Abstract

Epilepsi, aileleri ve hekimleri sıkıntıya düşüren tüm yaş gruplarını etkileyen ve çocukluk çağının sık rastlanan kronik nörolojik hastalığıdır. Epilepsi tedavisi, nöbetlerin durdurulmasını veya kontrolünün sağlanmasını ile aynı zamanda uygun maliyetli yaklaşımı ve ilaçların yan etkilerinin önlenmesini kapsamaktadır. Günümüzde de çok sayıda ilacın varlığına ve iyi tıbbi bakıma rağmen epileptik hastaların önemli bir bölümü nöbetlerden kurtulmuş değildir. Pratikte bu dirençli olgular da bazen ilacın yüksek dozda kullanımı tercih edilmekte veya birkaç ilaç beraber kullanılmakta bunların da yan etkisi fazla olmaktadır. Bu da hastanın yaşam kalitesini azaltmakta, maliyeti arttırmakta ve yüksek morbiditeye yol açabilmektedir. Çocuklarda aktif olarak kullanılan bu ilaçlara bağlı; santral sinir sistemi, retiküloendoteliyal sistem ve kemik iliği, gastrointestinal sistem, hepatobiliyer sistem, deri ve kutanöz dokular, motor hareket bozuklukları, uyku bozukluğu, duygu durum ve ruh hali değişikliği, allerjik ve kozmetik birçok yan etkiler olduğu bilinmektedir. Bu çalışmamızın amacı; epilepsili çocuklarda yeni kuşak antiepileptik tedavilerin klinik etkinliği, eski ve yeni kuşak antiepileptiklerin monoterapi ve politerapi olarak karşılaştırılması ve çocuklarda yeni kuşak antiepileptiklerin klinik ve labaratuar yan etkilerinin gözden geçirilmesidir. Çocuk Nöroloji Bilim Dalı polikliniğinden 01.01.2007 - 01.01.2009 tarihleri arasında epilepsi tanısıyla izlenen 630 hastanın dosyaları retrospektif olarak incelendi. Yeni kuşak antiepileptik ilaç ile kombine tedavi alan 309 hasta ile tek bir eski kuşak antiepileptik ilaç ile monoterapi alan 181 hasta olmak üzere toplam 490 hasta çalışmaya alındı. Hastalarımızın 274'ü (%55,9) generalize epilepsi, 170'i (%34,6) parsiyel epilepsi, 36'sı (%7,3) epileptik sendrom ve 10'u (%2) sınıflandırılamayan epilepsi tipine sahipti. Hastaların demografik ve karekteristik özellikleri, muayene bulguları, labaratuar ve radyolojik tetkik sonuçları incelenerek, dosyalardaki kayıtlardan, hastalar ve ailelerinden tedavilerin etkinliği ve yan etkileri hakkında bilgiler alındı. Bu iki grubun verileri için değişkenler arasındaki ilişkiler SPSS 16.0 istatistik programı kullanarak incelendi.Hastalarımızın (180 hasta) % 36.7'sinde klinik yan etki gelişti. Bunların 117'si eski kuşak antiepileptik tedavi, 63'ü ise yeni kuşak antiepileptik tedavi alırken gelişti. Sistemik ve lokal yan etkiler arasında, MSS'ne ait yan etkiler 108 hastada (%22) gelişti. Diğer sık rastlanan yan etkiler ise gastrointestinal sistem (%16.7) yan etkileri ile ruh hali ve davranış değişiklikleri (%16.5) ve deri allerjik yan etkileri (%15.5) idi. Valproik asit kullanan hastalarımızda %24.3, karbamazepin kullananlarda %25.7, fenobarbital ile %23.4, fenitoin ile %20 gibi benzer oranlarda yan etki gelişirken, yeni kuşak antiepileptiklerden; lamotrijine bağlı %22.6, topiramata bağlı %15.8, levetirasetama bağlı %22.8 ve vigabatrine bağlı %23.8 oranında yan etki geliştiği gözlemlendi. Ayrıca eski kuşak antiepileptiklerin (26.6±13.6 ay) kullanılma süreleri yeni kuşak AEİ kullanılma (18.1±11.4 ay) süresine göre uzun ve istatistiksel anlamlı idi (p:0.042). İlaçların epilepsi tedavisindeki etkinlikleri; eski kuşak antiepileptiklerden valproik asit ile %71.5, fenobarbital ile %67.8, fenitoin ile %64.9 ve karbamazepin ile de %62.7 oranında çok iyi nöbet kontrolü (%75 ve üzerinde nöbet kontrolü) sağlanırken, yeni kuşak antiepileptiklerden; topiramat ile (%59.6), lamotrijin ile (%57.1), vigabatrin ile %52.3 ve levetirasetam ile %49.9 oranında çok iyi nöbet kontrolü (%75 ve üzerinde nöbet kontrolü) elde edildi.Çalışmamızın sonunda yeni kuşak antiepileptiklerin klinik uygulama süreleri eski kuşak antiepileptik ilaçlara göre daha kısa olduğunu görmekteyiz. Belki bu nedenle yan etki potansiyellerini düşük saptadık. Ancak uzun vadeli yan etkilerinin artacağını düşünüyoruz ve bu yönden kıyaslanmalarının şimdilik zor olduğunu söyleyebiliriz.
Epilepsy is a neurological disease common among children as well as all age group and put strains on both families and doctors. Epilepsy treatment aims at preventing or controlling the seizures besides applying a convenient cost while reducing the side effects of the drugs. Most of the patients with epilepsy still suffer from seizures despite a wide variety of the antiepileptic drugs and proper medical care.In serious cases, the preferred method is sometimes to increase the dosage or use several drugs simultaneously in practice thus resulting more side effects. As a result, the quality of the patient?s life spoils, the cost of the treatment increases and therefore high morbidity may be resulted. The adverse effects of these drugs used by children involve disturbance of central nerve system, reticuloendothelial system and bone marrow, gastrointestinal system, hepatobiliary system, skin and cutaneum tissue, disturbance of motor system, sleep disorder, mood disorder and emotional state alterations, allergic and cosmetics side effects.This study will evaluate the clinical effectiveness of the antiepileptic treatments of children with epilepsy, the comparison of new and old types of antiepileptic drugs on the basis of monotherapy and polytherapy, and monitoring the clinical and laboratory side effects of new antiepileptic drugs used by the children.Between 01.01.2007 and 01.01.2009, the files of 630 patients with epilepsy in Department of Child Neurology were studied retrospectively. 409 patients were analyzed and 309 of them used new antiepileptic drugs with combined treatment while 181 of those used one type of antiepileptic drug with monotherapy. 274 (55.9%) patients suffered from generalized epilepsy, 170 (34.6%) partial epilepsy, 36 (7.3%) epileptic syndrome, and 10 (2%) of them suffered from unknown epilepsy type. We analyzed the demographic and characteristics features, findings resulted from examinations, the results of laboratory and radiology tests of the patients and we obtained data related to the effectiveness of the treatment and side effects from the families of the patients and thorough their files. SPSS 16.0 was administered to analyze the relations between the variables. 36.7% (180 patients) of the patients developed clinical side effects. While 117 patients using old antiepileptic treatment developed side effects, side effects occurred in 63 patients with new antiepileptic drug treatment. Side effects of MSS developed in 108 (22%) among systemic and local side effects. Gastrointestinal side effects (16.7%), side effects regarding alterations of emotional and behavioral states (16.5%) and allergic side effects on skin (15.5%) consist of other common effects. It was observed that 24.3% side effects occurred in patients using valproik acid, 25.7% in patients using karbamazepin, 23.4% in those using fenobarbital and 20% in those using fenitoin and among new antiepileptic drugs lamotrijine developed 22.6% side effects, topiramat 15.8%, levetirasetama 22.8% and vigabatrine 23.8% respectively. Also, the amount of time with respect to old antiepileptic drugs (26.6±13.6 months) was significant and longer statistically when compared to that of new AEI drugs (18.1±11.4 months) (p:0.042). The effectiveness of the drugs in the treatment is as following; among old antiepileptic drugs, valporik acid provided 71.5%, fenobarbital 67.8%, fenitoin 64.9%, karbamazepin 62.7%, resulting in an effective seizure control (75% and over seizure control); on the other hand, among new antiepileptic drugs, topiramat did 59.6%, lamotrijin 57.1%, vigabatrin 52.3%, levetirasetam 49.9%, resulting in controlling the seizure effectively (75% and over seizure control).We can conclude that evaluation of comparison related to the long term side effects of old and new antiepileptic drugs is difficult at present although it looks that new antiepileptic drugs has little side effects since the clinical administration time of the new drugs is shorter when compared to that of old drugs.
