Bortezomib ve lenalidomid tedavilerini alan multipl miyelom olgularının retrospektif incelenmesi

Abstract

Multipl Miyelom (MM) kemik iliğinde tek bir klondan köken alan malign plazma hücrelerinin kontrolsüz artışı sonucu gelişen hematolojik bir malignitedir. Tam şifa sağlayan herhangi bir tedavi seçeneği bulunmasada özellikle son 10 yılda gündeme gelen yeni ajan tedavileri ile güzel sonuçlar elde edilmektedir. Bu çalışmamızda Uludağ Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı Polikliniği'nde Nisan 2009-Haziran 2014 tarihleri arasında otolog kemik iliği nakil adayı genç veya 65 yaş üstü olgularda yeni tanı ve nüks hastalık durumunda uygulanan yeni ajanlardan bortezomib ve lenalidomid içeren tedavi protokollerini alan 72 erişkin hasta yer aldı. Hastaların ortanca yaşı 61 yıl (34-83) olup erkeklerin oranı kadınlardan bir miktar daha yüksekti (%54). Hastaların %46'sına Otolog Kemik İliği Transplantasyonu (OKİT) yapılmış idi. OKİT yapılan olguların %85'i 65 yaş ve altı olgular idi. Yeni ajan tedavilerini alan 65 yaş ve altı hastaların %21'i Lenalidomid/Len-Dex (Lenalidomid, Dexametazon), %47'si Bor-Dex (Bortezomib, Dexametazon), %32'si VRD (Bortezomib, Lenalidomid, Dexametazon) almış olup 65 yaş üstü hastaların %36'si Lenalidomid/Len-Dex, %48'i Bor-Dex, %16'sı VRD almıştı. Bor-Dex tedavisi alan 72 hastada tedavi sonunda %1'inde mükemmel tam yanıt, %32'sinde tam yanıt, %15'inde çok iyi kısmi yanıt, %11'inde kısmi yanıt, %31'inde stabil hastalık, %5'inde progresif hastalık saptandı. Lenalidomid/Len-Dex tedavisi alan 43 hastada ise %21'inde tam yanıt, %21'inde çok iyi kısmi yanıt, %5'inde kısmi yanıt, %16'sında stabil hastalık, %9'unda progresif hastalık görülürken VRD tedavisi alan 39 hastada ise tedavi sonunda %3'ünde mükemmel tam yanıt, %18'inde tam yanıt, %38'inde çok iyi kısmi yanıt, %18'inde stabil hastalık saptandı. Sonuç olarak; yeni ajan tedavileri gerek etkinlikleri gerek yan etkileri göz önüne alındığında transplanta uygun veya uygun olmayan hastalarda yeni tanı ve nüks hastalık durumunda uygulanabilen etkili ve güvenli tedavi seçenekleridir.
Multiple myeloma is a hematologic malignant disease derived from the uncontrolled increase of monoclonal malignant plasma cells. Although there is no curative treatment; good results have been achieved by using new chemotherapeutic treatment agents past ten years. Our study includes total 72 patients, young and over 65 years old, whom received bortezomib and lenalidomide including treatment protocols for in case of newly diagnosed and relapsed disease between April 2009 - June 2014 in the Uludağ University Faculty of Medicine Haematology Department. The median age of patients' was 61 years (34-83) and number of male patients was a little bit higher than women (54%).Forty six percent of patients underwent otolog stem cell transplantation. Eighty five percent of patients who had otolog stem cell transplantation were 65 years old or younger. The patients who are 65 years old and younger who had new agent therapies, 21% of them had lenalidomide/Len-Dex (Lenalidomide, Dexamethasone), 47% of them had Bor-Dex (Bortezomib, Dexamethasone), 32% of them had VRD (Bortezomib, Lenalidomide, Dexamethasone) treatment. The patients who are older than 65 years old, %36 of them had Len-Dex, 48% of them had Bor-Dex, 16% of them had VRD treatment. When we look at the outcome of 72 patients treated with Bor-Dex; 1% perfect complete response, 32% complete response, 15% very good partial response, 11% partial response, 31% stable disease and 5% progressive disease rate was observed. In 43 patients, who had Lenalidomide/ Len-Dex therapy, 21% complete response, 21% very good partial response, 5% partial response, 16% stable disease and 9% progressive disease rate were observed. Another 39 patients, who received VRD treatment,; 3% perfect vi complete response, 18% complete response, 38% very good partial response and 18% stable disease rate was found. As a result; new agent therapies are efficient and safe treatment options for patients, who are suitable for transplantation or not, when their efficiency and side effects considered.
