Abstract

Endokannabinoid reseptörleri, CB1 ve CB2'dir. Günümüzdeki sınırlı sayıdaki insan çalışmaları kannabinoidler ile non-alkolik yağlı karaciğer hastalıkları (NAYKH) gelişimi arasındaki ilişkiyi göstermektedir. CB1 reseptörlerinin stimulasyonuyla yağ metabolizmasında anahtar rol oynayan genlerin ekpresyonu ve karaciğer yağ asidi sentezi artar. CB1 reseptör antagonisti olan Rimonabant'ın (SR141716) kullanılması ile hepatik steatozun ve hepatomegalinin gerilediği gösterilmiştir. CB2 reseptörlerinin yağlı karaciğerde expresyonu metabolik sendromla olan ilişkisini göstermektedir. CB1 reseptörleri profibrogenik, CB2 reseptörleri ise antifibrogeniktir. Endokannabinoid sistemin, periferik dokulardaki metabolik fonksiyonları aktive etmesi, çeşitli hastalıkların tedavisinde kullanılabileceği fikrini geliştirmektedir.Çalışmamızda Non-alkolik Steatohepatitli hastalarda (NASH) ve normal insan karaciğer dokusunda CB2 ve CB1 reseptör ekspresyonu belirlenip, bu reseptörlerin karaciğer yağlanması ve metabolik sendromun paremetreleriyle ilişkisi araştırılmıştır. On yedisi Steatozlu, 54'ü NASH'lı toplam 71 NAYKH'lı hasta ve kontrol grubunda 40 normal karaciğer örneğinin CB2 ve CB1 reseptör ekspresyonları değerlendirilmiştir. NASH grubundaki hastalar ve steatoz grubundaki hastaların CB2-R ekspresyon yüzdesi ile NAS skorunu oluşturan paremetrelerle herhangi bir ilişki saptanmazken (p>0.05), NAYKH'lı hastalar topluca değerlendirildiğinde CB2 R ekspresyonu ile lobüler inflamasyon arasında ilişki saptandı. NASH grubundaki hastaların sistolik ve diyastolik arteryal tansiyon değerleri, HDL ve mikroalbuminürinin CB2-R ekspresyonuyla negatif yönde korelasyonu saptandı. Obez steatozlu hastaların CB2 R ekspresyonunun, obez NASH'lı hastalara göre daha fazla oranda olduğu saptandı. NAYKH grubundaki ve kontrol grubundaki hiç bir olguda CB1-R'leri nükleer boyanmadı veya çok hafif stoplazmik boyanma saptandı. Ancak CB2 R'lerinde tüm hasta ve kontrol karaciğer doku örneklerinde pozitif olarak immünhistokimyasal boyanma gözlendi.Sonuç olarak endokannabinoid sistemin yağlı karaciğer hastalığının patogenezindeki ve tedavi seçeneklerindeki rollerinin belirlenmesi için daha ileri çalışmalara gerek vardır.
CB1 and CB2 are endocannabinoid receptors. A limited number of human studies shows the relationship between the cannabinoids and the development of the non-alcoholic fatty liver disease (NAFLD). By the stimulation of the CB1 receptors, the expression of the genes that play a key role in fat metabolism and the hepatic fatty acid synthesis increases. The use of rimonabant (SR141716), which is a CB1 receptor antagonist, has been shown to reduce the hepatic steatosis, and hepatomegaly. CB2 receptor expression in fatty livers shows its relationship with the metabolic syndrome. The CB1 receptors are profibrogenic, whereas the CB2 receptors are antifibrogenic. The endocannabinoid system activates metabolic functions in the peripheral tissues. This suggests that this system can play a role in the treatment of various diseases.In our study, we have identified the CB1 and CB2 receptor expression in patients with non-alcoholic steatohepatitis (NASH) and in normal human liver tissue, and investigated the relationship of these receptors with steatohepatitis and the parameters of the metabolic syndrome. A total of 71 patients, 54 of whom have NASH and 17 with steatosis, and 40 patients with healthy livers in the control group, were evaluated in terms of CB2 and CB1 receptor expression. There was no relationship between the patients in the NASH group and the patients with steatosis in terms of the percentage of CB2 receptor expression (p>0.05), whereas, when the patients with NASH were evaluated as a whole, there was a relationship between the CB2 receptor expression and lobular inflammation. In the NASH group, the systolic and the diastolic arterial blood pressure values, HDL cholesterol and microalbuminuria had a negative correlation with CB2 receptor expression. The CB2 receptor expression was more likely in obese patients with steatosis than in obese patients with NASH. As far as the CB1 receptors were concerned, there were no cases with nuclear staining in the NASH and in the control group, or there was a weak cytoplasmic staining. However, in all patient and control liver tissue samples, there was a positive immunohistochemical staining of the CB2 receptors.In conclusion, in order to determine the role of the endocannabinoid system in the pathogenesis and the treatment options of fatty liver disease, further studies are needed.
