkilerini incelemektir. Kronik hipoksi modeli olarak siyanotik konjenital kalp hastalığı (KKH) olan çocuklar çalışmaya alınmıştır. Hasta ve Yöntem: Çocuk Kardiyoloji kliniğine başvuran ve siyanotik KKH tanısı alan 30 olgu hasta grubu ve çocuk genel polikliniğine başvuran 35 olgu kontrol grubu olarak değerlendirildi. Bulgular: Hasta grubunda 16 erkek (%53,3), 14 kız (%46,7) varken, kontrol grubunda 22 erkek (%62,9), 13 kız (%37,1) vardı. Yaş ortalaması hasta grubunda 4,37±4,51 (0,51-15,29) yıl iken kontrol grubunda 4,28±4,96 (0,71-16,78) yıldı. Gruplar arasında yaş ve cinsiyet olarak fark yoktu. Hastaların boy ve ağırlık SDS'si kontrol grubuna göre düşüktü. Hasta grubun O2 satürasyon ortalaması %76,7±6,27 saptandı. Glukoz değeri hasta grubunda 75 mg/dl, kontrol grubunda 83 mg/dl (p=0,033) idi. IGF-1 ve IGF SDS hasta grubunda belirgin (p=0,010) düşüktü. ACTH, kortizol düzeylerinde iki grup arasında fark yoktu. Altı hastada ACTH düzeyi çok düşük saptandı. DEXA z skoru -0,934 idi. Oksijen satürasyon derecesi ACTH ile pozitif (p=0,041, r=0,439), HOMA-IR (p=0,046, r=-0,420) ve insülin (p=0,017, r=-0,494) ile negatif korelasyon gösterdi. Trombosit sayısı hasta grupta daha düşüktü (p=0,022). Hematokrit, glukoz ile negatif (p=0,016, r=-0,458), ürik asit ile pozitif (p=0,037, r=0,542) koreleydi. Sonuç: Kronik hipoksi siyanotik KKH olan çocuklarda beslenme yetersizliği yanısıra IGF-1 değerini azaltarak büyüme üzerine olumsuz etki yapmıştır. Hipofiz-adrenal aks etkilenmiş olabilirken, hipofiz-tiroid aksında değişiklik gözlenmemiştir. Hipoksi kemik mineral yoğunluğunu azaltmış ancak serum Ca, P, ALP üzerine etkisi olmamıştır. Serum glukoz düzeyinde azalma gözlenmiş, insülin düzeyi ve insülin direncinde değişiklik saptanmamıştır. Siyanotik KKH olan hastalarda polisitemi ve hiperviskozite yanında, trombosit sayısı düşüklüğü gözlenmiştir.
The aim of this study is to investigate endocrine side effects of chronic hypoxia. Methods: Thirty patients with cyanotic congenital heart disease (CHD) and thirty-five healthy controls were enrolled in the study. Results: There were 16 boys (53.3%) and 14 girls (46.7%) in the patient group, and 22 boys (62.9%) and 13 girls (37.1%) in the control group. Mean age was 4.37±4.51 (0.51-15.29) years in the patient group and 4.28±4.96 (0.71-16.78) years in the control group. There was no statistical significance between the two groups as per sex and age. Height and weight SDS were significantly lower in the patient group. Mean oxygen saturation was 76.7±6.27%. Serum glucose levels were 75 and 83 mg/dl in the patient and control group, respectively (p=0.033). While IGF-1 levels and IGF-1 SDS were lower in the patient group (p=0.010), there was no statistical significance between the two groups in term of ACTH and cortisol levels. ACTH levels were very low in 6 patients. DEXA z-score was -0.934. There were positive correlation between oxygen saturation and ACTH levels (p=0.041, r=0.439), negative correlation between oxygen saturation and HOMA-IR (p=0.046, r=-0.420) and insulin levels (p=0.017, r=-0.494,). Thrombocyte count was low in the patient group (p=0.020). There were negative correlation between hematocrit and glucose levels (p=0.016, r=-0.458)) and positive correlation between hematocrit and uric acid levels (p=0.037, r=0.542). Conclusions: Chronic hypoxia has a negative impact on growth by reducing the IGF-1 levels and causing nutritional deficiency in the patients with cyanotic CHD. While hypophyseal-adrenal axis might have been affected, hypophyseal-thyroid axis was intact. Bone mineral density decreased, but serum Ca, P and ALP levels were normal. While serum glucose levels decreased, insulin levels and insulin sensitivity was not changed. Besides polycythemia and hyperviscosity, thrombocyte counts decreased in the patients with cyanotic CHD.
