Abstract

Modern tıptaki yeni tedavi gelişmelerine bağlı olarak, 1980'lerin başlarından itibaren özellikle yoğun bakım ve immün sistemi baskılanmış hastaların artan oranda ciddi mantar enfeksiyonlarına maruz kalması; amfoterisin B dışında yeni antifungallerin gündeme gelmesi ve antifungal direncin de görülmeye başlaması nedeniyle in vitro antifungal duyarlılık testleri giderek önem kazanmaya başlamıştır. Bu çalışma, 1995-2005 yılları arasında Uludağ Üniversitesi Sağlık Uygulama ve Araştırma Merkezi'nde yatan hastalardan izole edilen Candida izolatlarının antifungal duyarlılığını belirlemek, epidemiyolojik veri sağlamak ve bu bilgiler doğrultusunda ampirik tedaviye ışık tutmak amacıyla yapılmıştır.Çalışmada, kan ve steril vücut sıvılarından izole edilen 444 Candida izolatının antifungal duyarlılığına bakılmış ve amfoterisin B, flukonazol, itrakonazol, vorikonazol ve kaspofungin duyarlılıkları referans CLSI M27-A3 mikrodilüsyon yöntemi ve E-test yöntemi ile saptanmıştır. Değerlendirme, 35oC'da hem 24 hem de 48 saat inkübasyon sonunda gözle yapılmış ve mikrodilüsyon yönteminde, amfoterisin B için üremeni inhibe edildiği son kuyucuk, diğer ilaçlarda ise üremenin kontrol kuyucuğuna göre %50 azladığı son kuyucuk minimal inhibitör konsantrasyon (MİK) olarak alınmıştır. E-test yönteminde ise inhibisyon zonunun antifungal strip ile birleştiği nokta MİK olarak değerlendirilmiştir. Amfoterisin B duyarlılığında sınır değerler belli olmadığından MİK aralığı, MİK50 ve MİK90 değerleri saptanmış, diğer ilaçlarda ise sınır değerler belli olduğundan duyarlı ve dirençli suşların yüzde oranları hesaplanmıştır. İki yöntem arasındaki uyuma ise (±) dilüsyonda bakılarak uyum yüzdesi belirlenmiştir.Duyarlılık sonuçları her iki yöntemde de hem 24. saat hem de 48. saatte okunabilmiş ve 48. saat okumasında hafif bir yükselme dışında fazla bir değişiklik olmadığı görülmüştür.Amfoterisin B'de E-test ile referans yönteme göre daha geniş bir MİK aralığı elde edilmiş ve >1?g/ml MİK değerleri sadece C. glabrata ve C. krusei suşlarında saptanmıştır. Azol grubunda en etkili ilacın vorikonazol, en az etkili ilacın itrakonazol olduğu görülmüştür. Flukonazol direnci %1.2 olarak bulunmuş ve flukonazole dirençli beş suştan (iki C. albicans, iki C. glabrata ve bir C. parapsilosis) sadece bir C. glabrata suşunun vorikonazole de dirençli olduğu saptanmıştır. Tüm suşlar arasında kaspofungine duyarlı olmayan sadece bir C. guilliermondii suşu tespit edilmiştir. İki yöntem arasında, referans yöntemle dirençli iken E-test yöntemi ile duyarlı büyük uyumsuzluk olmamış; E-test ile dirençli, referans yöntemle duyarlı küçük uyumsuzluk sadece flukonazol ve itrakonazolde görülmüş, vorikonazol ve kaspofunginde saptanmamıştır.Sonuç olarak bu çalışmada; 10 yıllık bir süreçte izole edilen Candida suşlarında duyarlılığa bakılarak epidemiyolojik veri elde edilerek, hastanemizde kandidemilerde ilk sırada kullanılan flukonazole, totalde direncin yüksek olmadığı, vorikonazol ve kaspofunginin en etkili ilaçlar olduğu ve amfoterisin B duyarlılığında seçilmesi gereken yöntemin E-test olduğu belirlenmiştir.
Depending on the new treatment developments in modern medicine, upon particularly the patients who were under intensive care and whose immune system was oppressed being exposed to serious fungal infections in increasing proportions as of the early 1980?s, in vitro antifungal sensitivity tests started to be more significant due to the facts that new antifungals were brought to the agenda in addition to amphotericine B and that antifungal resistance started to emerge as well. This study was performed for the purpose of determining the antifungal sensitivity of Candida isolates that were isolated in the patients who stayed in Uludağ University Health Practice and Research Center from 1995 to 2005, to provide epidemiological data and to set light to empiric treatment on the basis of this information.In the study antifungal sensitivity of 444 Candida isolates isolated from blood and sterile body fluids was examined and the sensitivities of amphotericine B, fluconazole, itraconazole, voriconazole and caspofungin were detected using reference CLSI M27-A3 microdilution method and the method of E-test. The assessment was performed at 35oC with eye both at the end of 24 hour and 48 hour incubation and in the micro dilution method, the last tiny borehole where the reproduction for amphotericine B was inhibited and in other medicines the last tiny borehole where reproduction decreased by 50% compared to the control tiny borehole was taken as minimal inhibitor concentration (MIC). And in the method of E-test the point on which the inhibition zone combined with the antifungal strip was assessed as MIC. Since the limit values were not definite in amphotericine B, MIC range, MIC50 and MIC90 values were detected and since limit values were definite in the other medicines the percent proportions of the sensitive and resistant strains were calculated. And the harmony between the two methods was examined in (±) dilution and the harmony percentage was determined on this basis.In both methods the results of sensitivity could be read in both 24th hour and 48th hour and in the reading of the 48th hour it was understood that there was no significant change other than a slight increase.In amphotericine B a broader MIC range was acquired compared to the method referenced with the E-test and >1?g/ml MIC values were only detected in C. glabrata and C. krusei strains. It was understood that the most effective medicine in the azole group was voriconazole, and the least effective medicine was itraconazole. The resistance of fluconazole was found to be 1.2% and it was detected that only a C. glabrata strain among the five strains resistant against fluconazole (two C. albicans, two C. glabrata and a C. parapsilosis) was resistant against voriconazole as well. Among all the strains only one C. guilliermondii strain was detected to be not sensitive against caspofungine. While it was resistant with the reference method from the two methods and it was sensitive with the E-test method and no great disharmony emerged and the small disharmony between resistance with the E-test and sensitivity with the reference method was only the case with fluconazole and itraconazole and it was not detected in voriconazole and caspofungin.As a consequence, in this study it was determined by acquiring epidemiological data on the basis of the sensitivity in Candida strains isolated for a period of 10 years that the resistance against fluconazole used at first rank in our hospital was not high in total and that voriconazole and caspofungin were the most effective medicines and that the method to be selected for B sensitivity of amphotericine was E-test.
