Abstract

Özofagus webleri lümeni tam veya yarım halka şeklinde daraltan, mukoza ve submukozadan oluşan ince, şeffaf, membranöz yapılardır. Çalışmamızda web saptanan hastaların demografik özelliklerinin, anemi parametrelerinin, eşlik eden hastalıklarının, endoskopik bulgularının, semptom süresinin, web bölgesinin, tanıda özofagografi kullanımının, uygulanan tedavilerin ve Plummer Vinson Sendromu (PVS) görülme sıklığının değerlendirilmesini amaçladık. Çalışmamıza 01 Ocak 2009-31 Mart 2016 tarihleri arasında Uludağ Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Gastroenteroloji Polikliniğine başvuran ve özofagial web saptanan 68 hasta dâhil edildi. Hastaların demografik özellikleri, klinik ve laboratuvar bulguları elektronik ortamda hastane bilgi yönetim sistemi ile retrospektif olarak incelendi. Çalışmaya dâhil edilen hastaların yaş ortalaması 50,97 (18-89) olup, hastaların %89,7'si kadın (n=61), %10,3'ü erkekti (n=7). Hastaların %91,8'inde (n=62) disfaji mevcuttu. Yapılan endoskopilerin %85,3'ünde (n=58) tek web, %14,7'sinde (n=10) multiple web saptandı. Weblerin %89,7'si (n=61) proksimal özofagustaydı. Tedaviyi kabul eden 65 hastanın %81,5'ine (n=53) buji dilatasyon uygulandı. %18,5'inin (n=12) ise webleri endoskop ile yırtıldı. Başlangıçta dilatasyon yapılan 53 hastanın %47,2'sine (n=25) tekrar dilatasyon uygulandı. 26 hastada PVS triadı (disfaji, demir eksikliği anemisi ve proksimal özofagusta web) vardı. Çalışmamızda hastaların yaş ve cinsiyet dağılımı, saptanan ek hastalıkları literatür ile benzerdi. Hastaların yaşı arttıkça multiple web saptanma ve proksimal dışı web saptanma sıklığının arttığı görüldü. 1 hastada PVS tanısı konulduktan 3 yıl sonra özofagus skuamöz hücreli kanseri saptandı.
The esophagus webs are thin, transparent, membranous structures consisting of mucosa and submucosa, narrowing the lumen to full or half ring. In our study, we aimed to evaluate the demographics, anemia parameters, comorbidities, endoscopic findings, symptom duration, web site, esophagography, treatment and frequency of Plummer Vinson Syndrome (PVS) in the patients identified on the web. The study included 68 patients admitted to the Gastroenterology Depertmant of Uludag University Health Practice and Research Center between January 1, 2011 and March 31, 2016 and were detected esophageal webs. The patients' demographics as well as their clinical and laboratory findings were scanned retrospectively from medicalelectronic files. The mean age of the patients included in the study was 50,97 (18-89) and 89,7% of the patients were female (n=61) and 10,3% were male (n=7). Dysphagia was present in 91,8% of the patients (n=62). Single web was found in 85,3% (n=58) and multiple webs were found in 14,7% (n=10) of endoscopies. 89,7% (n=61) of the webs were in the proximal esophagus. Bougie dilatation was applied to 81,5% (n=53) of 65 patients who accepted treatment. 47,2% (n=25) of these 53 patients were re-dilated. 26 patients have triad of PVS (dysphagia, anemia of iron deficiency and web of proximal esophagus). In our study, the age and gender distribution of patients and comorbities were consistent with the literature. As the age of the patients increased, the frequency of multiple web detection and non-proximal web detection increased. Esophageal squamous cell carcinoma was detected 3 years after the PVS diagnosis in 1 patient.
